Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations: Plastic skirt for door was draped over the end touching French fried potatoes (corrected, discarded).

Core violations: Using cup as a scoop in sugar bin; must be handled with handle stored out of product. Storing cases on floor of walk-in cooler. Standing water on floor of walk-in cooler.

Burger King

Priority foundation violations: Faucet on three-vat sink is leaking; facility is using an absorbent towel to knock down spray; remove towel, repair faucet. Plumbing leak from pipe under three-vat sink; repair.

Core violations: Tiles missing under three-vat sink; replace so that it is smooth, easily cleanable. Gaps in drive-thru window which would allow entry of weather and pests; repair.

Priority violations: Breakfast items are not being cooled properly. Train employees on proper handwashing.

Priority violations: Maintain cold food at 41 degrees or below in savory cooler; kolache 54 degrees. Employee did not wash hands properly, long enough prior to putting on gloves. No soap in front handwashing sink (corrected).

Core violations: Monitor cooling units to ensure they are working properly.

Priority violations: Manager must follow and teach proper handwashing sanitation and food handling. Must use soap to wash hands; must clean hands for at least 20 seconds. May not have bare-hand contact with ready-to-eat foods. No sanitizer available in facility. Sour cream 48 degrees, rice 58 degrees, salsa 46 degrees; cold holding is 41 degrees or below.

Core violations: Prep table 47 degrees, display cooler 46 degrees; cold holding units must be able to maintain food at 41 degrees or below. May not sit frozen chicken out at room temperature. May not have unauthorized persons in the kitchen. Need to use scoop with handle for bulk food items. Employee involved in food prep/handling must wear a hair restraint; may not wear jewelry on hands or wrist except for a plain band. May not store chips in trash bag.

Priority violations: Cut watermelon 53 degrees and quiche 50 degrees in three-door prep unit; cold foods must have an internal temperature of 41 degrees or below.

Priority violations: Employees must follow proper handwashing procedures. Must wash hands prior to glove use. Sanitation buckets tested 0 ppm; should be 150 to 400 ppm for Quaternary ammonia.

Core violations: Employees involved in food prep must wear a hair restraint, i.e. beard restraint.

Olde English Fish and Chips

Priority violations: Sanitizer tested below 10 ppm; should be 50 to 100 ppm for chlorine.

Core violations: Need to have hand drying provisions at handwashing sink. Handwashing sink must be operational at all times. Employees involved in food prep/handling must wear a hair restraint. Employees involved in food prep/handling may not wear jewelry on hands or wrist except for a plain band. Need chlorine test strips. May not use bucket in food service.

Priority violations: Sanitation sink and bottles tested over 200 ppm; should be 50 to 100 ppm for chlorine.
