Sliced ham stored above the load line is not at 41 degrees or below. Several items in cooling units not date-marked as needed.

Noncritical violations: Food workers shall wear hair and/or beard restraints. Exterior of the bulk bins (pizza area) have an accumulation of grease/dirt; clean frequently as needed to prevent the accumulation of grease.

Pinnacle Golf and Country Club

Critical violations: Employee did not lather with soap long enough prior to rinsing off soap; lather for 10-15 seconds prior to rinsing. Employee handled raw meat and just removed gloves but did not wash hands before starting new task; employee touched face and did not wash hands before going back to food prep. Employee handled ready to eat food with bare hands. Nacho beef in cooler No. 4 still 93 degrees after 3.5 hours; food is to be reheated and cooled properly in the correct amount of time or destroyed. Butter in bakery cooler is 45 degrees and half and half is 43 degrees; check unit to ensure it is working properly as the doors had been open stocking some items.

Noncritical violations: Food on display shall be protected from contamination from consumers; no sneeze shield on the dessert end of the buffet. Food workers shall wear hair and beard restraints. Excessive condensation build up on exhaust hood over dish washer and dripping off of exhaust frame (repeat violation; a work order was supposed to have been submitted already during the last inspection).

Critical violations: Employee did not lather with soap long enough before rinsing off the soap; lather for at least 10 seconds before rinsing off soap. Employee did not wash hands before putting on gloves between task change. Different batches of cole slaw, pot pie mix and cole slaw dressing in the walk-in are all at 43 degrees. Walk-in thermometer is reading 39 degrees.

Noncritical violations: Condensation leak in the walk in cooler. Condensation leak in walk in freezer; do not store any open boxes or food items under leaking area (repair within 30 days). Ice build up on freezer door; gasket is still on order from the last inspection conducted on 1/13/16. Heat strip on freezer walk in door is not working allowing the ice to build up so door does not shut properly. Tiles in walk in cooler are chipped. Repair or replace to prevent the accumulation of ice build up on the floor between walk in and freezer. Faucet in the ladies room does not shut off even when it is in the "off" position; replace faucet.

Asian Chef

315 C, U.S. 412 West, Siloam Springs

Critical violations: Employee washed hands at the three compartment sink not the designated handwashing sink. Employee just rinsed off hands with water after handling raw meat before going onto next task. Cooked items or other potentially hazardous foods not used in 24 hours must have a date mark (repeat violation). No time marked on the rice used for sushi, the time the rice is taken out of hot holding and the vinegar is added is the time that needs to be marked on the rice held at room temperature; the rice must be used by or discarded in four hours.

Noncritical violations: Some food stored uncovered in the walk in. Shelves above items are not clean: debris from shelves can fall into food. Knives stored behind an unfinished piece of wood on the wall in the back prep area; wood must be finished, easily cleanable and not absorbent. Cardboard being used to cover the shelves above the prep table and other table; cardboard is not an approved material for food areas and should be removed; clean shelves (repeat violation).
