Washington County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Baked chicken in deli department was 130 degrees; time/temperature control for safety hot foods shall be held at 135 degrees or hotter unless using time as a control. Noncritical violations: Egg salad sandwich ingredient label lacked bread, bread ingredients, and wheat allergen information. Halogen light bulb over deli fryers lacked protective cover or shield; all light bulbs in food preparation/serving areas shall be shielded or shatterproof.

2894 W. Sunset Ave., Springdale Critical violations: Ground beef packages were stored above roast beef packages; ground pork is stored over pork steak, also found in the five for $25 bin. Except when combined as ingredients, separate types of raw animal foods from each other during storage, preparation, holding and display. Prepackaged item did not show the list of ingredients on package of diced fruits, yogurt covered snacks and other items. Noncritical violations: Diced melons and chopped lettuce were recorded at temperatures outside of the acceptable range.

April 19

Western Sizzlin’

3492 W. Sunset Ave., Springdale Critical violations: Person in charge failed to demonstrate knowledge of food safety, evident throughout the facility or by appropriate responses and actions and by the number of critical and priority violations at time of inspection. Active managerial controls to maintain food safety were not observed during time of inspection. Dishwasher observed moving from handling dirty dishes to clean dishes with only a 3-second hand wash without using soap in a non-hand washing sink. Food prep worker placing desserts on serving plates neglected to wash hands before donning gloves. Employee was observed dumping sanitizer buckets and cloths into hand wash sink. Several unopened packages of uncooked ground beef in the meat walk-in were dripping and were brown to gray in color. Some food containers lacked lids or coverings or lids were cracked and broken; broth in walk-in, gelatin in refrigerator, some items in plastic bins in walk-in and opened bag of cornmeal in dry storage that was under water drip. Unless using time as a public health control, all temperature controlled for safety foods shall be kept at 41 degrees or colder; multiple items in buffet, coolers and walk-ins exceeded temperature standards. All sanitizer buckets and dishwasher concentrations exceeded allowed standards for chlorine sanitizer; sanitizer was improperly used by mixing chlorine sanitizer with detergent and effectively decreasing effectiveness of the sanitizer. Noncritical violations: Broth, cornmeal and bacon bits were uncovered and had water from walk-in condensers dripping on them. Many damp wiping cloths not in use were observed unattended on the counters. Several shelves in coolers need to be replaced or repaired; unfinished wood observed in walk-in and needs to be painted, varnished, or replaced; many broken, cracked floor tiles throughout the kitchen need repair or replacing. Chemical sanitizer test strips are not being used to confirm sanitizer concentration within acceptable range. Entire back of house would benefit from a thorough and regular cleaning to remove accumulation of soil residues. Floor surfaces throughout kitchen area and walk-in coolers were slippery and not easily cleanable evidenced by the multiple water puddles and difficulty walking through kitchen even while wearing slip-resistant footwear. Light bulbs and fluorescent bulbs throughout kitchen and in walkin coolers lacked shielded or covered bulbs increasing risk of physical contamination.

Sassy’s Red House

Bulk sugar scoop is stored handle downward in the sugar. Bulk flour plastic scoop is damaged. Small section of ceiling finish in food preparation area lacks repair. 3418 W. Sunset Ave., Springdale Critical violations: Some items such as deli meat in small refrigerator lacked lid or plastic wrap to protect from environment. Chili was being cooled in a pan of ice; the tall container was preventing temperature to drop rapidly enough for safety and temperature not being recorded. Foil wrapped baked potato at 127 degrees observed in hot holding unit which increases the risk of botulism. Potato was rapidly reheated to 171 degrees which minimized risk. Milk, diced ham, diced tomatoes and other time/temperature control for safety food items exceeded 41 degrees; these foods shall be kept at 41 degrees or colder unless using time as a control. Time/temperature control for safety foods that are prepared or commercially packed containers that are opened and kept for more than 24 hours shall be labeled with date and discarded within seven days; several items in small refrigerator lacked date marking. Noncritical violations: Chili was found cooling in tall container; adequate cooling shall be accomplished by placing in a shallow pan, placing in smaller containers, placing items in a ice bath, etc. Chicken was found thawing in bucket near dish machine. Cooks and other staff lacked thermometers to ensure proper cooking temperatures achieved, cooling achieved or adequate hot and cold storage. Mushroom container, cottage cheese cartons, buckets and other items being reused for food storage. Food contact and nonfood contact surfaces shall be kept free of accumulation of dirt, food residue and other debris; ice machine and other surfaces in the kitchen need thorough cleaning and sanitizing.

Las Palmas

1115 E. Henry de Tonti Blvd., Tontitown Critical violations: Some items in walk-in and freezer lacked lid or plastic cover to protect items. Some readyto-eat foods in walk-in lacked a date marking. Noncritical violations: Employee bathroom door shall be self-closing. Light bulb in the store room is not shielded or covered.
