Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Bare-hand contact with ready-to-eat potentially hazardous food, lettuce for salad (corrected).

Priority violations: Shrimp boxes stored directly on floor.

Core violations: Lids and outside of dry ingredient containers covered with grease and food debris. Portions of meat stored in take-out bags.

Priority violations: Bare-hand contact with parsley going out to customer.

Core violations: No soap at bar hand-wash sink.

Priority violations: Reach-in not maintaining food at 41 degrees or below. Filter powder stored next to food utensils and single-service items.

Core violations: No hot water at hand sink in front of store, alternate sink available. Bags of ice for sale in store not labeled with name, address or contact information.

Priority violations: Utensils not reaching 160 degrees in ware washing machine. Use three-compartment sink until temperature reached.

Priority violations: Whole shell eggs stored above ready-to-eat (corrected). Raw bacon delivered 50 degrees.

Sept. 5

Sunrise Cafe

136 S. Gentry Blvd., Gentry

Priority violations: Pot and other items in handwashing sink; handwashing sink not accessible because items are in the way; food debris in sink. No paper towels at either handwashing sink (corrected); employees using cloth towel to dry hands. Employee did not use soap when washing hands. Raw meat stored above ready-to-eat foods and raw meat package touching packages of ready-to-eat foods. Put tongs used on raw chicken back in bowl used for fried potatoes; employee stopped (corrected).

Core violations: Excessive water dripping from cooler unit; bags of food sitting in the water. Cooler is not keeping food at 41 degrees or below; food is 50 degrees or more (food destroyed as needed).

Pam's Sugar Shack

138 Rebecca St., Gentry

Priority violations: Potatoes held at room temperature during cooling process and not completing the cooling process; potatoes still 81 degrees. Some splash-over from hand sink on to clean dishes; either put up a barrier or be extra careful to prevent splash-over. Not all items date marked as needed.

Core violations: Thawing foods at room temperature. Dishes drying directly on towel; dishes must be dried on something that is smooth, easily cleanable and nonabsorbent. Dishes leaning against the wall; paint peeling off wall. Bags of onions stored on the floor. Seal all wooden items to ensure wood is smooth, easily cleanable, nonabsorbent, including under coffee maker.

Core violations: Three-compartment usage: wash, rinse, sanitize, air dry, 50 to 100 ppm chlorine. Sink stoppers required at three-compartment area sinks.

Upper Elementary School

Core violations: Kitchen self-serve cooler thermometer requires calibration. Sneeze shields require reinstallation on serving line. Moist wipes at food prep require sanitizer residual.
