RESTAURANT INSPECTIONS: March 25th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

March 1

Casey’s General Store

400 S.W. 14th St., Bentonville

Priority violations: Food prep employee was eating a sandwich in the food prep area.

Core violations: Scoop in sausage pizza toppings has handle touching sausage; must store scoop with handle out of product (corrected).

Emma Ruth’s Family Recipes

3404 S.E. Macy Road, Bentonville

Core violations: Employee doing food preparation did not have proper hair restraint (corrected). Missing quaternary ammonia sanitizer test strips.

Which Wich

1406 S. Walton Blvd., Suite 2, Bentonville

Priority violations: Running water for scoop’s dipper well was turned off (corrected). Employee, after touching sanitizer wiping cloth, did not change gloves; wash hands before changing gloves before preparing sandwiches (corrected).

Core violations: House chips and Rice Krispy treats that are prepackaged are missing labels indicating ingredients listed in order by weight and address of place of manufacturing/packaging (repeat violation).

Priority violations: Egg cartons are stored above 15-inch aloe vera leaves (corrected); store eggs so they are not directly above produce.

March 5

Coleman’s Children Academy

105 Presidential Drive, Lowell

Priority violations: Food prep employee did not know the correct range for chlorine sanitizer; must be between 50-100 ppm (corrected).

Core violations: Reusing 4-pound animal cracker containers to store sugar, 35-ounce cheese balls container to store flour; containers are not approved for multi-use.

Dollar General Store

106 N. Bloomington St., Lowell

Core violations: Missing thermometer in cooler for eggs.

Ron’s Hamburger and Chili

109 N. Bloomington St., Lowell

Priority violations: Chili 46 degrees, 48 degrees cooling overnight in walk-in cooler; must be cooled from 135 degrees to 70 degrees within two hours and then 70 degrees to 41 degrees within additional four hours. Chicken 56 degrees on counter; must be kept at 135 degrees or above after cooking to 165 degrees or above. Employee did not wash hands before putting on gloves.

Core violations: Reusing spice containers to store French fried potatoes. Duct tape being used to repair spray nozzle on three-compartment sink. Floor tile under three-compartment sink and near back kitchen door needs repair. Missing test strips for quaternary sanitizer. Using cup as a scoop in chicken container; must be handled with handle out of product. Bathroom doors are not self-closing. Lid to trash bin not closed.

Subway

200 S. Bloomington St., Lowell

Priority violations: Mouse droppings found in cabinet under soda machine.

Core violations: Knobs on handwashing sink are difficult to turn water on and off. Lid on trash bin outside is not closed. Area under soda machine in bottom of cabinet is not a smooth/cleanable/nonabsorbent surface.

The Kid’s Studio

5500 Pinnacle Point Drive, Rogers

Priority violations: Studio 9 refrigerator is reading 44 degrees.

Core violations: Remove cardboard from top freezer shelf (corrected). Thermometer in Studio 6 is more than 41 degrees; no foods requiring temperature control stored in it today; check refrigerator and thermometer for accuracy. Gasket in refrigerator Studio 7 torn; replace as needed. Some teacher personal food stored in day care refrigerators.

Grub’s Grill

3001 Market St., Rogers

Priority violations: Broken thermometer in nacho station; glass from thermometer is broken; use covered thermometers (removed). Maintain grilled onions, peppers and mushrooms at 135 degrees or above once grilled/heated (corrected). Dish washer did not wash his hands before handling clean dishes after handling dirty ones; recontaminating clean dishes (corrected).

Core violations: Portion cup in salsa on server’s table. No scoop for chips, tortilla strips or croutons; using a bowl or portion cup or bare hands to put items on plates. Drain for three-compartment sink not draining into floor drain but hitting the floor instead. No thermometer in one bar cooler with half and half. No single use towels at bar handwashing sink; using cloth towel at bar. Using handwashing sink as a dump sink in bar. No test strips for bar dishwasher.

Jack’s Bakery

1100 S.E. 14th St., Suite 8, Bentonville

Priority violations: Sausage rolls (kolaches) were 48 degrees in cake display cooler (corrected), rolls discarded; sausage rolls must be kept with an internal temperature of 41 degrees or below.

McDonald’s

400 S. Walton Blvd., Bentonville

Core violations: Missing paper towels for hand drying at handwashing sink. Missing sanitizer test strips for quaternary 200 ppm and for chlorine 50-100 ppm.

