RESTAURANT INSPECTIONS: February 11th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Jan. 18

Pizza Hut

814 W. Cypress St., Rogers

Priority violations: Sauce 48 degrees, ham 56 degrees, sausage 54 degrees in large pizza prep; must be kept at 41 degrees or below (corrected).

Priority foundation violations: Large pizza prep is not keeping foods at 41 degrees or below (all products destroyed).

The Silk Road Thai Restaurant

1023 S. Eighth St., Rogers

Priority violations: Raw beef being stored above lettuce in double-door glass cooler (corrected). Sliced tomato 56 degrees, bean sprouts 56 degrees; at room temperature on top of prep table; must be kept at 41 degrees or below or use time as a control.

Core violations: Thawing raw beef at room temperature (corrected). Chest-type freezer lid is being repaired with duct tape; not a smooth/cleanable/nonabsorbent surface. Fifty pound bag of sugar, box of onions and box of carrots stored on floor; must be at least six inches off floor.

Dollar General 1981

Core violations: Spill of eggs under cartons; milk on refrigerator door.

Brightfield Middle School

5101 S.W. Bright Road, Bentonville

Priority violations: Ham dated 1/18/13 pulled from freezer; no pull date from freezer. Some hot items not held at 135 degrees or above; reheat as needed.

Core violations: Brushes on baking side are not food grade and cannot be used on food items; new ones on order; do not use these brushes; find another product until appropriate utensils are received. Ice scoop laying down in ice in ice machine; store scoop on holder or handle up only.

Follow-up inspection

Priority violations: Tongs used for hot dogs and scoop used for ice were not sanitized after washing (repeat violation).

Priority violations: Raw meat in pan stored above bell peppers and lettuce in three-door refrigerator.

IHOP

4604 W. Walnut St., Rogers

Priority violations: Employee did not wash hands prior to going back to work. Foods stored uncovered in both reach-in coolers. Pulled roast beef task left to sit on prep table at room temperature while employee did something else; product 51 degrees. Server juice cooler right side is not holding food at 41 degrees or below.

Core violations: Uncovered crepes with a package of tortillas on top. No thermometer in meat cooler. Tiles in the dish machine area are broken and missing. The floor is not easily cleanable; water standing stagnant and dirty. Tiles under produce prep sink are missing; dirty water standing. Leak in drain of sprayer sink.

Follow-up Inspection

Core violations: Employee not properly washing hands. Employee must wash hands after handling raw meat before touching ready-to-eat foods. No bare hand contact with ready-to-eat foods. Food contact surfaces must be sanitized after cleaning.

La Estrella 2

115 N. Dixieland Road, Suite 17, Rogers

Priority violations: Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. Sanitizer tested over 500 ppm; should be 200 ppm for quaternary ammonia.

Core violations: Employee involved in food prep must wear a hair restraint. Need sanitizer test strips. May not reuse bulk containers.

Core violations: Food in customer self-service must be protected, identified. Floor in kitchen needs to be smooth and easily cleanable.

Priority violations: Front three-compartment sink is not registering with bleach or quaternary sanitizer in the final rinse sink (corrected); use rear three-compartment sink until corrected at front.

Core violations: Quaternary ammonia test strips are not reading sanitizer strength accurately; replace with test strips that measure accurately.

Pizza Hut 4057

Priority violations: Dish machine registered no sanitizer in final rinse (corrected); sanitizer was primed; final rinse had 50 ppm chlorine.

Priority violations: Diced tomatoes 50 degrees and hot dog 49 degrees in prep fridge (corrected). Food in cold storage must be 41 degrees or less.

Priority violations: Raw eggs being stored above box of butter in refrigerator in basement (corrected).

Sandy’s Hot Dog Push Cart

3616 Rainbow Road, Rogers

Priority violations: Sanitizer in three-compartment sink and in wiping bucket was at 0 ppm for chlorine; must be 50-100 ppm for chlorine (corrected).

Core violations: Tongs were being stored in sanitizer water before use; must be dry after washing/sanitizing before using (corrected).

Applebee’s

528 N. 47th St., Rogers

Priority violations: Dishwasher not making heat sanitation (corrected).

Core violations: Employee involved in food prep must wear a hair restraint, i.e. beard restraint. Cracked tile in cooler to freezer door way.
