Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted. Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

April 9

Priority violations: Maintain cold food at 41 degrees or below. Top pizza prep table is holding food at 40-44 degrees. Salad cooler is holding some foods at 43-44 degrees. Salad bar is not holding food at 41 degrees or below.

Core violations: Slow draining handwashing sink. Clean dough pressing machines; excessive buildup along non-food contact parts. Clean backs of microwaves; sign and cords hanging in the area; excessive food and debris; and back of smaller prep table. Clean floor and wall under and behind dish machine. Clean along the corners and edges of floor wall joints. Effective hair restraints are required for food employees.

Core violations: Leak in silver upright cooler; excessive water in the bottom of unit. Clean exhaust hood behind cooking units, all sides of cooking units, wall and floor behind and under cooking units; excessive grease everywhere. Clean shelves; excessive grease dripping from shelves. Clean window and wall behind shelving units. Replace shelf lining or clean to remove grease.

Core violations: Buckets with holes are no longer easily cleanable; cracked and broken and food and debris still in the holes.

Priority violations: Ground goat and cut goat meat pieces not packaged in an approved food contact surface. Ground goat is packaged in a black plastic trash-type bag. Cut goat parts are packaged in a regular grocery-type bag. Goat meat does not have proof of origin or invoice where product came from. Goat meat pieces packed in T-shirt bags, ground goat meat packaged in black trash bag. Goat delivered in black trash bags. Quarantined ground goat meat, goat meat pieces in T-shirt bags and meat packaged in trays until proof of origin can be obtained; ground goat meat and meat in T-shirt bags will be destroyed.

Core violations: Food stored on the floor in produce walk-in (corrected). Bags of onions stored on the floor in the back storage room (corrected). Warehouse area next to mart walls are not finished so insulation is exposed, part of the floor is not finished. Walls need to be finished to cover the exposed insulation. Pallets of food and other food storage must be stored six inches away from the wall. Clean spilled foods in warehouse.

Hapa’s Hawaiian Grill

Follow-up Inspection

Priority violations: Quaternary sanitizer is not 200 ppm.

Core violations: Seal around the handwashing sink. Finish the floor wall joint but sealing bottom of wall or installing cove base under new sink locations. Ensure water does not splash under the wall above floor drain. Fill holes in wall where handwashing sink was. Repair holes in wall by front counter.

Napolis Italian Restaurant

4408 W. Walnut St., Rogers

Priority violations: Waiter did not wash hands prior to making salad (corrected). Sanitizer at dishwasher is too strong, about 200 ppm; should be 50-100 ppm. Salad prep table is not holding food at 41 degrees or below.

Core violations: Broken thermometers in two units. Label containers with contents. Ice scoop handle laying down in ice. Ice scoop stored on top of ice machine (corrected). Need test strips for chlorine sanitizer. Beer drip pan must be emptied at least once a day.

Core violations: Monitor sanitizer to ensure the pre-mixing unit is working properly.

Sam Walton Development

2001 S.E. 10th St., Bentonville

Priority violations: Hamburgers using time as a control, no time indicated for product; need written policy. Employee did not wash hands between glove changes. Handwashing sink at coffee bar is not connected; must be connected and used.

Core violations: Excessive condensation leak in freezer. Ice is falling onto product and one case is open. Do not store open product under water leaks (repeat). Using a regular paint brush at coffee station; use only NSF approved equipment or food grade. Using a temporary wash, rinse, sanitizer set up for coffee bar. Sanitizer bucket is the same as the one used for wiping cloths. Need proper wash, rinse and sanitize area set up if going to do that at coffee bar or use alternative.

April 25

Carrabba’s Italian Grill

3300 Pinnacle Hills Parkway, Rogers

Priority violations: Employee took drink from canned beverage sitting on service line, did not wash hands prior to going to work. Appetizer station is not keeping food at 41 degrees or below. Automatic dishwasher is not reaching proper wash temperature or proper sanitizing temperature; manually sanitize until unit can be repaired.

Core violations: Handwashing sink on the line not accessible; pans of pasta sitting in front of it (corrected). Uncovered beverage on work area (corrected).

White Oak Station 23

139 N. Curtis Ave., Pea Ridge

Priority violations: Items using time as a control are not marked with the time (corrected). No date mark on hot dogs; hot dogs 48 degrees (destroyed). Cooler is not holding food at 41 degrees or below; mega Italian sandwich 56 degrees. Made-to-Go cooler is not holding food at 41 degrees or below. This cooler cannot be used for potentially hazardous foods. This cooler has a history of not working properly. Re-inspection required before using again for potentially hazardous foods. Walk in customer service case not holding food at 41 degrees.

