Because of an error on the Arkansas Department of Human Services’ Child Fatality Notification website, an article published Feb. 27 incorrectly named the agency investigating the death.
The Phillips County sheriff's office is investigating the death of an infant who died Oct. 17 at home, according to information released Thursday on the Arkansas Department of Human Services' Child Fatality Notification website.
A Human Services Department official deferred questions about how 5-month-old Zachary Adams died and other details about the case to the Phillips County sheriff's office. A message left for investigators was not returned Thursday.
The child fatality website noted previous Human Services Department involvement with the family.
