Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Aug. 5

Taco Bueno

4303 Pleasant Crossing Blvd., Rogers

Critical violations: Wash hands anytime contamination could have occurred, when changing tasks and as described in the Arkansas Rules and Regulations pertaining to food establishments section 2-301.14. Sanitizer concentration at three-compartment sink not strong enough; new sanitizer was run and concentration level was corrected; may have been diluted during the washing process. Container was not clean but was put away where clean equipment is stored. Foods on the make line were not at 41 degrees or below; check unit to ensure it is working properly and mechanics are clean. Portioned salsa in the top portion of the refrigeration unit in the drive-thru is not at 41 degrees or below; check unit to make sure it is working properly and mechanics are clean.

Noncritical violations: Personal drinks stored in reach-in freezer by fryers and on a shelf in the walk-in freezer. Multiple cracked or broken plastic containers being used; replace as needed. Condensation leak in walk-in freezer; repair as needed. Water leak above the make line in drive-thru side.

Aug. 6

Siloam Springs Family Aquatic Center Concessions

120 S. Mount Olive St., Siloam Springs

Critical violations: Cream refrigerator is not holding all food at 41 degrees or below.

Noncritical violations: No test strips available. Using plastic bus tubs to wash and sanitize utensils instead of sinks; bus tubs or any alternative is not approved. No plugs on location for sinks.

Aug. 7

Kentucky Fried Chicken

1115 U.S. 412 W., Siloam Springs

Critical violations: None

Noncritical violations: Food boxes stored on floor in walk-in refrigerator and walk-in freezer; items should be stored at least six inches off the floor. Condensation dripping from almost all of the air vents; food contact items are stored under some of the vents; move food contact items or store them to prevent contamination from the condensation; repair in seven days. Grout is missing from several areas throughout the kitchen and storage area; standing water, flour and other food debris is in the space between the tiles making them hard to clean properly; re-grout as soon as possible. In chicken walk-in cooler, the sealant at the edge of the tile where it meets the wall is missing in spots and separating away from the wall in other places allowing for food and water to get into the cracks; redo caulking/sealant as soon as possible.

Pizza Hut

391 E. U.S. 412, Siloam Springs

Critical violations: Quaternary ammonia sanitizer is not dispensing; no sanitizer concentration is registering on the test strip; call for service made during inspection. Interior of dish machine filter/screen is full of food debris; cleaned during inspection. Interior of ice machine needs to be cleaned. Salad bar is not holding food at 41 degrees or below. Items held above the cutting table exceed the date marking.

Noncritical violations: Walk-in cooler door handle is sharp and no longer smooth and easily cleanable; hands could be cut on the exposed metal; replace. Walk-in cooler door is rusty and no longer easily cleanable; replace as needed. Clean behind and under dish machine. Clean wall behind the cut table. Clean behind ice machine/drink station. Floor under storage shelves is not clean; may be a beverage spill. Clean the exhaust vent and hood all the way to the exterior; excessive grease accumulation on hood and vents. Permit posted is expired; new one needs to be posted.

Subway

5204 Village Parkway, Rogers

Critical violations: None

Noncritical violations: Standing water in the soda line conduit; remove all water from conduit and seal conduit to prevent water from standing in the conduit.

The Crumpet

5212 Village Parkway, Suite 12, Rogers

Critical violations: Employee did not wash hands between glove changes (corrected). Container of diced turkey was sitting on top of another uncovered container (corrected). Items in the sandwich/fruit prep table are not being held at 41 degrees or below; items moved or discarded as needed. Green Goddess dressing exceeded the allowable number of days from preparation; foods must be used or discarded after seven days; discarded as needed.

Noncritical violations: Reusing single-use containers for food storage; containers must be meant to be washed and sanitized after use or prior to refilling. Reusing plastic elastic banded plastic bag-like covers for pie pans; these are a single- use item only. Rubber bands are not an approved method for keeping food containers closed. Cardboard box being used under to-go containers; cardboard is not an approved method for food contact items. Standing water in the bottom of the sandwich/fruit prep table. Ice machine is not draining properly; water is standing in the ice bin portion.

Carrabba's Italian Grill

Critical violations: None

Noncritical violations: Threshold in walk-in has standing water under it; replace and/or reseal to prevent the water from standing under the threshold. Re-grout walk-in floor and the dish area; observe the make line and re-gout as needed.

Bonefish Grill

3201 Market St., Suite 100, Rogers

Critical violations: Cold foods in the upper portion of the prep tables are not being held at 41 degrees or below. Foods on ice are not at 41 degrees or below on the make line; ice must be at or above the level of the food to be effective. Foods in the walk-in cooler are not being held at 41 degrees or below; may be improper cooling (foods checked). Container of Bang Bang sauce is not marked with time on container. Sani-buckets stored on top of the preparation table next to the grill and on top of the plate chiller. Chemical spray bottle hanging next to the oil spray bottles at the grill.

