Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

April 14

Cold Stone Creamery

3301 Market St., Rogers

Priority violations: Clean waffle cone storage containers (corrected).

Core violations: Clean handles on reach-in doors. Clean display case more frequently as needed. Clean "Got to have it to go" cabinet, doors and handles. Seal tile/back splash gap on front counter as needed. Gaskets torn on blast freezer; replace as needed. Drying racks are rusty and no longer easily cleanable; replace. Beard restraints required for food employees.

Tropical Smoothie Cafe

3351 Pinnacle Hills Parkway, Rogers

Core violations: Effective hair and beard restraints are required for food employees. Floor tiles cracked in front of smoothie cooler; replace as needed. Wall needs to be replaced under smoothie sinks; exposed plumbing. Broken pitchers stored in sugar bin; destroyed. Ice scoop is cracked; replace as needed.

April 23

John Q. Hammons Center

3201 Pinnacle Hills Parkway

Priority violations: No dates on in-house made dressings.

Core violations: Condensation leak in freezer. Container of feta cheese has mold growing on lid and around top; destroyed. Chicken breast thawing at room temperature by the ovens (corrected). Half and Half in banquet cooler is 54 degrees. Paper towels not dispensing at banquet service handwashing sinks. Refrigerator at serving station is not holding food at 41 degrees or below.

Janie Darr Elementary School

6505 S. Mount Hebron Road, Rogers

Priority violations: Cut tomatoes and cut lettuce being held at room temperature.

Core violations: Employee touched glasses with glove hand (corrected). Ends on scoops are missing on some; replace as needed.

Bentonville Butcher and Deli

1201 S. Walton Blvd., Bentonville

Core violations: Hinge broken on ice machine. Replace ice scoop; it is no longer smooth and easily cleanable.

Walmart Neighborhood Market

Core violations: Bakery spray bottle is not labeled with contents (corrected). Storing product above the load limit line. Clean fan guards as needed.

Walmart Neighborhood Market Deli

Priority violations: Employees not washing hands long enough or properly.

Core violations: Storage area by roaster boxes are getting dirty from splashing from roaster.

Follow-up inspection

Priority violations: Raw chicken 54 degrees in walk-in cooler; must be kept at 41 degrees or below (repeat).

Core violations: Walk-in cooler is not keeping foods at 41 degrees or below (repeat).

Jesus Loves Me Academy

Complaint inspection

Priority violations: Need to date mark ready-to-eat potentially hazardous foods. Must use hand drying provisions to turn off faucet at handwashing sink. Must wash hands when changing gloves and after touching classroom doors.

Core violations: Refrigerator in infant room 50 degrees; needs to be able to maintain food at 41 degrees or below. Thermometer in infant refrigerator is broken.

Priority foundation violations: No soap available at hand sink near dishware station, repeat (corrected). Mold growth on soda nozzles; front of refrigerators and other equipment is dirty; clean to prevent incidental contamination of food; plastic in ice bin with mold.

Core violations: Extra upright refrigerator not in use is dirty; clean and remove from premises if you aren't going to repair it.

Lucy's Diner

511 W. Walnut St., Rogers

Priority violations: Raw steak being stored above green peppers and tomatoes in walk-in cooler (corrected). Food prep employee handled French fries and bread with bare hands; no bare-hand contact with ready-to-eat foods (corrected).

Core violations: Missing thermometers in prep cooler and in up-front drink cooler (corrected). Missing a mop sink.

Acambaro Restaurant

215 S. Eighth St., Rogers

Complaint inspection

Core violations: Debris found on bottom of some chip baskets ready to be used for customer service; must clean to remove debris; must be washed, rinsed, sanitized, dried; must use a smooth, cleanable, nonabsorbent basket.

Lin's Garden Chinese

2101 S.E. Walton Blvd., Bentonville

Priority violations: EHS was present when employees walked into kitchen prep area; nine out of 12 employees did not wash their hands before beginning food prep. Observed employee begin to put on gloves without washing hands before handling raw chicken (repeat, corrected). Two employees used alcohol-based hand sanitizer without washing their hands to put on gloves (corrected).

Glasgow's Cafe

411 S.E. Walton Blvd., Bentonville

Priority violations: Sour cream in walk-in cooler is stored in walk-in cooler without a lid (repeat, corrected).

Core violations: Nonfood contact surfaces of shelves in walk-in cooler are dirty with grime; clean.

Las Fajitas

Core violations: Hand sink in bar area and hand sink in rear of kitchen is coming off the wall; resecure to the wall.

Centerton Head Start

211 Commercial St., Centerton

Priority violations: Sanitation bottle tested more than 200 ppm; should be 50 to 100 ppm for chlorine.

Jim's Centerton Cafe

1084 W. Centerton Blvd., Centerton

Complaint inspection

Priority violations: Employee must wash hands after touching raw hamburger before handling ready-to-eat foods. Employee must remove gloves and wash hands after taking glasses off face before returning to food service. Must wash hands prior to glove use. Employees must follow proper handwashing procedures. May not contact ready-to-eat foods with bare hands. Sanitation bucket tested 200 ppm; should be 50 to 100 ppm for chlorine.

Core violations: Handwashing sink is not operational. Employees using mop sink to wash hands. Employees involved in food prep must wear a hair restraint, i.e. beard restraint. Wiping cloths need to be stored in a sanitation bucket, not in back pocket or on counter top. Employees need to know how to wash hands.

Priority violations: Raw chicken thawing in sink, not under running water (corrected). Not all food items date marked as needed. Vegetable cooler is not keeping food at 41 degrees or below.

Core violations: Employee did not wash hands after handling raw meat before changing task (corrected). Storing shakers on top of chips (corrected). Cracked container fixed with duct tape; destroyed. Clean rusty shelves in upright cooler; rusty shelves in reach-in prep tables; replace. Vacuum breaker on mop sink has been replaced with something else; ensure there is a vacuum breaker.
