Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Feb. 14

White Oak Station

Priority violations: Mold on sliced cheddar and mozzarella cheeses (corrected). Chili 88 degrees for service, cheeseburger 100 degrees (corrected). Cans of tomato ketchup compromised; hermetically sealed can (corrected).

Core violations: Incomplete label products made in White Oak kitchen, such as egg salad, chef salad, ambrosia salad. Condensation buildup in walk-in freezer.

Arby's

Core violations: Loose baffle, missing screw in ice machine, rust marks from screw onto baffle. Broken cove tile floor-wall juncture.

McDonald's

Core violations: Rubber detached around ice machine.

Dunkin' Donuts

2309 S.E. 14th St., Bentonville

Priority violations: Donut holes below electrical boxes on icing containers creation of harboring conditions for pests; food debris in corners around shelving on floor. Food debris/icing on thermometer, not clean and sanitized.

Core violations: Product stored directly on floor in walk-in freezer and cooler. Employee food and store food mixed together in walk-in cooler. Light out in walk-in cooler. Ice lids have food debris. Employee staff walking through with no hair restraint. Jackets with food debris under electrical boxes.

Nina's Little Doves

112 S.E. J St., Bentonville

Priority violations: No date mark on ready-to-eat potentially hazardous foods for time/temperature (corrected). Mold in cheese sauce. Food and single-service items stored on floor (corrected).

Walmart Neighborhood Market Deli/Bakery

Core violations: Grease buildup on light covers over chicken rotisserie; light bulbs out too.

Kum & Go

2400 W Hudson Road, Rogers

Priority violations: Ice machine scoop holder grimy (corrected).

Core violations: Single-service items and coffee cases on floor (corrected).

William's Soul Food

915 S. Eighth St., Rogers

Core violations: Using bowls as scoops in flour and sugar; must be handled with handle stored out of product. Thawing raw chicken in standing water at room temperature in three-compartment sink; must thaw either under refrigeration at 41 degrees or below or under constant running water that drains constantly (corrected, repeat). Missing thermometer in refrigerator.

Rogers Heritage High School

1114 S. Fifth St., Rogers

Priority violations: Bean mix 45 degrees on top portion in ice bath; must surround ice up to food level on container; must be kept at 41 degrees or below (corrected).

Shipley's Donuts

2119 W. Walnut St., Suite A, Rogers

Priority violations: Tray with uncooked kolaches just made is stored on top of trash can (corrected). Do not use trash can with or without lid as a prep surface. Use prep tables that are designed to be smooth and easily cleanable.

Core violations: Facility has procedures to use time as a public health control to limit bacterial growth of kolaches, but is not writing the time kolaches are removed from oven down.

Kum & Go

978 Pleasant Grove Road, Rogers

Priority violations: Missing handwashing sink in back prep/dish washing area. Using spray bottle marked for restroom cleaner to store "water only" used for breads (corrected).

Moxie Nutrition

1607 Empire St., Lowell

Core violations: Test strips are wet and no longer usable. Missing thermometer in refrigerator.