Las Palmas

301 N. Walton Blvd., Bentonville

Priority violations: Cover for grease trap pipe was broken. Flan in walk-in cooler date marked 2/24, was still being kept over seven days under refrigeration; flan was discarded (corrected). Raw beef above shrimp and milk in reach-in cooler (corrected). Raw shrimp above sour cream and cooked beef (corrected). Dish washing employee did not wash hands after handling dirty dishes and before handling clean dishes. Sanitizing wiping bucket stored above chips container (corrected). Chlorine sanitizer in wiping bucket was more than 200 ppm (corrected). Using ice for beverages after being used for cooling coils and tubes for soda/beverage machine. Beef fajita meat 124 degrees in hot well; must be kept at 135 degrees after cooking to 165 degrees or above (repeat violation). Refried beans 83 degrees, rice 70 degrees, chicken 115 degrees on stem table were not reheated to 165 degrees or above before placing in hot holding (corrected).

Core violations: Lid to trash bin outside was not closed (corrected). Missing light cover in dry food storage area (walk in).

California Cafe

Priority violations: Using time as a control for hot sandwiches. Time must be marked in a way to identify when food must be used by or destroyed by four hours after removing from temperature control (corrected).

John Brown University

2000 W. University St., Siloam Springs

Priority violations: Employee did not wash hands prior to putting on gloves (corrected).

Core violations: Floor drain in front of the steam kettles is not draining completely and the concrete is no longer smooth and easily cleanable allowing standing water; grates are rusty and not easily cleanable.

Joe Mathias Elementary School

1609 N. 24th St., Rogers

Core violations: Light shield needed over bulb in walk in without shield.

Hibachi Grill Buffet

102 S. 21st St., Rogers

Priority violations: Fried chicken at 60 degrees sitting at room temperature. Cooler reading 58 degrees; food in unit is at 56 degrees (food destroyed as needed). Dirt from shelves in reach-in are dirty and food and dirt is falling in food below. Using used plastic bags as liners between uncovered food bags that were used to hold shredded cheese. Leak in walk-in ceiling; water dropping on uncut fruit and covered containers; walk-in freezer too. No time on sushi rice (corrected). Maintain hot food on buffet at 135 degrees or above. Maintain cold food on buffet at 41 degrees or below.

Core violations: Food thawing at room temperature (moved to cooler). Clean inside upright freezer meat unit. Keep all foods covered; clean interior and shelves.

ABC Happy Kids Learning

2808 W. Walnut St., Rogers

Priority violations: Spray bottle labeled soapy water, but has another chemical in it, Tiger’s room; bottle in Koala’s not labeled. Thermometer in Koala’s room is reading 44 degrees; temperature log is not completed for any days in March.

Core violations: Gasket tearing on the freezer door of refrigerator. Personal items stored with establishment goods in pantry; designate an area specifically for personal items. Sippy cup top spouts are chewed up and no longer smooth and easily cleanable or durable; replace as needed. Cloth towels in bottom of Koala’s refrigerator.

Eureka Pizza

2119 W. Walnut St., Rogers

Priority violations: Black ash-like substance on the back portion of prep table and area behind (corrected). Top portion of prep table is holding food at about 42 degrees to 43 degrees. Pizza sauce is front fridge is 42 degrees; thermometer is reading 42 degrees. Using paint mixer attachment to mix sauce (not food grade) and paint is peeling (thrown away).

Core violations: Need to test sanitizer reach time it is made; test strips got wet and might not be accurate.

March 8

Papa Murphy’s

2006 W. Walnut St., Rogers

Priority violations: Employee did not wash hands after returning to the kitchen before going back to food prep (corrected).

Core violations: Using single-use container in flour; need scoop with handle, do not store any items in floor.

Ginnie’s Place Coffee

1200 W. Walnut St., Rogers

Priority violations: Flat of raw eggs stored on top of ready-to-eat foods on the upper portion of the prep table (corrected). Deli meats standing above the load line. Open, uncut deli meats not date marked.

Havana Tropical Grill

1207 W. Walnut St., Rogers

Priority violations: Bags of food in the handwashing sink. Thawing food at room temperature. Employee handling ready-to-eat foods with bare hands. No date marking on foods. Sour cream 48 degrees. Bag of raw chicken thawing in mop sink. Using eggs from home; eggs must be from an approved source. Need to install a handwashing sink at the bar. Handwashing sinks must be conveniently located and easily accessible.

Core violations: Beer in the bottom of beer top cooler.

March 11

Carniceria Guanajuato

400 N. Eighth St., Rogers

Priority violations: Drying chicken on racks under grill hood at room temperature; must be kept at 41 degrees or below or 135 degrees or above after cooking to 165 degrees. Cooked beef 51 degrees in small cooler in grill area; must be kept at 41 degrees or below. Chicken in heat lamps display at 115 degrees; must be at 135 degrees or above (repeat violation). Small cooler in grill area is not keeping foods at 41 degrees or below.

Core violations: Missing ingredients listed in order by weight on salsa containers and place of packaging (repeat violation). Missing light shields on fluorescent lights above produce area (repeat violation).
