Benton County restaurant inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

June 8

EZ Mart

801 W. Main St., Gentry Critical violations: Ice guard inside the ice machine is not clean, there is an accumulation of debris on the bottom part of the guard. Walk-in is not holding food at 41 degrees or below, cheese slices were at 46 degrees and hot dogs were at 53 degrees; items were removed and destroyed as needed. Items in the hot case are not marked with time; facility uses time as a control for hot case. Noncritical violations: Countertop around the three-compartment sink is being held together with duct tape, which is not an approved repair method (repeat violation). Wall around the mop sink and back hand sink has a hole in it and areas where the paint is peeling and exposing the sheet rock underneath; area is no longer in good condition and can not be cleaned properly.

Metfield Clubhouse

3 Euston Road, Bella Vista Critical violations: None Noncritical violations: There are chemicals in front of the hand wash sink limiting accessibility. Current permit is not posted.

WACD/Gentry Early Learning Academy

185 Rebecca St., Gentry Critical violations: Employee did not lather with soap long enough before rinsing off the soap; lather for 10 to 15 seconds before rinsing. Items are not date marked as needed. Noncritical violations: Children’s food stored in the refrigerator labeled employee refrigerator.” If this refrigerator is going to be used for children’s items, the unit will need to have a thermometer and separation of employee and facility food. Kitchen employee hair is not effectively restrained. Spray hose at three-compartment sink hangs below the rim of the sink; an air gap must be maintained between the sink and the spray hose. Leak in the sink/drain of the three-compartment sink, therefore it does not hold water; this is the third time this violation has been observed, the situation must be corrected immediately. Paint and wall behind the dish machine is damaged; paint is starting to bubble.

Miss Mary’s

468 Gentry Blvd., Gentry Critical violations: Bag of ham stored under raw meat items. A flat of raw shell eggs is sitting at room temperature; maintain shell eggs at 41 degrees or below. Bag of ham exceeded the use-by date. Noncritical violations: Condensation leak in two different places in the walk-in cooler, leak was dripping on a box of open food, do not store food under the area until the leak is repaired repeat violation). Gasket is torn on the three-door servers cooler. Cove base is missing from dish area; missing floor tile under dishwasher.

Vendor Club

3001 N.E. 11th St., Bentonville Critical violations: No date marking on cut leafy greens or cheese sauce. Noncritical violations: Kitchen approved with two-compartment sink and dish machine, remodel requirements would include a three-compartment sink.

June 9

McDonald’s

467 Gentry Blvd., Gentry Critical violations: Front hand washing sink is not working; service order has been submitted, but it has been nearly two weeks since submitted. Burritos in the upright cooler are at 42 degrees; moved to walk-in cooler for further cooling. Some items are not marked with time stickers. Noncritical violations: Broken thermometer in the milk cooler, thermometer is on order. There is no sanitizer concentration in the wiping cloth bucket.

June 12

Cave Springs Coffee Co.

137 N. Main St., Cave Springs Critical violations: Employees did not lather with soap for the required 10 to 15 seconds prior to rinsing off soap. Employee left preparation area and did not wash hands prior to continuing with food preparation. Employee did not wash hands prior to task change and putting on gloves. Sanitizer is not strong enough. Date marking on dressings exceed the seven day useby date, items mixed in-house cannot exceed a seven day use-by date; ensure that in-house date marking does not exceed the seven day use-by date from the facility where sliced meats are purchased. Noncritical violations: Food workers shall wear hair and beard restraints (repeat violation). Sanitizer for wiping cloths is not strong enough; check sanitizer concentration every time it is made. Test strips for sanitizer are no longer effective, need new test strips. Employee did not rinse soap off the utensils before putting them in sanitizer.

June 13

King Burrito

1120 S. Walton Blvd., Suite 152, Bentonville Critical violations: There was a beverage container at food prep without a lid; discontinue consuming food, use of unapproved beverage containers or use of tobacco in food preparation or warewashing area. Noncritical violations: None

Flying Burrito

1401 S. Walton Blvd., Suite 1, Bentonville Critical violations: An employee was gloving at burrito prep station without washing hands, wash hands any time contamination could have occurred, when changing tasks and as described in the Arkansas rules and regulations. There is no hot water at front hand washing sink. Front hand washing hot water handle requires repair to dispense hot water as needed. Noncritical violations: None

June 14

805 Moberly Lane, Bentonville Critical violations: 100 ppm quaternary ammonia in the three-compartment sink at the coffee bar. Noncritical violations: Items must be stored to prevent contamination; person washing hands at the coffee bar must cross above clean dishes in rack at the end of the sink.

Sunshine Montessori Infant and Toddler Center

2124 W. University St., Siloam Springs Critical violations: Employee handled vegetables with bare hands. Items in refrigerator are not date marked as needed. Medicine in the refrigerator is not being stored according to regulations; medicines belonging to employees or to children in the day care center that require refrigeration and are stored in a food refrigerator shall be stored in a package or container and kept inside a covered, leakproof container that is identified as a container for the storage of medicines. Noncritical violations: Reusing a sherbet container for food storage of leftover chicken salad; do not use containers for food storage.

Thai Cafe Asian Cuisine

808 S. Mount Olive St., Siloam Springs Critical violations: Sanitizer is not strong enough. Food is not being cooled properly. Items are not date marked as needed. Noncritical violations: Live roach was in the paper towel dispenser above the hand washing sink; contact a professional pest control operator and maintain a control plan. Grease and food accumulation on the stove, fryer and other equipment. Pass through door between kitchen and dining room is broken and not clean; replace and maintain in clean condition.

There is a gap in the weather stripping in the back door of the kitchen; no door sweep on Lookout Lounge exterior door.

Cathy’s Corner

18446 Highway 16 E., Siloam Springs Critical violations: Employee did not wash hands after coming in from outside; did not wash hands between glove/task changes, hand washing is required between glove or task changes. Food in grill side hot case is not at 135 degrees or above, white gravy is at 123 degrees, sausage patties are at 121 degrees, items were made fresh in the morning, adjust the unit as needed. Food in prep table and the salad prep table are not being held at 41 degrees or below, some items were prepared this morning, some items were destroyed as needed, other items not prepared today were destroyed; all time/temperature control for safety food must be destroyed at the end of service. Not all food is date marked as needed. Items in the children’s section of the menu cannot be offered undercooked (hamburger, eggs); items on children’s menu should not be marked with an asterisk. Noncritical violations: Sanitizer used for wiping cloths is too strong. Some plastic food containers are broken or cracked.
