RESTAURANT INSPECTIONS: January 14th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Dec. 18

Gravette Corner Store

601 First St., Gravette

Priority violations: Management is not ensuring employees are knowledgeable of proper sanitation. Sanitizer bucket tested 0 ppm. It should be 50 to 100 ppm for chlorine. Must sanitize after washing and rinsing food contact items.

Sonic Drive-In

702 First Ave., Gravette

Core violations: Employees involved in food prep must wear a hair and beard restraints.

Einstein Bros. Bagel

1728 S. 46th St., Rogers

Priority violations: Employee, after touching wiping cloth with gloved hands, did not remove gloves and wash before touching food (corrected).

Bariola’s Pizzeria

1748 S. 46th St., Rogers

Priority violations: Missing thermometer in large prep cooler. Thermometer is broken in double door reach-in cooler. Live adult roach found under double door freezer near gas stove. Sausage at 44 degrees, cut onions at 43 degrees and chicken at 44 degrees in large prep table cooler (sample taken of sausage and this was a repeat violation). All cold storage must be at 41 degrees or below. Large prep table cooler is not keeping foods at 41 degrees or below (repeat violation).

Core violations: Small jalapeno container has duct tape wrapped around it and is not in a smooth/cleanable/nonabsorbent surface. Cold water valve is not working on three-compartment sink. Five-gallon buckets of sauce on walk-in cooler floor must be stored at least 6 inches off the floor. Missing test strips for chlorine sanitizer in dishwasher (repeat violation). Reusing 5-gallon buckets to store sauce; these are not approved for multiuse (repeat violations). Beer is leaking under cabinets from dispenser in bar area. There is a pool of beer under the cabinet in the bar area.

Priority violations: Dishwasher not making temperature on final rinse; using three-compartment sink until fixed.

Core violations: Wiping cloths need to be stored in sanitation buckets (corrected).

Gingerbread Kids Learning Center

610 S.W. A St., Bentonville

Priority violations: Soy milk 48 degrees, soy dog 48 degrees. Cold holding is 41 degrees or below. Sanitation bottle tested over 200 ppm. It should be 50 to 100 ppm for chlorine.

Core violations: Refrigerators temperature is 48 degrees. It should be able to maintain food at 41 degrees or below. Need chlorine test strips.

Waffle House

2308 S.E. Walton Blvd., Bentonville

Priority violations: Tomato slices 44 degrees and diced onions 49 degrees on cook’s prep. They must be kept at 41 degrees or below or use time as a public health control (with policy, procedure and records) (discarded, corrected).

Core violations: Storing ice scoop on top of ice machine; not a sanitized surface (repeat violation; corrected). Sleeve of ice machine needs to be cleaned. Missing test strips for chlorine sanitizer.

Bean Palace

11045 War Eagle Road, Rogers

Priority violations: Beans 48 degrees after being cooked yesterday; cool items properly; procedure is from 135 degrees to 70 degrees in two hours or less and from 70 degrees to 41 degrees in four hours or less. Automatic dishwasher is not working properly; no chemicals being dispensed; wash and rinse manually.

Restaurant La Corena

Priority violations: Dumping mop water outside; must dump in designated mop sink.

Core violations: Standing water on walk-in floor.

Tony C’s Italian Gardens

14528 E. Arkansas 12, Rogers

Core violations: Reusing 5-gallon buckets to store sauce in outside walk-in cooler; these are not approved multiuse containers. Employee washed hands in three-compartment sink; must wash hands in designated handwashing sink. Floor under bar in restaurant area is not a smooth/cleanable/nonabsorbent surface.

Dollar General

17691 Marshall St., Garfield

Core violations: Milk cooler thermometer 46 degrees; monitor to ensure is working properly; may be in the defrost mode.

God’s Pantry

17938 Dennis Mitchell Road, Garfield

Priority violations: Evidence of rodent droppings found in packaging room. Clean all areas to ensure evidence is old and not a reccurring problem. Pest company was out about two weeks ago to treat pest issue. Train all volunteers on how to sanitize.