Core violations: No cold water at the handwashing sink; handle stripped. Ice scoop stored in container with staples; container broken. Scoop cannot be stored with non-food items.

Priority violations: Prep table is not holding food at 41 degrees or below; thermometer is reading 54 degrees. Diced beef and chicken are not being held at 135 degrees or above (adjusted). Container of raw meat stored on top of boxes of tortillas which are on the floor (corrected). Employee went from dirty dishes and water to food prep. Employee was stopped by inspector and had to be told to wash hands and shown how. Bin of salt has been contaminated with something. Sugar scoop is covered with something brown (corrected). Food not used in 24 hours must be date marked.

Core violations: Flies coming in drive-through window.

Priority violations: Goat meat must be from an approved source; quarantined.

Core violations: Packaged items with no label; do not sell items that do not have a label. Kaju Katli has what appears to be silver foil on it but it is not declared on the ingredients label. Goat meat repackaged on location; no label on packaging.

Priority violations: Employee did not wash hands after breaking raw eggs (corrected). Employee did not wash hands prior to handling food (corrected). No sanitizer at three-compartment sink; bottle empty (corrected). Use proper cooling for all foods each time.

Core violations: Food employees wearing jewelry during food preparation. Check sanitizer at bar daily. Need new test strips; some not accurate out of batch. Need test strips at bar.

Comfort Suites

2011 S.E. Walton Blvd., Bentonville

Priority violations: Hot-holding unit is not maintaining sausage at 135 degrees or above; ensure product is being reheated to 165 degrees before serving. Customer refrigerator is not maintaining food at 41 degrees or below; boiled egg 50 degrees; yogurt 43 degrees, cream cheese 43 degrees. Employee does not know if dishwasher is heat or chemical sanitizer.

Core violations: Clean interior of milk dispensing unit. Milk cooler thermometer is reading 42 degrees; product is 37 degrees; check thermometer for accuracy. Dishwasher gauge is not reading proper heat sanitizing temperature but using an alternate check method; it was verified it is getting hot enough. Gauge must be accurately reading water temperature. Establishment must get another way to verify dish machine is working; it must be checked every day.

Priority violations: Using commercial cleaner with bleach mixed with water for sanitizing; not an approved sanitizer solution; must be either chlorine and water only at 50-100 ppm or quaternary ammonia solution per label for ppm (corrected). Dishwasher is not getting dish temperature to 100 degrees or above; must use three-compartment sink to wash, rinse, sanitize dishes until automatic dishwasher is repaired.

Core violations: Handwashing sink up front needs a knob repaired. Missing cover on light bulb in walk-in freezer.

Large pizza prep and cook’s prep are not keeping foods at 41 degrees or below (repeat).

John Q. Hammons Center

3201 Pinnacle Hills Parkway, Rogers

Priority violations: Employee cutting ready-to-eat cabbage using bare hands. No cold water at the handwashing sink by the ice machine. No date marking op open packages of lunch meat. Cartons of expired milk. Hot water at handwashing sink (three-compartment sink) does not turn off. Replace faucets as needed where water constantly drips and is shut off because of it. Employees cannot wash hands properly.

Core violations: Condensation leak in freezer. Sanitizer is connected to hot water; testing range is 65-75 degrees. Excessive grease and dirt accumulation behind and under cooking equipment. Bulk sugar lid has duct tape on it. Duct tape is not an approved repair method. Clean interior of coffee brewing unit in server’s station. No soap at handwashing sink in server’s station 1. No paper towels at handwashing sink in server’s station 1. No cover on coffee bins at coffee machine server’s station 1. Clean around and under ice machines and all equipment at both server’s stations.

Clean interior of ice machine and exterior, especially top doors. Flats of raw eggs above cooked pasta. Clean interior of the bar nozzle holder frequently (corrected). Lettuce is being stored in black trash bags in Brute. Do not store food in bags that are not food grade.

Core violations: Seal around server’s handwashing sink; sink loose and water dripping below. Upright salad stock cooler gasket is torn; shelves are rusty and not easily cleanable in places. Chicken upright cooler door has holes and dented making cleaning difficult. Back prep handwashing sink has no hot water. Clean under cooling units frequently. Some tiles broken in front of cooking equipment. Ice buildup freezer door walk-in freezer. Temperatures in meat cutting room 50 degrees. Clean interior of the keg cooler, especially left side corner in front. Kegs stored in front of raw meat shelves.
