Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Subway

405 S. Eighth St., Rogers

Priority violations: Tuna salad 50 degrees on line for two hours; discarded. Meatballs 63 degrees in hot well; was not reheated properly; reheated to 167 degrees.

Acambaro Restaurant

215 S. Eighth St., Rogers

Priority violations: Chicken fajita 105 degrees and steak fajita 94degrees in hot well; cheese sauce 122 degrees; must be kept at 135 degrees or above after cooking to 165 degrees or above (corrected).

Core violations: Soap dispenser not working at handwashing sink (corrected).

Casey’s General Store

1531 S. Eighth St., Rogers

Priority violations: Sausage, ham, Canadian bacon opened Dec. 29, 2013, with use-by date of Jan. 5, 2014, had exceed seven days opened in refrigeration (corrected, discarded). Spray bottle not labeled with content; must be labeled on bottle.

Core violations: Thawing beef brisket at room temperature; must thaw under constant running water that constantly drains or under refrigeration 41 degrees or below (corrected).

Dickey’s BBQ Pit

1951 Highway 412 East, Siloam Springs

Core violations: Hair restraints are required for all food employees when preparing food. Beard restraints are required for food employees. Employee drink stored above establishment food in cooler; store employee food/drinks below other foods.

Core violations: Missing paper towels at handwashing sink (corrected).

Panda III

2701 W. Walnut St., Rogers

Priority violations: Raw steak 56 degrees and lo mien noodles at 53 degrees on hibachi selection island; food in cold storage must be at 41 degrees or below (corrected). Drain near ice machine is backed up with water standing on floor near ice machine; water draining from ice machine must go directly to an unclogged drain without passing over the floor (repeat). Front of refrigeration units and sides of prep tables are caked with greasy grime; these should be cleaned daily to prevent incidental contamination of food. Floor in dry storage room has exposed gravel in concrete. Vent hood has large amounts of grease built up on hood which can create a fire hazard.

Core violations: No chlorine test strips available to test dish machine.

Core violations: Growth on ice machine sleeve inside ice machine (repeat). Ice scoop being stored on top of ice machine, not a clean/sanitized surface. Boxes of beer and other drinks being stored on floor of walk-in cooler. Missing test strips for sanitizer for three-compartment sink. Missing drying rack for three-compartment sink.

Encarnacion Market

1900 S. First St., Suite 3, Rogers

Priority violations: Eggs being stored above milk and queso in cooler display (corrected).

Core violations: Missing test strips for chlorine sanitizer.

Family Dollar

1900 S. First St., Suite 6, Rogers

Priority violations: Eggs being stored above ready-to-eat butter and juices (corrected).

Core violations: Trash bin lid is not closed and trash is around trash bin.

EZ Mart

1749 W. Walnut St., Rogers

Priority violations: Wastewater from soda machine is being drained to a green detergent bucket that is overflowing; waste/drain water should be drained to a sanitary sewer and not to an overflowing bucket on the floor.

Core violations: Soda nozzles have a growth of a black mold; clean per soda company’s directions as often as necessary to prevent growth.

The Silk Road Thai Restaurant

1023 S. Eighth St., Rogers

Priority violations: Cooked ground pork 71 degrees, cooked egg 71 degrees, sprouts 48 degrees in glass door cooler; moved to other cooler; raw sprouts 59°F and raw mixed vegetables 58 degrees on line (repeat).

Priority foundation violations: Glass door cooler is not keeping foods at 41 degrees or below; prep line cooler not keeping foods at 41 degrees or below (repeat).

Core violations: Missing thermometer in glass door cooler and refrigerator and prep line cooler. Lid to chest-type freezer is repaired with duct tape; not a smooth, cleanable, nonabsorbent surface.

Kentucky Fried Chicken

507 S. Eighth St., Rogers

Core violations: Exposed plywood inside walk-in cooler by door is not smooth, cleanable, nonabsorbent surface (repeat).

Fulbright Junior High School

4303 Bright Road, Bentonville

Priority violations: No time as a control for hot grab-and-go station; need time indicated when items taken out of temperature control. Sliced lunch meat has only a seven-day hold time once original package is open or when taken out of freezer, including pizza toppings (meats) and some canned items; not to exceed manufacturer use-by date.

Core violations: Condensation buildup inside freezer around ceiling/wall in back of unit; contact installation company to check seals to ensure unit installed properly.

Firehouse Subs

2009 Promenade Blvd., Rogers

Core violations: Wiping cloths stored on cutting boards; observed one employee use dry wiping cloth to wipe knife off after making sandwich (corrected).

EZ Mart

Follow-up inspection

Priority violations: Soda machine still has ice water draining into green bucket; must drain into a sanitary sewer.

Core violations: Soda nozzles and ice chute on soda machine have mold growth (repeat, corrected).

Atlanta Bread Co.

Core violations: Soda nozzles and outside of ice chute have a slight growth of mold; soak nozzles nightly in carbonated soda water or clean per manufacturers direction to prevent mold growth.
