Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Jan. 14

Daylight Donuts

155 Highway 412 East, Siloam Springs

Priority violations: Random customer not scheduled to work was waiting on customers at front counter. Sausage in back refrigerator is 55 degrees and date marked 12/5.

Core violations: Cove base behind the doughnut machine is coming unglued. Thermometer in back refrigerator may not be accurate. Crates are not easily cleanable; too many ridges.

Pizza Hut

391 Highway 412 East, Siloam Springs

Priority violations: Some items in the bottom of the pizza make line are not 41 degrees or below (corrected, moved to walk-in).

Core violations: Clean under side of shelves in walk-in. Walk-in door and handle issues have been addressed and are being worked on; unknown completion date. Back wall behind dishwasher is scheduled to be cleaned sometime next week.

Grub's Grille

3001 Market St., Rogers

Priority violations: Employee handling bun with bare hands (corrected). Chili date marked 1/6 in walk-in; potentially hazardous foods must be used or discarded in seven days or less. Pans of chicken not labeled/dated in walk-in (corrected).

Core violations: Employee using handwashing sink to rinse out sauce bowl. Cove base missing from base of wall in dish room and other parts of the kitchen and bar. Prep person in back using mop sink to wash hands. Condensation leak in freezer; freezer door has ice buildup and is not closing properly. Bar floor is no longer smooth and easily cleanable; concrete wearing away allowing standing water. Dish machine at bar appears to be leaking. Clean wall at bar, especially around hand sink and coolers. Employees not wearing beard restraints.

The Kid's Studio

5500 Pinnacle Point, Rogers

Core violations: Clean the top of the dish machine. Clean cabinet, sticky where test strips are stored.

Subway

5204 Village Parkway, Rogers

Core violations: Thermometer in reach-in reading 48 degrees; product temperature should be 41 degrees or below.

Panaderia Vega

115 N. Dixieland Road, Suite 25, Rogers

Follow-up inspection

Priority violations: One live adult cockroach found on molding beside customer bathroom. Two dead ones observed in pot on clean dish shelf. Breads and pastries available for customer self-service do not have ingredients listed by weight or allergens identified.

Hibachi Grill Buffet

102 S. 21st St., Rogers

Follow-up inspection

Core violations: Walls and ceiling in kitchen prep areas are coated with grease, grime and food debris; clean. Vent hoods above hibachi grill and woks are coated with grime and grease. Ice cream scoop in water available for use of all customers at room temperature. Do not allow customers to scoop own ice cream unless it is from a dispenser to prevent contamination from multiple users. No test strips available for quaternary ammonia sanitizer at three vat or chlorine test strips for dish machine.
