Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Core violations: Reusing 5-gallon pickle bucket to store; reusing grocery-type bag to hold beef in steam table.

Hibachi Grill Buffet

102 S. 21st St., Rogers

Priority violations: Employee dipped celery in three-vat sink with sanitizer and began to prepare it for cutting (corrected); celery thrown away.

Priority foundation violations: Paper towels not available at two hand sinks in food prep areas including sink by walk-in freezer, which is the nearest hand sink for employees chopping meat and vegetables (corrected, repeat).

Core violations: Cooked white rice in cooker and uncooked rice in plastic container in dry storage room had plastic bowls used as scoops (corrected); bowls discarded. Use a scoop with a handle pointed away from food during storage so that food is not incidentally contaminated by hands when food is scooped. Rear ice machine has orange and black mold growth on plastic nonfood contact surface; at end of night empty ice machine, clean and then allow to refill ice overnight.

Priority violations: Creamy feta dressing date 5/12 in walk-in cooler, tuna salad use-by date 7/23, egg salad 7/21, chicken salad 7/21 (repeat); must not be kept over seven days in refrigeration after opening; must not exceed manufacturer use-by date; must date mark after opening. Meatball 121 degrees on top portion (discarded); all portions must be kept at 135 degrees or above after cooking to 165 degrees for hot holding.

Flavors Indian Cuisine

3200 S.W. Regency Parkway, Bentonville

Priority violations: Curd rice on buffet 51 degrees, chicken salad on buffet 68 degrees, chicken biryani in walk-in 48 degrees; cold holding is 41 degrees or below. Sanitation bucket was over 200 ppm; should be 50 to 100 ppm for chlorine. No sanitizer available for use. Dishwashing machine sanitizer is empty. Must wash hands prior to putting on gloves. May not have bare-hand contact with ready-to-eat foods. Kitchen manager needs to know and follow proper food safety.

Core violations: Wiping cloths need to be stored in a sanitation bucket. Large pots unable to fit in three-compartment or dishwashing machine need to have a way to be properly cleaned and sanitized. Must label items out or original container. Floor needs to be smooth and easily cleanable. Plumbing behind ice machine and underneath it is leaking. Food may not be stored on the floor. May not have employee drinks in food prep areas. Must thaw in refrigeration or under constant running water. Employees involved in food must wear a hair restraint. Cold holding on buffet not able to maintain food at 41 degrees or below.
