Critical violations are those factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Baked potato soup was at 114 degrees in hot holding.

Smokin' Joe's Ribhouse

2504 E. Central Ave., Bentonville

Critical violations: Dishes were blocking access to a handwashing sink. Shredded meat was at 50 degrees in hot holding. Items in a prep cooler were not at 41 degrees or below.

Noncritical violations: There was a dead fly in an ice maker. An ice scoop was stored in an ice bin. Grease and oil from a smoker were draining into the parking lot. The floor under the soda box dispenser needs to be repaired.

Starbucks

2500 U.S. 415 East, Siloam Springs

Critical violations: There must be an adequate amount of hand cleanser and paper towels at each handwashing sink; water temperature at handwashing sinks must reach 100 degrees. Sanitizing chemicals must be available at the three-compartment sink.

Noncritical violations: There was no thermometer in the walk-in cooler or the front display case. Test strips must be used to test the strength of chemical sanitizer. The ice machine needed to be cleaned and sanitized. The hot water needs to be adjusted at one handwashing sink.

Sushi House

2501 S.E. 14th St., Suite 5, Bentonville

Critical violations: The handwashing sink in the drink area was blocked. There was no parasite destruction form available for salmon. Food contact equipment and utensils need to be cleaned and sanitized. Raw fish was stored above fresh vegetables in the walk-in cooler. Items used to prepare raw food items must only be used for raw food items. Fried rice was not cooled properly before being placed in cold holding. Food in a cold holding unit was at 45 degrees. A cold holding unit was not maintaining food temperatures of 41 degrees or below.

Noncritical violations: A plate that was being used as a rice dispenser was stored in rice. A carpenter drill with paint/plaster mixing bit was being used for food preparation.

Village Inn Restaurant

Critical violations: None

Noncritical violations: Handles to a prep unit need to be replaced.

Sept. 17

Angus Jack Burgers and Fries

1001 S.W. Regional Airport Blvd., Bentonville

Critical violations: An employee did not wash hands and change gloves between tasks. Sliced tomatoes were at 55 degrees, sliced onions were at 57 degrees and pico de gallo was at 48 degrees in cold holding.

Critical violations: Raw eggs were stored above ready-to-eat food in cold holding.

Iron Horse Coffee Co.

220 S. First St., Rogers

Critical violations: Deli meat was not being thawed in accordance to established procedures.

Noncritical violations: Food packaged in the facility wwas not labeled in accordance to standards of identity. Food workers must wear proper hair restraints. The oven needs cleaning. Fans need cleaning; the ceiling in the food, bar and customer areas needs repair.

La Nueva Parroquia Market

400 S. Eighth St., Suite A, Rogers

Critical violations: A handwashing sink must be accessible to employees at all times; there were no paper towels at a handwashing sink. Papaya in the produce section had mold. Canned goods had dents that might affect the seals. A band saw in the meat department needed cleaning.

Noncritical violations: Ham packaged on site was not marked with a discard date. There is a gap in the back doors; a used drink cup near the back door had fruit flies and larvae. The seal around a walk-in freezer needs repair. A protective grate needs to be installed over an exhaust fan in the produce section; equipment that is not necessary to the operation of the facility needs to be removed from the premises.

Las Palmas

Critical violations: There was no soap at a handwashing sink in the bar area.

Noncritical violations: An ice scoop was stored in an ice maker.

Miss Saigon Pho

310 S. Walton Blvd., Bentonville

Follow-up inspection.

Critical violations: Employees were not washing hands and changing gloves between tasks. There were multiple flies in the kitchen. Buckets of food were stored on the floor of the walk-in cooler; cardboard boxes of food were stored on the floor of the walk-in freezer.

Noncritical violations: A grate needs to be installed over a floor drain near the walk-in freezer. The floor of the walk-in freezer needs to be repaired. Several nonfood contact surfaces needed to be cleaned.

Natural Muse

Critical violations: An employee's drink did not have a lid and straw. Diced tomatoes and sour cream were not at 41 degrees or below in cold holding.
