Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Noncritical violations: Two hand wash sinks up front, only one required; first hand wash sink has no cold water (repeat).

La Sirenita

505 Thomas Blvd., Springdale

Critical violations: When inspector went to wash hands there were dishes in the sink; a hand washing sink should not be used for purposes other than hand washing. Owner was not able to produce shellfish tags, must keep documentation for three months. Avocados were placed on the floor in a box at time of inspection; food should be six inches off the floor. Coleslaw salad was temperature measured at 53 degrees, should be 41 degrees and below. Multiple items were observed without a date identification; foods held for more than 24 hours should be clearly marked to indicate discard date. Manager stated that he cleaned his meat slicer with isopropyl alcohol; chemical sanitizer applied to food contact surfaces shall meet the requirements specified.

Critical violations: Dishwasher sanitary level was reading 0 ppm at the time of visit, should be 50-100 ppm (corrected). Bean sprouts were observed at a temperature of 48 degrees; should be kept at 41 degrees or lower (corrected).

Noncritical violations: The kitchen area could benefit from a cleaning of floor areas; the physical facilities shall be cleaned as often as necessary to keep clean.

Large container of refried beans at 128 degrees; put in shallow pans or in ice bath. Label all chemical spray containers with working name; sanitizer, window spray, degreaser, etc.

Noncritical violations: Rear hand wash sink pipes leaking and no hot water. Fix leaking pipe and turn on hot water valve. Cooling shall be accomplished in accordance with the time and temperature criteria using effective method; use shallow pans or ice bath for cooling. No thermometer in waitress cooler with milk; keep at 41 degrees or below. Fix torn screen door in back entrance. Keep food covered in walk-in; fix condensation dripping in walk-in cooler. Stain or paint bare wood in walk-in cooler; must be smooth and nonabsorbent. No hot water to rear hand washing sink.

Nellie B's Bakery

2421 N. Center St., Suite B, Fayetteville

Critical violations: Cooked soups and open packages of ready to eat sliced turkey and ham refrigerated for greater than 24 hours lack discard date marking.

Noncritical violations: Food packaged in Retail Food Establishment for customer self-service lack required labeling as specified in law. Several ceiling tile in food preparation area are not smooth. Posted retail food permit has expired.

Wasabi

313 W. Dickson St., Suite 105, Fayetteville

Critical violations: Raw eggs were stored above cut produce, ready-to-eat foods in walk-in and small cooler. No sanitizer in three-compartment sink. Chemical spray bottle is not labeled with common name. Sanitizer in original degreaser spray bottle. Make sure old container is clean before using another product.

Noncritical violations: Temperature measuring device must be calibrated and accurate within the 1 degree if in Celsius and Fahrenheit or 3 degrees if only in Fahrenheit, sushi cooler thermometer shows 51 degrees, my temp 33 degrees. Several ceiling tiles in the food prep area are absorbent, not smooth; bare wood at sushi bar needs to be stained or painted. No test strips for quaternary ammonia. Ceiling in food prep area is not clean. Cover of lights in food prep are broken; replace, cover or shield.

Food containers in walk-in are not covered, should be covered by lid or wrapping to protect from contamination. Sanitizer buckets tested at 200 ppm, should be between 25-100 ppm. Beans were cooked this morning and cooling on the counter for preparation to mashed beans, correct by reheating beans and cooling in small pans in ice bath. Cabbage salad was held in prep area not cooled at 51 degrees, should be 41 degrees or below. Several items were not dated with made-by or discard date.

Noncritical violations: Door opened to outside. No permit posted.

Tang's Asian Market

Critical violations: Items in cooler are not marked with discard date. Owner shall have a written procedure for time as a control.

Noncritical violations: Trash can in ladies' room did not have a cover. Garbage dumpster needs a drain plug.

Supermercado Mi Hacienda

1501 W. Sunset Ave., Suite A, Springdale

Critical violations: No paper towels were available at the hand wash sink; correct within 24 hours. Food in walk-in cooler lacked covering. Diced melons were recorded at 50 degrees. Items in the cooler are not marked with made-by date or date to discard. Sanitizer was in unlabeled bottle and was not diluted with water.

Noncritical violations: Food utensils were stored, dirty, under shelf unit. No permit located.

Tequila Bar & Grill Restaurant

Critical violations: Working container chemical spray bottle was not labeled; labeled with common name.

Noncritical violations: Second hand wash sink has no hot water (corrected). Food must be stored in compliance with established regulations. Noodles for soup needs sneeze guard or to be covered. Several food prep employees wore no hat or hair net; gel is not considered an effective hair restraint. No hot water in hand wash sink.

Bordino's

310 W. Dickson St., Fayetteville

Critical violations: Ice machine was being used for cooking procedures and cooling, showed accumulation of mold residue, ice machines shall be cleaned at a frequency necessary to preclude accumulation of soil or mold. Lasagna to be reheated did not have a used-by date and was reported as being made Tuesday; foods not used within 24 hours shall be clearly marked with use-by date.

Kraken Killer Seafood

402 W. Dickson St., Fayetteville

Critical violations: Discontinue consuming food, use of unapproved beverage containers, or use of tobacco in food preparation or warewashing area; open cup in food prep, must have top and straw (corrected). No sanitizer dish washer, chlorine container empty (corrected) added new container, sanitizer now 100 ppm chlorine; in the future, if no chlorine, will require audible alarm. All chemical spray bottles must be labeled with common name (corrected), chemical spray bottle now labeled Windex.

Noncritical violations: Must have a sign or poster that notifies food workers to wash their hands at all hand washing sinks used by food workers and shall be clearly visible to food workers, both bathrooms and several other employee hand wash sinks no hand wash signage. Has a consumer advisory on the menu, must identify which foods under advisory (raw oysters) on the menu. No thermometer in small cooler with salad dressings. Bag of flour and wrapped bread stored less than six inches off the floor (corrected). Bowl was being used for a scoop in boil mix. Female bathroom has no top on the trash can.

Soup and Salad Buffet

315 W. Tulsa St., Siloam Springs

Critical violations: Containers stacked inside other containers of uncovered or unprotected food. Sanitizer concentration is not strong enough. Food storage containers have a greasy accumulation. Mixed greens are not being held at 41 degrees or below. Items are not date marked as needed (repeat violation).

Noncritical violations: Colander is cracked and no longer in good condition; replace (repeat violation). Lid for customer self serve ice bin is cracked; replace (repeat violation). Restroom doors are not self-closing. Men's restroom has a strong urine smell; ensure tiles and area around toilet are clean and in good repair. Permit is not posted in customer view (repeat violation).

Doe's Eatery

Critical violations: Tubs of steaks and fries were uncovered at the time of inspection.

Noncritical violations: Back of the house could benefit from a good cleaning.
