Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Pork 61 degrees; hot holding is 135 degrees or above. Not sanitizing food contact surfaces; no chemical for sanitizing available in facility. Must use hand-drying provisions to turn off faucet at handwashing sink. Employees need to be aware and follow proper handwashing sanitation and temperature control. May not store two-cycle oil in food areas.

Priority violations: Establishment is not following its HACCP plan for sushi rice; must cool rice from 135 degrees to 70 degrees within two hours, then cool from 70 degrees to 41 degrees within an additional four hours. Sushi rice 82 degrees on counter, discarded. (repeat, corrected).

Core violations: Missing soap at handwashing sink (corrected).

McDonald's

Priority violations: Must use hand-drying provisions to turn off faucet at handwashing sink. No sanitizer available.

Core violations: Wiping cloths need to be stored in a sanitation bucket. Ice scoop handle in the ice; needs to be stored out of the ice.

Bentonville Butcher & Deli

1201 S. Walton Blvd., Suite 105, Bentonville

Priority violations: Must use hand-drying provisions to turn off faucet at handwashing sink. Must wash hands when putting on gloves, changing gloves. Sanitation bucket tested 0 ppm; should be 200 ppm for quaternary ammonia. Lids for food containers dropped on the floor and employee picked up and placed on shelf to use. Employee's open drink on cutting/prep table; may not store open drink on work surfaces.

Core violations: Employees involved in food prep handling may not wear jewelry on hands or wrists except for a plain band. Boxes on the floor in freezer. Wiping cloths need to be stored in a sanitation bucket.

Harps Deli & Bakery

Priority violations: Must wash hands prior to glove use. Must wash hands when soiled. Diced chicken 53 degrees; cold holding is 41 degrees or below. Must date mark ready-to-eat foods that are potentially hazardous held more than 24 hours.

Core violations: Need to label food out of original container. Wiping cloths need to be stored in a sanitation bucket. May not use single use containers for continuous food storage. Water leaking by handwashing sink.

Priority violations: Must wash hands when going from taking a phone order to food prep.

Core violations: Need a scoop for cheese, not a bowl for a scoop.

Priority violations: Cookie dough in display fridge was 56 degrees; fridge was accidentally unplugged for about an hour.

Priority violations: Bare-hand contact with ready-to-eat foods; cook making sandwiches and touching bread (corrected). Cooked chili meat had no date; potentially hazardous food that is held in establishment greater than 24 hours needs a date (corrected).

Core violations: No quaternary ammonia test strips for wiping bucket. Jerk seasoning stored in single-use container; single-service items are only one-time use.
