Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Sept. 8

McDonald's

4208 Pleasant Crossing Blvd., Rogers

Priority violations: Missing time markings on condiment items being kept under time as a control method (repeat). Cheese slices, diced onions on condiment bar are being kept at 61 degrees; must be kept at 41 degrees or below or on a time control procedure. Jalapeno peppers 69 degrees in jar on counter states refrigerate after opening. Dressings 64 degrees are not being kept at 41 degrees or below.

Core violations: Debris on plastic sleeve inside ice machine in back storage area.

McDonald's

2601 Pleasant Crossing Blvd., Rogers

Priority violations: After sweeping employee proceeded to put on gloves to prep food without washing hands first; must wash hands before putting on gloves (corrected). Quaternary ammonia sanitizer was over 400 ppm (repeat).

Core violations: Manager preparing food did not have proper hair restraint (repeat).

Sept. 9

Lucy's Diner

511 W. Walnut St., Rogers

Core violations: No paper towels at handwashing sink (corrected). Cooling roast in pickle bucket near handwashing sink on floor without lid; must prevent possible contamination from splashes from sink; must be stored at least six inches off floor. Re-using 5-gallon pickle buckets to store roast and other food items; not an approved multi-use container. Food debris on shelving of prep table across from automatic dishwasher; must be cleaned at least once a day.

Elza Tucker Elementary School

121 N. School Ave., Lowell

Priority violations: Egg with lima bean salad 45 degrees and sliced pineapple 44 degrees in line reach-in cooler.

Priority foundation violations: Line reach-in cooler is not keeping foods at 41 degrees or below.

Loyd's Little Land

1723 N. Concord St., Lowell

Priority violations: Drying dishes on a towel; not a smooth, cleanable, non-absorbent surface; must have drain racks that drain back into sink (repeat). Dressing 45 degrees in refrigerator 2; must be kept at 41 degrees or below.

Core violations: Refrigerator 2 is not keeping foods at 41 degrees or below.

Core violations: Wiping bucket was over 200 ppm; must be 50 to 100 ppm for chlorine sanitizer.

Core violations: Missing test strips for sanitizer (repeat).

Sept. 15

Frank Tillery Elementary School

211 S. Seventh St., Rogers

Priority violations: Cottage cheese 45 degrees in walk-in cooler; must be kept at 41 degrees or below.

Core violations: Walk-in cooler is not keeping foods at 41 degrees or below. Bathroom sink for employees is missing faucet; employees unable to wash hands in bathroom as required; employees are washing hands at handwashing sink in kitchen.

Harp's Bakery-Deli

Core violations: Improper, no time log on approximately 20 pounds of sushi rice; discarded. Approximately 27 containers of prepared sushi without documented sushi rice time control log; discarded. Paper towel dispenser needs removed from over food prep table.

Acropolis

905 N.W. 13th St., Bentonville

Core violations: Improper cold/hot holding temperature (corrected); voluntarily discarded. Splash contamination risk at handwashing sink. Tongs stored on handwashing faucet. Date marking required on food containers over 24 hours. Unlabeled spray bottle.

Fast Lane Entertainment

1117 N. Dixieland St., Lowell

Priority violations: Potato salad 45 degrees in cook's cold well (corrected); juice 45 degrees and olives 45 degrees in bar cooler.

Core violations: Cook's cold well is not keeping foods at 41 degrees or below; bar cooler is not keeping foods at 41 degrees or below. All potentially hazardous foods discarded.

White Oak Station

701 S.W. Regional Airport Blvd., Bentonville

Priority violations: Using time as a control for grab-and-go, but items not marked with time. Items using time as a control not disposed of as required. No date marking on items in the walk-in. Eggs say to keep frozen so if they are thawed they must be used within seven days. Some expired over-the-counter medication. Clean interior of cappuccino machine. Fresh spot grab-and-go cooler thermometer reads 46 degrees and 58 degrees; food varies from 43 degrees to 63 degrees and various in between (repeat).

Core violations: Gelate case door broken; no longer smooth and easily cleanable. Egg patties not covered in walk-in. Foods in freezer stored open, uncovered. Condensation leak in walk-in cooler on beer side.

Walmart Sushi Bar

Core violations: Missing soap at handwashing sink (repeat, corrected).

Sept. 17

TNT Express

1196 S. Highway 59, Decatur

Core violations: Raw chicken stored above ready-to-eat container in reach-in cooler (corrected). Wipes need stored in sanitizer between uses (corrected). Store ID required on bagged ice from on site. Date marking required on all prepared ready-to-eat food with seven-day discard date.

Maysville Handi Stop

11827 Highway 43 North, Maysville

Core violations: Must thaw frozen potentially hazardous food under running cold water or thaw in fridge (corrected). Must wash hands before gloving with ready-to-eat products (corrected). Easily readable thermometer required in each cooling unit.

Coleman's Children Academy

103 Presidential Drive, Lowell

Priority violations: Missing date marking on ham (corrected); must date mark potentially hazardous food kept longer than 24 hours; must not exceed seven days in refrigerator; must not exceed manufacturer use-by date.

Core violations: Missing soap at handwashing sink (corrected). Missing paper towels at handwashing sink (corrected).

Gold Town Sushi

1100 S.E. 14th St., Bentonville

Priority violations: Bowl used to mix kim chi sauce was set on top of trash can (corrected); cannot use trash can as a prep surface; bowl moved to prep table.

Core violations: Screen is not attached securely to back door and leaves a gap between wood and screen on door.

Priority violations: Raw chicken being stored above raw beef (corrected).

Core violations: Re-using 5-gallon buckets to store other items such as spices; not an approved multi-use container. Using a Styrofoam cup to scoop spices; must be food grade quality scoop with handle and handle stored out of spices. Tile behind raw chicken display cooler has tile chipped and water standing inside chip. Missing test strips for chlorine sanitizer (repeat).
