Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Follow-up inspection

Priority violations: Must wash hands prior to putting on gloves. May not have bare-hand contact with ready-to-eat foods. Cut leaf lettuce 60 degrees, sliced tomatoes 48 degrees; cold holding is 41 degrees or below. Sanitation bucket tested over 200 ppm; should be 50 to 100 ppm.

Core violations: Ice buildup on condenser fan in walk-in freezer; hash brown patties on floor. Single-service items, cases of lids and cups and pizza sauce stored on floor.

Priority violations: Beef fajita 100 degrees and chicken fajita meat 105 degrees in hot well; must be kept at 135 degrees or above after cooking to 165 degrees or above (repeat, corrected). Guacamole 46 degrees and pico de gallo 44 degrees in prep cooler; must be kept at 41 degrees or below.

Priority foundation violations: Door to prep cooler does not easily close.

Core violations: Missing thermometer in prep cooler and bar cooler. Prep cooler is not keeping foods at 41 degrees or below.

Kobe Sushi & Grill

1702 S. Walton Blvd., Bentonville

Priority violations: Raw beef over cooked rice. Salad bowl 49 degrees, tofu 57 degrees, rice 65 degrees, lettuce 72 degrees, cabbage 68 degrees; cold holding is 41 degrees or below. Employee must wash hands after putting on hair restraint prior to returning to food prep; must wash hands prior to putting on gloves.

Core violations: Stand-up salad refrigerator 49 degrees; walk-in refrigerator 58 degrees; cold holding is 41 degrees; units must be able to maintain food at cold holding. Facilities need to be cleaned; food debris throughout kitchen. Need an audio or visual alarm on chemical dishwasher; not able to see chemical dispensed. Must use food grade container to store food. Must store utensils handle up.

Priority foundation violations: Appetizer menu does not have a consumer advisory disclosure or reminder about items served with raw or undercooked ingredients and remind customers that consuming may increase their risk of food-borne illness.

Priority violations: Bare hand contact during sandwich assembly. 0 ppm chlorine in wipe buckets. Raw whole shell eggs stored on top of milk; two shells cracked (corrected).

Priority violations: Roundup spray next to mint syrup and creme pre-syrup (corrected).

Core violations: Top of ice machine not in contact with ice; dirt and debris on top of machine.

Carniceria Guanjuato

400 N. Eighth St., Rogers

Priority violations: Bare-hand contact when assembling tacos and food from prep station. Cook did not wash hands prior to preparing food or prior to glove use; used incorrect sink.

Core violations: No ingredient label on tortilla for sale in store.

Employee prepared pupusa with bare hands after cooking; no bare-hand contact with ready-to-eat foods (corrected).

Core violations: Handwashing sink was not accessible, filled with pots and other items (corrected).
