RESTAURANT INSPECTIONS: July 15th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

June 7

Casey’s General Store

1531 S. Eighth St., Rogers

Priority violations: Spray bottle not labeled with contents (corrected). Sanitizer was more than 400 ppm for quaternary ammonia; must be 200 ppm per label (corrected). Food prep employee stated vinegar and water mix was used for sanitizing food contact surfaces (corrected).

Core violations: Hinge on prep table is broken and repaired with a screw.

Priority violations: Hot food not held at 135 degrees or above (corrected). Ice cream cooler holding mix at 42 degrees. Line water is dripping onto the rack of covered onion rings (repeat). Employee did not wash hands prior to opening an ice bag and scooping ice. Employees did not know temperature or proper reheating temperature. Ensure all employees know all aspects of the their duties.

Core violations: Leak in ice cream cooler. Condensation leak in cooler.

Priority violations: White refrigerator is not holding food at 41 degrees or below; limit the amount of food stored in it; keep most of food in big cooler.

Priority violations: Employees not washing hands prior to glove changes (corrected). Employee did not was hands at all when coming in from outside (corrected). Sanitizer too strong; should be 200 ppm (corrected). Raw meat stored above hot dogs (corrected).

Bean Palace War Eagle Mill

Priority violations: Not all items date marked. Automatic dishwasher is not dispensing enough sanitizer; should be 50-100 ppm; manually sanitize until repaired. Flat of eggs sitting at room temperature; keep butter under refrigeration; keep items at 41 degrees or below.

Core violations: Thawing raw meat in a pan of hot water on counter.

Rock Road Store

21504 Highway 12 East, Rogers

Priority violations: Water turned off at the handwashing sink in the kitchen. Maintain cold food at 41 degrees or below. Cool foods properly; gravy at 84 degrees still.

Core violations: Patch hole in floor by display case.

Camp War Eagle

Priority violations: Foods from picnic yesterday are still more than 50 degrees; items not cooled properly and if were out of the building should not even be brought back or reserved (corrected). Sanitizer out at three-compartment sink (corrected).

Core violations: Condensation leak in freezer. Clean under the dish table area, especially around the drain, baseboards and unit under sink; stagnant water and mold-like buildup; clean area properly every day.

Thailand Restaurant

104 N. 12th St., Rogers

Priority violations: No prepped crab rangoon, cut vegetables, sauces or salads are date marked. Time/temperature controlled foods held in establishment greater than 24 hours must be date marked; food that requires time/temperature control cannot be held in establishment more than seven days. Pesticide is for home use; only commercial products allowed for use in food establishment.

Core violations: Ice layer with debris covering area in white freezer. No handwashing signs posted at handwashing sinks.

Gaby’s Bakery

1803 S. Eighth St., Suite 8, Rogers

Priority violations: Employees must wash hands prior to putting on gloves (front counter).

Core violations: Clean all equipment completely under, around, top to bottom; buildup of flour, dough and debris on all surfaces. Wash, rinse and sanitize all food contact surfaces. Need test strips for bleach water.

Tortilleria Palomino’s

1803 S. Eighth St., Suite 4, Rogers

Priority violations: Employees handling ready-to-eat foods with bare hands. Sanitizer is too strong; should be 50-100 ppm (corrected).

Core violations: Handwashing sink did not look used; items in sink. Reusing single-use containers for food storage. Handwashing sink is no longer smooth and easily cleanable in tortilla room. Wall by masa mixer is no longer cleanable.

Core violations: Gap in vent hood filters. Non-food contact surfaces of cooking equipment lack cleaning. Portions of floor grout eroding.

Core violations: Non-food contact surfaces of cooking and refrigeration equipment lack cleaning. Portions of kitchen floor grout eroding. Bulk ice bin upper lids and flip top refrigerator lid are cracked. Food employee blew into disposable glove.

Priority violations: Food employee touched cooked pizza with bare hands (corrected).

Core violations: Ceiling vent shroud near three-compartment sink condensation dripping. Few floor tile coving damaged.

Priority violations: Waitress touched cooked food with bare hands (corrected). Food employee drink cup with a lid stored on food cutting board, also ate a chip with gloved hand in the preparation area (corrected).

Core violations: Manual can opener lacks cleaning, also bean storage rack in walk-in refrigerator. Plastic cup used for scooping wait station ice and bowl used for chips, also dipper well water is not running (corrected).

June 26

A Chau Oriental Market

115 N. Dixieland Road, Suite 12, Rogers

Priority violations: Hand sink not operational at time of inspection; leaking drain produce sink; three-compartment sink drain leaking. Debris on cleavers used to chop produce and meat and fish. Chemicals stored among food delivery (corrected).