Noncritical violations: Employee purse stored above shelves where in-use salad bowls are stored.

Crabby's Seafood Bar and Grill

2800 S. 52nd St., Suite 102, Rogers

Critical violations: Employee did not wash hands after eating; another employee did not wash hands after handling their drinking cup; employee did not wash hands between glove changes. Employee dipped hands in sanitizer bucket instead of washing hands with soap at the hand-washing sink. Employee handled ready-to-eat food with bare hands; food destroyed as needed. Employee removed gloves and then handled ready-to-eat foods without washing hands and putting on new gloves; food destroyed as needed. Employee did not know if fish used for ceviche has gone through proper parasite destruction or raised to ensure no parasites are present (salmon). Not all hot food is being held at 135 degrees or above; may be improper reheating procedure; ensure food is reheated to 165 degrees prior to placing in hot holding. Not all cold food is being held at 41 degrees or below; may be improper cooling procedure. Not all items are date marked as needed. Consumer advisory is on menu but foods are not marked with an asterisk to identify foods requiring the advisory.

Noncritical violations: Using Styrofoam cups in bulk items; use scoop or other item with a handle and store handle up. Lids on bulk containers are cracked and no longer smooth and easily cleanable; replace as needed. Wall behind dish machine and three-compartment sink is no longer in good condition, nonabsorbent, easily cleanable; wall is not clean; area under the dishwasher has old food debris and standing water. Excessive buildup in several areas in kitchen and prep area; clean thoroughly. Leak in drain under spray sink by dish machine; drains are not clean and have build up in the floor sink. Condensation leak in walk-in freezer. Freezer door has ice building up on frame; door does not close properly.

La Hacienda

2010 E. Main St., Siloam Springs

Critical violations: Employees not washing hands long enough with soap; they are rinsing the soap off before all surfaces of the hands have been scrubbed properly. Employees not washing hands after removing gloves used for raw meat before they start a new task; wash hands any time contamination could have occurred, when changing tasks and as described in the Arkansas Rules and Regulations Pertaining to Food Establishments section 2-301. No bare-hand contact with food or ice; use gloves, spatulas, tongs, deli tissue or other dispensing equipment; employee handled tortillas and cilantro with bare hands. Clean and sanitize the interior of the ice machine; debris buildup on the drop guard; clean all areas of the machine. Foods in hot holding unit have not reached proper reheating temperature; items must be heated to 165 degrees or above in two hours or less prior to placing in hot holding unit; foods reheated on the stove as needed. Foods in the cold holding prep table are not being held at 41 degrees or below. Salsa at the server's station is not being held at 41 degrees or below. When ice is used to keep food cold, the ice must be at or above the level of the food.

Noncritical violations: Some foods stored uncovered in the walk-in; shelves in walk-in cooler are rusty and have food debris on the wire racks which could get into the food; replace shelves as needed. Ice scoop stored on top of the brew machine; store the scoop in or on something that can be cleaned and sanitized daily. Hand-washing sink drain was disconnected and draining into a bucket; it was disconnected because the sink does not drain properly; reconnected during inspection. Permit posted is expired.

Domino's Pizza

2075 E. Main St., Suite A, Siloam Springs

Critical violations: None

Noncritical violations: Cardboard used to cover mushrooms in the storage container; reusing cardboard; even the mushroom box is not approved for food contact. Employee's ponytail is hanging over her shoulder and is not effectively restrained. Tighten ladies restroom door hinge a little so that the door is self-closing; door is not completely self-closing.

Critical violations: Raw eggs stored above ready-to-eat foods. Cooler where raw eggs are stored is reading 55 degrees; raw eggs must be stored at 45 degrees; adjusted.

Noncritical violations: Sesame granola does not have ingredients listed; if two or more items are in an item, all ingredients must be listed.

Aug. 14

Rogers Aquatic Concession

1707 S. 26th St., Rogers

Critical violations: Diet Coke bottle with no lid left open on small prep fridge with cutting board (corrected); thrown away. Designate an area for employee drinks away from food prep areas. Drinks should be stored with a sealing lid or a full lid with a straw. Pizza in hold units was placed in hold units at 1:50 p.m. but the time was not marked on the glass of their equipment as required by procedures (corrected); time written on glass.

Pinnacle Golf Country Club Grill

Critical violations: Some foods not at 41 degrees or below; could be from transporting. Chicken dated 6/16/15 but it is not the 16th yet.

Aug. 19

Lin's Garden Chinese

2101 S.E .Walton Blvd., Bentonville

Critical violations: A glass of cola with no lid was located on a prep table on the back of a meat slicer. Beverage containers in food preparation areas must be covered with a lid and require the use of a straw to prevent incidental contamination of food.

Noncritical violations: Rice paddle near rice on buffet is stored in a container of water at room temperature; must be stored in water at safe food holding temperatures or in a dipping well. Recommend using a small heated pot that can keep utensil at 135 degrees or above.
