Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Dish machine leaking onto floor during cycle. Eggs (whole shell) stored next to cheese, milk, ice cream mix that are in cartons; some eggs broken underneath and adjacent to cheese, milk and ice cream mix.

Core violations: Test strips for sanitizer buckets discolored (corrected). Missing strip on ice machine door. Growth on ice machine baffle and walls of ice machine; not in contact with ice.

Priority violations: Provolone and pepper jack cheese packages opened exceed disposal date of seven days for ready-to-eat potentially hazardous foods held at 41 degrees or below (corrected). No sanitizer for three-compartment sink. No sanitizer for food contact surfaces (corrected).

Core violations: Mop sanitizer not functioning. Gap between counter top and wall in caulking next to hand sink. Grab-n-go products that contain two or more ingredients do not have complete label, i.e. lunch meat and cheese sandwiches and peanut butter and jelly.

Walmart Apparel Cafe

203 S.E. Martin Luther King Jr. Parkway, Bentonville

Priority violations: No consumer advisory for eggs cooked to order; advisory must contain disclosure and reminder.

Core violations: Labels on grab-n-go products such as protein packs and egg salad sandwiches do not contain complete list of ingredients; no declaration of artificial colors, weight, flavors and chemical preservatives for items in food containing two or more ingredients.

Core violations: Dust on compressor fans and food debris on floor in walk-in cooler. Holes to outside of building around piping of hot water system. Missing section of flooring exposed concrete not smooth, washable, cleanable.

Priority violations: 47 degrees milk in up-front cooler; must be kept at 41 degrees or below (corrected, discarded). Quaternary ammonia sanitizer is over 400 ppm in three-compartment sink; must be between 200 to 400 ppm per label.

Core violations: Up-front cooler is not keeping foods at 41 degrees or below. Food prep employee (manager) did not have proper hair restraint while prepping food.

Allen's Discount Grocery-Deli

Priority violations: Must label chemical container/sanitation bucket.

Core violations: Employee involved in food prep/handling can have no jewelry on hands or wrists except for a plain band.

Priority violations: Ham 57 degrees, shrimp 54 degrees, diced grilled chicken 55 degrees; cold holding is 41 degrees or below. Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. Sanitation bottle tested over 200 ppm; should be 50 to 100 ppm for chlorine. Must change gloves and wash hands when going from raw meat to ready-to-eat foods; may not just change gloves; must wash hands prior to glove use. No bare-hand contact with ready-to-eat foods. Need consumer advisory for undercooked foods.

Core violations: Prep table maintaining temperature at 56 degrees; must be able to keep food at 41 degrees or below. Employees involved in food prep/handling must wear a hair restraint.

Priority foundation violations: Low temperature dish machine is hooked up to two bottles of rinse aid and no sanitizer; dish maintenance company called.

Core violations: There is urine dried on the wall in the men's restroom; clean. Walls in dish wash area are black with grime and mold; risers in dish wash area covered with carpet. Filters missing from ventilation hood; greasicles hanging from behind fryer equipment and vent hood.

Cabana's Restaurant

115 N. Dixieland Road, Suite 9, Rogers

Priority violations: Noodles 51 degrees, rice 48 degrees, beans 50 degrees were held above 41 degrees in three-door refrigerator (corrected, product discarded).

Priority foundation violations: Three-door refrigerator was not keeping food at an internal temperature of 41 degrees or below; the thermometer inside read at an ambient temperature of 60 degrees; check equipment routinely to ensure that it is in good repair.

Carniceria Cabana's

115 N. Dixieland Road, Suite 8, Rogers

Priority violations: Chicharrones with meat 72 degrees (corrected, product discarded); must be held hot at 135 degrees or above.

Tortilleria La Popular

400 S. Eighth St. Suite F, Rogers

Priority violations: Bakery items on shelves available for customer self-service do not have a list of ingredients by weight (repeat).

Gentry Early Learning Academy

185 Rebecca St., Gentry

Core violations: Condensation leak in walk-in cooler; looks like a hole in catch pan (rusted through). Do not store items under until leaks are fixed. Floor in walk-in is rusty. Seal in floor of walk-in is coming apart, loose. Dishwasher is not draining properly; should be indirect waste drain. Check with plumber and building inspector for verification. Thermometers in walk-in are reading different temperatures; they may not be accurate anymore. Contact pest control company to develop a fly control plan.

Gentry Baseball Concession

Priority violations: Label spray bottles with contents.

Priority violations: Brown rice 100 degrees; must reheat to 165 degrees or above. No sanitizer available, using dry rag to wipe prep table after cutting chicken before cutting vegetables. Employee washing, sanitizing and rinsing food contact surfaces; must wash, rinse and sanitize. Employees only rinsing hands, not washing; no soap being used. May not thaw raw meat above ready-to-eat foods. Must date mark ready-to-eat potentially hazardous food held more than 24 hours.

Core violations: May not thaw at room temperature. Employees involved in food prep must wear a hair restraint. May not reuse single-use containers. Need to use scoop with handle, not a bowl for bulk food items.

Follow-up inspection

Priority foundation violations: Greasicles hanging from ventilation hood and equipment behind fryers.

Core violations: No chlorine residual detected in dish machine after full cycle. Dish machine primed and left with a 50 ppm chlorine residual after final rinse (repeat).

Priority violations: No sanitizer available; no chemical in building to make sanitizer. Chicken tenders 48 degrees; cold holding is 41 degrees or below. Must wash hands after touching personal clothing or face prior to handling clean dishes. Food in self-service operations must be protected.

Core violations: Side-by-side cooler unit temperature 48 degrees; needs to be able to hold food at 41 degrees or below. Employee involved in food prep handling must wear hair restraint, i.e. beard restraint.