Core violations: Food delivery from yesterday, boxes of food and bags of produce stored on floor. Vegetable and other debris on vegetable wrapping machine. Meat thawing open air from delivery this morning (corrected). Packaged labels on meat do not have packer name or common name of food. Utensils stored in between wall. No test strips for quaternary ammonia.

Super Mercado Taranda

1618 S. Eighth St., Rogers

Priority violations: Fruit stored with frozen meat. Need date marking on items not used in 24 hours; ham, cooked foods or lunch meat in meat case and cooler. Maintain food at 41 degrees or below; cooler may be in defrost mode. Storing raw chorizo and bacon above ready-to-eat foods. Spray bottle not labeled with content. Carne seca is a special process and requires a variance to ensure process is safe.

Core violations: Fly strips hanging over meat department work area. Can of Raid stored with spices. Reusing single-use containers for food storage. Clean mixer regularly and area around mixer. Walk-in cooler is dripping condensation, leak in big cooler. Repackaging and cutting some vegetables. Need three-compartment sink and handwashing sink in vegetable room. Remove cardboard from all shelves. Label packaged food. Walk-in cooler floor and walls no longer easily cleanable. Repair door on vegetable cooler.

Pollo A La Parrilla

2505 N. 15h St., Rogers

Priority violations: Refrigerator is not working; food is 48 degrees to 57 degrees. Owner knew refrigerator is not working but used it anyway. Food on ice not held at 41 degrees or below. Ice used is not from an approved source. Employees not washing their hands. Cool foods properly.

Core violations: Drain on back side of grill is clogged and smells very badly. Cockroaches seen during inspection.

Outback Steakhouse

4509 W. Poplar St., Rogers

Core violations: Chef’s knife stored in shoe in employee restroom (corrected). Chefs should store personal knives in areas that are approved for food contact surfaces to prevent incidental contamination. Areas under storage shelves have dirt; clean.

El Taco Nazoo 2

2510 S. Old Wire Road, Rogers

Priority violations: Cheese slices in prep unit are 78 degrees; cold foods should be at 41 degrees or below.

Core violations: Water supplied to mobile unit appears to be through a garden hose; hose for water supply should be food grade, such as that for an RV water supply.

Walmart Supercenter Deli

2110 W. Walnut St., Rogers

Priority violations: Rotisserie chicken 47 degrees; stacked above cold line in deli display; must be kept at 41 degrees or below (corrected, discarded). Gravy 128 degrees in hot holding display by sliding door; must be kept at 135 degrees or above (corrected, discarded).

McKinney Boys & Girls Club

2801 N. Walker St., Bentonville

Priority violations: Milk in stainless steel up-right cooler was at 59 degrees; it needs to be kept at 41 degrees or below.

Priority foundation violations: No paper towels at handwash sink.

Mr. B’s Steakhouse

1043 W. Walnut St., Rogers

Priority violations: Food not at 41 degrees or below in steak reach-in cooler; adjusted. Raw meat stored over raw fish in meat cooler. Raw fish stored over ready-to-eat foods in salad cooler. Clean interior of ice machine, especially the guard. Sanitizer at three-compartment sink is too strong; it is more than 400 ppm. Spray bottle not labeled with contents. Clean nozzle gun holder; ensure drain for holder should not come in contact with ice in the bottom.

Core violations: Reusing single-use containers for food storage in wait cooler. Storing ice scoop on cloth towel; store scoop on something that is nonabsorbent and easily cleanable. Repair wall behind handwashing sink; dry wall is gone and area is no longer easily cleanable or smooth. Repair wall around handwashing sink at server’s station.

Priority violations: Eggs may be cooked to order. Place consumer advisory notifying customers that eggs may be cooked to order and that consuming them may increase their risk of food borne illness on breakfast menu.

India Orchard

Establishment voluntarily closed due to excessive fly problem.

Priority violations: Excessive flies everywhere in the kitchen, on the floors, boxes, prep tables, everywhere a person works. Food contact surfaces are not clean and sanitized. Employees not washing their hands when coming in from outside. Employees not washing hands prior to and between glove changes. Employee not washing hands between dirty and clean dishes.

Core violations: Floor is not in good repair around dish area; holding water; tiles cracked or missing. Three-compartment sink drain is leaking.

Core violations: Back storage area where soda boxes for fountain drinks, flour and other prepackaged foods floor is not clean (repeat). Missing three-compartment sink for cleaning ice scoop; must wash, rinse, sanitize, air dry (repeat). Missing sleeves on Fluorescent lights in walk-in cooler and back storage area, Pepsi cooler and front display of walk-in cooler.
