Critical violations: Onions stored on a broken shelf is within 2 inches of the running water from the ice machine. Food in the walk-in cooler lacks a cover (roast, diced ham, diced potato). The walk-in cooler is holding food at 54 degrees. Temperature control for safety foods shall be held cold at 41 degrees or below. No date on the food products in the walk-in cooler.

Noncritical violations: Under the onion shelf was rags and pot holders and water with black residue buildup.
