Aug. 16

Cannonball Cafe

311 E Pickens Road, Pea Ridge

Critical violations: Uncovered coffee mugs and Styrofoam cups with employee beverages throughout prep area. Cups and utensils present in front of house handsink at time of inspection. Dish machine 0 ppm chlorine.

Noncritical violations: Fans in walk-in cooler have dust build-up.

TNT Express

1196 S Highway 59, Decatur

Critical violations: Employee cleaning tables and sweeping floor applied hand sanitizer but did not wash hands prior to handling food utensils or containers serving customers. Container of raw chicken stored above a container of dressing in the two-door cooler. Hamburger steak and onion, as well as chicken gizzards not at 135 degrees or above. Items stored on the right side of the hot case and the sliding door was ajar at the time of inspection. Raw chicken stored at room temperature. Ice is only on the top of the chicken. Items checked in the walk-in are not at 41 degrees or below. No date marking on the cream in a box at the self serve station. Items in the sliding two-door cooler are not date marked as needed. Ice and water in the three-door freezer is falling onto uncovered food items.

Kwality Ice Cream & Bakery

3404 SE Macy Road, Suite 18, Bentonville

Critical violations: Raw chicken marinating above romaine lettuce and other ready-to-eat food items in walk-in cooler. Front prep cold-hold unit had food items above 41 degrees: Sliced tomatoes at 61 degrees, butter and chicken at 54 degrees, and sliced cucumbers at 60 degrees. Foods under temperature control shall be held at 41 degrees or below or 135 degrees or above. Bag of onions stored directly on kitchen floor.

Noncritical violations: Multiple non-food contact surfaces throughout the facility have a build-up of grease and food residue. This build-up was seen on walls, outside surfaces of equipment, inside of microwave, and around grill and fryers. No certified food safety manager for facility.

La Huerta

129 Fowler, Suite B, Gentry

Critical violations: Food temperatures in the walk-in cooler are not all at 41 degrees or below. Beans at 43 degree, salsa at 42 degrees, chicken at 42 degrees, Chili Relleno at 41 degrees and internal thermometer reads 44 degrees. This is a repeat violation. Some foods in the walk in cooler are not covered. Unwrapped cooked taco shells stored in an unlined cardboard box that originally contained cooked shrimp.

Noncritical violations: Packages of tongue sitting in a pan of water at room temperature thawing. Food accumulation on the underside of the shelf above the steam table. No certified food protection manager.

Noncritical violations: Sprayer hose is lower then the sink flood rim. Paint peeling from ceiling tiles in kitchen. Rip in corner of vinyl flooring in kitchen.

Critical violations: Active managerial controls to maintain food safety not observed during time of inspection. Multiple areas of food safety were not observed during this inspection. There is not a person in charge but the employees working and responsible for food preparation were not demonstrating principles of food safety. No actual designated person in charge. The employee working at the time took responsibility because there is no one else on site. Establishment does not have an established Health Reporting Policy. Employees not washing hands between glove changes. Employees were not lathering hands with soap prior to rinsing the soap off with water. One employee dried his hands on his pants. Flat of raw shell eggs stored above bags of onions and carrots. Employee was washing utensils but not sanitizing them prior to putting away to dry. Foods checked in the three-door cooler, the preparation table, the vegetable cooler and the small upright cooler were not at 41 degrees or below. Rice being used for fried rice sitting at room temperature. Employee did not mark the sushi rice indicating the time the rice needs to be used by.

Noncritical violations: Rice cooling on a shelf at room temperature. Rice at the time of inspection was 165 degrees but employee was not able to tell inspector proper cooling procedures and says the rice stays out until it is cooler and then puts in the refrigerator. No written procedure for sushi rice on location. Raw shrimp thawing in a pan at room temperature.

Critical violations: Bare hand contact with ready-to-eat food. Bottle of porter under raw meat. Hamburgers cooking rare at 125 degrees.

Noncritical violations: Open beverage container on counter. Cook in back not wearing hair or beard covering. Wiping cloths used and not returned to sanitizer bucket.

Fairfield Inn and Suites Rogers

Mazzio’s Pizza

Critical violations: Employee did not lather for the required 10-15 seconds prior to rinsing soap off. Employee came into the food preparation area after handling money and did not wash hand prior to putting on gloves. Sanitizer dispensed from the dish machine is not strong enough. Interior of the sandwich and pizza prep table doors, hinges and area around the pans are visibly dirty. The sliced turkey, salami, sliced cheeses, artichoke hearts and containers of pasta are not at 41 degrees in the sandwich prep table.

Noncritical violations: Food workers shall wear hair restraints, beard restraints, and clothing that covers body hair. Except for a plain ring such as a wedding band, while preparing food, food workers may not wear jewelry (including medical information) on their arms and hands. No test strips for quaternary ammonia sanitizer. Repeat violation. Food preparation equipment and the areas around the equipment is visibly dirty. Flour around the dough mixer and the under the bags of flour. Repeat violations. Condensation leak in the walk in freezer. Do not store open cases of food under the water leak. Walk in door does not close. The door gasket it coming off the door and it appears if it is preventing the door from closing. Employees are storing cases of chemicals in front of the door to keep it closed. Water leaking from the ceiling right next to the dough mixer. Current permit is not posted in customer view.

Taqueria 3-Brothers

699 Main St., Centerton

Critical violations: Raw bacon on top of canned drinks in fridge. Salvadorian cream and sour cream in ice box at 56 degrees and undated. Sour creams stored above 50 degrees and undated. Chicken in water in washing sink with dirty food container on top, very used wiping cloth touching raw chicken. Used wiping cloths around food contact surface areas and in direct contact with thawing chicken.
