Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted. Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Refrigeration units are not keeping food at 41°F or below (repeat). Cool foods properly.

Core violations: Establishment must have cooling units that will keep foods at 41°F or below.

India Orchard

3404 S.E. Macy Road, Bentonville

Priority violations: Keeping foods out of refrigeration during service. Foods not being held at 41°F or below. Raw eggs above ready-to-eat foods (corrected). Dish washer did not wash hands between dirty and clean dishes. Clean and sanitize all surfaces and equipment; food debris on equipment and prep surfaces. Cool foods properly; rice sitting at room temperature to cool. Employees did not wash hands between gloves changes.

Core violations: Storing food in grocery-type bags. Employee using handwashing sink for other things. Replace floor tiles as needed.

Mazzio’s Pizza

1117 N. Walton Blvd., Bentonville

Priority violations: Boiled eggs 46°F and pasta salad 46°F, diced ham 50°F on salad buffet; foods should be 41°F or below in cold storage (corrected).

Core violations: Outer surfaces of dish racks are grimy; wire racks for storage are dusty; clean. Buffet unit is not maintaining cold food at 41°F or below; maintenance arrived to adjust salad bar.

Priority violations: Utensils stored in dipper well, but water is not on; utensils are dirty. Top portion of pizza station is not holding food at 41°F or below; thermometer reads 50°F. Top portion of pasta station not holding food properly at 41°F or below. Top portion of saute station is not holding foods at 41°F or below; thermometer reads 50°F in unit. Minestrone soup held at 124°F. Tomato slicer stored dirty (corrected).

Core violations: No soap at line handwashing sink (corrected).
