RESTAURANT INSPECTIONS: July 22nd

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

June 25

McAlister Deli

900 S.E. Walton Blvd., Bentonville

Core violations: Batteries required in kitchen handwash towel dispenser. No sanitizer in wipe bucket. Uncalibrated food thermometer is 12 degrees off. Improper reheat at steam table; reheat to 165 degrees then maintain at 135 degrees. Unlabeled oil and vinegar squeeze bottles at front make table. Improper hot holding temperature in rear steam table.

Hilton Garden Inn & Bar

2204 SE Walton Blvd., Bentonville

Core violations: Apron hanging over dried/drying washware; keep all clothing in designated area. Unlabeled squeeze bottle of food product at grill; label all containers for content. Moist towel at wait station without sanitizer residual; store in sanitizer between uses. Lysol spray at wait station; use only approved for restaurant use chemicals. Bar warewasher chlorine dispenser is empty; manager replacing empty reservoir.

Holiday Inn Express

Core violations: Probed food thermometer required; digital thermometer preferred. Easily readable thermometer required in front self-service reach-in cooler. Washware drying on moist hand towels; let all washware air dry on racks.

Denny’s Classic Diner

Core violations: Rear door seals require repair to prevent insect and vermin entry.

Buffalo Wild Wings

2207 S.E. Moberly Lane, Bentonville

Core violations: Hand towels required at handwash sink beside walk-in freezer. Splash guard required at handwash beside make table. Wiping towel should be stored in sanitizer between uses. Bar reach-in cooler jug of milk should be stored at lowest shelf.

Bentonville Butcher & Deli

1201 S. Walton Blvd., Bentonville

Core violations: Moist wiping cloths at meat slicer without sanitizer residual; store in sanitizer between uses. Date and expiration markings required on all cut deli meats. Butcher processing area cooling condensation requires plumbing for splash diversion. No hand towels at butcher area handwash sink.

July 2

Table Mesa Bistro

100 E. Central Ave., Bentonville

Priority violations: Guacamole 46 degrees, hummus 46 degrees, dressing 46 degrees on large prep table top; must be kept at 41 degrees or below.

Core violations: Prep table top is not keeping foods at 41 degrees or below.

Taj Indian Cuisine

310 S. Walton Blvd., Bentonville

Priority violations: Curd rice 60 degrees; must be kept at 41 degrees or below (repeat).

Core violations: Missing paper towels at handwashing sink (corrected). Containers of food stored on walk-in cooler floor; must be six inches off floor; soy sauce stored on floor in dry good storage area. Using Styrofoam cup as a scoop in dry lentil storage. Cold buffet does not keep foods at 41 degrees or below. Food prep employee did not have proper hair restraint. Faucet on handwashing sink is loose and hard to turn on and off (repeat).

La Petite Academy

215 N. Walton Blvd., Bentonville

Priority violations: Sanitizer in three-compartment sink was at 200 ppm for chlorine; must be between 50-100 ppm (corrected).

July 3

Ken’s Hiwasse Dairy Freeze

Core violations: Pie scoop being stored in standing water; must be running water that drains constantly or at 135 degrees or above or dry and store on sanitized surface (corrected).

McDonald’s

Complaint Inspection

Priority violations: Flies in food prep area. Ice in ice machine in contact with unsanitized handle of ice scoop; must discard ice and clean and sanitize. Raw eggs above sauce packages in walk-in cooler (corrected).

Core violations: Standing water in cabinet under soda dispenser in lobby from leak. Leak under soda fountain in lobby. Storing container as a scoop in ice machine; must be handled with handle stored out of ice. Ice scoop was buried at bottom of ice in ice machine; must discard ice and clean and sanitize.

Panda III

2701 W. Walnut St., Rogers

Priority violations: Fried chicken nuggets on buffet 108 degrees; must be kept at 135 degrees or above in hot holding. Sweet and sour chicken dumplings 58 degrees; vanilla soft serve mix 57 degrees in walk-in cooler; chow mien 59 degrees in prep cooler; food in cold storage must be at 41 degrees or below. Cold items on buffet, such as dressing, are 59 degrees; if you are going to use time instead of temperature to limit bacterial growth, it must be recorded.

Priority foundation violations: Peppers in two-door reach-in growing white mold; thrown away (corrected). There is no plumbing under the three-vat sink connecting the three-compartment sink to the drain. Food scraps and debris that should be scraped from dirty dishes are on floor under sink. There are none of the hand sinks in the prep areas that are stocked with soap and disposable paper towels; all hand sinks must have warm water, soap and disposable paper towels to encourage handwashing.

Core violations: Fronts of prep sink are coated with oily grease grime; clean to prevent incidental contamination of food during preparation.

The Sugar Shack

Core violations: Plastic bags were reused for ice storage; do not reuse items designed for single service.

July 11

Panda III

2701 W. Walnut St., Rogers

Follow-up Inspection

Priority violations: Items stored in walk-in cooler with an ambient temperature of 72 degrees; cream cheese 65 degrees; apple and orange juice 72 degrees; biscuits 72 degrees had exploded in roll due to temperature (corrected); time temperature control for safety; food was thrown away unless they were delivered this morning. Prep unit 47 degrees this morning; walk-in cooler 72 degrees this morning; do no use refrigeration units that aren’t keeping food at 41 degrees or below. Apple juice and orange juice containers swollen from temperature abuse; biscuit dough swollen from temperature abuse; discarded (corrected). Grease on vent hood is long enough that they can drip on food and cause incidental contamination.

Priority foundation violations: There is a greasy black buildup on floors, walls and some equipment; clean, especially in areas near grill and dishwasher. Floor is not smooth and easily cleanable; gravel in cement floor is exposed; grout in tile floor is eroded; a non-smooth floor will trap food debris and allow the attraction of pests.
