Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Dec. 20

Restaurant La Corona

14340 Highway 12 East, Rogers

Priority violations: Roast/prime rib 128 degrees cooking overnight in oven; must reach at least 145 degrees for 15 seconds within two hours (discarded). Ice scoop being placed on cloth between uses; must be stored on a sanitized, smooth, cleanable surface.

Finnegan’s Country Market

17723 East Highway 12, Rogers

Priority violations: Missing date marking on toppings in prep cooler; must date mark any potentially hazardous foods kept under refrigeration over 24 hours.

McAlister’s Deli

900 S.E. Walton Blvd., Bentonville

Core violations: Plastic steam table pans have cracks; once plastic pans crack they are no longer easy to clean and sanitize. Floor in front of prep unit has eroded grout; food is getting trapped in eroded grout. Floor has large amounts of food scraps and crumbs in corner near office. Dirty cloth in sanitizer bucket; sanitizer can only sanitize when used in conjunction with clean cloths and clean surfaces (corrected).

India Mart

3404 S.E. Macy Road, Suite 8, Bentonville

Core violations: Food stored in warehouse suite next door is not locked up during business hours; lock it up to prevent theft or intentional contamination.

Fred’s Hickory Inn

1502 N. Walton Blvd., Bentonville

Priority violations: Sliced tomatoes 54 degrees, cheese 51 degrees; cold holding is 41 degrees or below. Sanitation bucket tested over 200 ppm; should be 50-100 ppm for chlorine.

Core violations: May not reuse bulk food containers for food storage. Handwashing sink in prep cooking area is broken. No soap at handwashing sink by dishwashing machine.

Bentonville Butcher and Deli

1201 S. Walton Blvd., Suite 105, Bentonville

Priority violations: Chicken breast 108 degrees; hot holding is 135 degrees or above. Must date mark ready-to-eat hazardous foods held more than 24 hours.

Acropolis

905 N.W. 13th St., Bentonville

Priority violations: Must wash hands prior to glove use. Must keep handwashing sink accessible at all times. Handwashing sinks need handwashing signage. Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. Must identify chemical containers. Sanitation bucket tested over 200 ppm; should be 50-100 ppm for chlorine. No bare-hand contact with ready-to-eat foods. Eggs 55 degrees; cold holding is 41 degrees or below. No soap being dispensed into dishwasher; may not use dishwasher until corrected; must use three-compartment sink. Manager not following proper handwashing, sanitation or cleaning.

Core violations: Food out of original container must be labeled. Employee involved in food prep/handling must wear a hair restraint. Handwashing sink in back kitchen not working; need to follow directions for unit. May not reuse bulk food containers for food storage.

McDonald’s

406 S. Walton Blvd., Bentonville

Priority violations: Need to mark items under time as a control; need to discard items at end of time. Need to wash hands prior to food handling.

Core violations: Sanitation bucket tested 0 ppm; should be 50-100 ppm for chlorine. Employee involved in food prep must wear a hair restraint.

Pho Thanh

206 S. Walton Blvd., Bentonville

Priority violations: Must wash hands prior to glove use. Yellow rice 117 degrees; hot holding is 135 degrees or above.

Core violations: Must keep hand-drying provisions at handwashing sink. Need consumer advisory warning. May not store noodles in trash bag. Employees involved in food prep must wear a hair restraint. Wiping cloths need to be stored in a sanitation bucket.

McDonald’s

100 N. 46th St., Rogers

Core violations: There is dirt on floor in kitchen; floor requires frequent sweeping and mopping while workers must walk through unpaved parking lot to work and perform job functions.

A Chau Oriental Market

115 N. Dixieland Road, Suite 12, Rogers

Priority violations: Back hand sink near office is not operational; have working so that employees have multiple hand sinks available for handwashing.

Core violations: Milk, beer and other items stored in the bin used to refill ice around frozen fish at butcher seafood area (corrected). Spilled rice on floor from large bags of rice on pallets; clean and sweep often to prevent attraction of pests.

Shogun

3606 Magellan Blvd., Bentonville

Priority violations: No chlorine sanitizer detected in final rinse of dish machine; no residual detected on plastic and metal pots put in dish machine for sanitizing after the wash and rinse cycle (corrected). Large pan of white rice in walk-in cooler made on Tuesday was 51 degrees (corrected). No time for use written on sushi rice log for rice made today (repeat, corrected).

Core violations: Push for ice handle on ice machine has orange and black mold growing on it; you have to clean all parts of Coke machine to prevent growth of mold (corrected).

Shogun

Follow-up inspection

Priority violations: Large pans of rice in walk-in cooler made yesterday were 48 degrees and 51 degrees (corrected, repeat).

Core violations: Employees could not provide a thermometer to show how they are measuring the rice temperature.

Habachi Grill Buffet

102 S. 21t St., Rogers

Priority violations: No paper towels at read hand sink in kitchen (corrected).

Core violations: Bowl used to scoop white rice out of cooker in back kitchen (corrected); use a utensil with a handle that can be grabbed without touching tool.

International House of Pancakes

4606 W. Walnut St., Rogers

Core violations: Sanitizer is leaking from tube on top of dish machine when it is primed; repair.
