Critical violations are those factors leading to food-borne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.

BENTON COUNTY

March 16

Acambaro Food Inc.

215 S. Eighth St., Rogers

Critical violations: Cook arranged taco beef with bare hands. Tilapia thawed under refrigeration in original packaging.

Noncritical violations: Door of hot hold cabinet is covered with grime. Walk-in cooler wall has black filth and shelves are rusted in walk-in cooler. Grout has eroded between floor tiles. Many floor tiles throughout kitchen are cracked and broken. Kitchen floor holds mop water.

Flying Burrito

1401 S. Walton Blvd., Suite 1, Bentonville

Critical violations: Employees were unsure of what temperature the cold food items should be at for safety. Corn (date prepared on 3/15) was at 56 degrees in the walk-in cooler. Hot sauce (date prepared on 3/14) was at 55 degrees in the walk-in cooler. Pico de gallo was at 55 degrees at the front prep unit.

Noncritical violations: Posted retail permit expired 9/30/2018.

Happy Donut

Critical violations: None

Noncritical violations: Exposed wood on the side of the cabinets and the back side of the counter is not sealed.

Hibachi Grill & Buffet

Critical violations: None

Noncritical violations: Ice cream is available to be scooped by customers for self-service.

Jimmy John's

3200 U.S. 412 East, Suite 10, Siloam Springs

Critical violations: Person in charge did not have knowledge of how to check sanitizer concentration. Employee did not lather with soap for the required 10-15 seconds prior to rinsing off soap.

Noncritical violations: Except for a plain ring such as a wedding band, while preparing food, food workers may not wear jewelry on their arms and hands.

Waffle House

2723 U.S. 412 East, Siloam Springs

Critical violations: Raw shell eggs stored above ready-to-eat foods in the front refrigerator.

Noncritical violations: Bottom of back room refrigerator is visibly dirty. Floor tiles missing between the front and back rooms. Excessive ice buildup on freezer unit and boxes of food stored on top shelf. Retail permit is not posted in customer view.

March 17

Azul Tequila

1000 S.E. Walton Blvd., Suite 22, Bentonville

Critical violations: Multiple white hand towels were in sink closest to oven in kitchen. It is the only hand sink in the kitchen. Full steam table pan of carne molida at 51 degrees in walk-in cooler was made last night. Spray bottles of degreaser, bleach, and oven and grill cleaner are not labeled.

Noncritical violations: None

Catfish John's

Critical violations: None

Noncritical violations: Cardboard boxes of food items being stored directly on floor of walk-in cooler and kitchen area. Ice scoop being stored on top of of ice maker, which is an unsanitized surface.

Hunan Village

3402 U.S. 412 East, Siloam Springs

Critical violations: Employees are not washing hands between task changes or coming back into the kitchen. All fried items on one of the buffet stations are not at 135 degrees or above. Ice cream in the hopper is at 54 degrees. Product instructions say to keep at 41 degrees or below. Pre-fried chicken pieces (for sweet and sour chicken) sitting at room temperature. Store food in refrigerator when not being prepared.

Noncritical violations: Containers of food stored in the walk-in uncovered. Using a drill and mixing stick to blend batter for chicken. Mixing stick is not designed for food use. It is not easily cleanable where it is welded together and the finish is coming off. Interior and doors of the three-door refrigerator is not clean. Food debris and grime on shelves and bottom of the unit and handles/doors. Shelves in the walk-in have food debris on them.

Lucy's Diner

511 W. Walnut St., Rogers.

Critical violations: No paper towels available at time of inspection at warewash handsink.

Noncritical violations: Lights in warewashing area have no protective cover.

Super Donuts-Deli Bakery

901 First Ave., Suite A, Gravette

Critical violations: Chicken and rice dish being eaten in the kitchen area. Flour and cinnamon in large container with no lid. Used wiping cloths on wheeled rack. No sanitation in the three-compartment sink and no test strips available.

Critical violations: Spanish rice on burrito bar at 120 degrees. Open container of tuna salad not date-marked.

Noncritical violations: None

Workman's Travel Center FS

Critical violations: Hand washing sink near fryer blocked by bucket and boxes.

Noncritical violations: No chlorine test strips for sanitizer at small dish machine.

March 18

Fresh Fish Market Place

607 S.E. Fifth St., Bentonville

Critical violations: No parasite destruction forms available at time of inspection.

Noncritical violations: No test strips available for dish machine and three-compartment sink. Accumulation of ice on curtain to walk-in freezer, as well as floor around the door. Floor is also damaged around door as well.

White Oak Station

701 S.W. Regional Airport Blvd., Bentonville

Critical violations: None

Noncritical violations: Bottom shelf of prep table is rusting and is no longer smooth and easy to clean. Pots are stored on this surface. Inside of microwave has an accumulation of food debris and residue. Shelves near frying area have an accumulation of grease and dust. Cold handle was not working at the hand wash sink near the ice machine, so cold water was not available for hand washing.

March 19

Harps Deli Bakery

715 N. Second St., Rogers

Critical violations: Employee washed hands in three-compartment sink. Access to handwash sink obstructed by mop bucket and other supplies.

Noncritical violations: Cappuccino machine has accumulation of beverage powder on dispensing nozzle.

La Huerta

1270 U.S. 412, Suite A, Siloam Springs

Critical violations: Pan of queso closest to the finish station is not at 135 degrees or above. Unit was turned off. Pan of beans in the walk-in 48 degrees. May be improper cooling from the previous day. Salsa at servers station is 44 degrees in the center but along the edge where the ice is, the product is 41 degrees. Milk in the servers cooler is at 45 degrees. Plate of chili relleno stored on the pans of food in the fajita station. Crab meat not date marked as needed.

Noncritical violations: Steak chimichanga is not being cooled properly. Pan is sitting on the table to cool. Pan put in an ice bath during inspection. Bags of onions stored on the floor in the store room. Except for a plain ring such as a wedding band, while preparing food, food workers may not wear jewelry on their arms and hands. Unless wearing intact gloves in good repair, a food worker may not wear fingernail polish or artificial fingernails when working with exposed food.

Sonic

702 First Ave. S.E., Gravette

Critical violations: Open drinking cup near the ice machine. Two employees on the cold hold burger prep and french fry area handling food bare handed. Food prep table has food debris on it. Top of ice cream machines have dust with ice cream buckets upside down touching unsanitized surface. Hot dogs in hot hold at 45 degrees. Used dirty wiping cloths on the grill and prep area.

Noncritical violations: Tomatoes in cold hold unit stacked too high, 45 degrees. Accumulation of food crumbs and residue on floor beside ice machine.

Starbucks

1803 E. Centerton Blvd., Bentonville

Critical violations: None

Noncritical violations: Worker not wearing head hair restraint.

The Lion Stop

601 First Ave. S.W., Gravette

Critical violations: Old fridge in back is struggling to maintain temp. The unit has a large amount of dust and debris buildup and the inside surfaces have black on the shelves and grill. Cheese and chicken in the back fridge at 43 degrees. Raw chicken in the fridge with no date mark.

Noncritical violations: None

Will Yum's/Top Dog Catering

101 Shane Drive, Centerton

Critical violations: Handwash sink full of containers. Ribs, 55 degrees and small fridge items discarded. No date marking present. Nonfood contact surfaces, microwave, prep table and floor have food debris and detritus buildup.

Noncritical violations: None