Dec. 21

Mister B’s Steakhouse

1043 W. Walnut St., Rogers

Core violations: Repair walls around both handwashing sinks. Walls are no longer smooth and cleanable. Replace/repair tile that is broken (work already scheduled).

Maria’s Mexican Restaurant

2503 S. Walton Blvd., Bentonville

Priority violations: No sanitizer in back three-compartment sink. Employees with cuts on hands or arms must cover cuts with an unpenetrable bandage so handwashing is still done without getting wound wet.

Core violations: Back handwashing sink not accessible. Uncovered cut pineapple in cooler; keep covered. Need handwashing signs at handwashing sinks.

Cook’s Natural Market

726 W. Walnut St., Rogers

Priority violations: Must date-mark ready-to-eat potentially hazardous foods that are held more than 24 hours. Must have sanitizer available.

Core violations: Boxes on the floor in the freezer.

Pupuseria Salvadorena

1610 S. Eighth St., Rogers

Priority violations: Chicken tamales 63 degrees and beef tamales 65 degrees that were made yesterday; cold holding is 135 degrees to 70 degrees in two hours and 70 degrees to 41 degrees in four hours or less. Beef tamales 105 degrees. Hot holding is 135 degrees or above. Must wash, rinse and sanitize food contact equipment. Sanitation bottle with bleach tested greater than 200 ppm. Should be 50 to 100 ppm for chlorine.

Core violations: May not have bare hand contact with ready-to-eat foods. Need quaternary ammonia test strips for sanitizer at three-compartment sink. Deep freezer lid is broken.

Thailand Restaurant

104 N. 12th St., Rogers

Priority violations: Employee with Band-Aid on finger must wear gloves. Must sanitize after washing and rinsing food contact items. Brown rice 105 degrees. Hot holding is 135 degrees or above. Sanitation bucket tested 0 ppm. Should be at 50 to 100 ppm for chlorine.

Core violations: Wiping cloths need to be stored in a sanitation bucket. May not reuse single-use items. Must use food-grade containers for bulk food. Back door needs to be fixed.

Hog Wild General Store

180 N. Main St., Cave Springs

Core violations: Reusing single-use containers for food storage. Store single-use silverware handle up only to prevent contamination from customers.

The Creek Golf Course

1499 S. Main St., Cave Springs

Priority violations: Storing raw meat and eggs below ready-to-eat foods (corrected). Some expired luncheon meat in refrigerator (corrected).

Carrabba’s Italian Grill

3300 Pinnacle Hills Parkway, Rogers

Core violations: Leak in drain in three-compartment sink. Grate at soda machine is rusty and no longer smooth and easily cleanable.

Metro Italiono Restaurante

3607 S.E. Metro Parkway, Bentonville

Priority violations: Meatballs stored above the food line were 44 degrees (corrected). No date-mark on most potentially hazardous foods.

Core violations: Monitor the quaternary sanitizer to ensure that it is dispensing properly.

Panda III

2701 W. Walnut St., Rogers

Priority violations: No sanitizer detected; sanitizer bucket was not attached to dish machine; employee stated business ran out yesterday and had not refilled it (corrected). Will use a sanitize sink at three-compartment sink at 50 to 100 ppm chlorine until sanitizer or dish machine is replaced. Raw eggs stored over canned vegetables in two-door refrigerator (corrected). No written record of when sushi items are made or put on display in buffet. Emergency Health Services will work with staff to explain requirements in native Chinese language. No soap or paper towels available at rear hand sink near walk-in and three-compartment sink.

Core violations: Floor in kitchen is not easily cleanable. Areas with tiles have eroded grout and are missing tiles. Areas that are concrete are rough with a gravel-like texture that is difficult to clean. The plumbing in the three-compartment sink empties to floor drain. Emergency Health Services will work with facility to find basic sanitation information in Chinese.

Las Fajitas

1402 Shane Lane, Bentonville

Priority violations: No chlorine detected in sanitation sink of back three-compartment sink (corrected). Sanitizer should be used at 50 to 100 ppm for chlorine.

Core violations: Handwashing sink in kitchen has ice in it. Dumping items down hand sink may be reason the front hand sink is slow to drain. Use handwashing sinks for handwashing only to prevent incidental contamination of hands (corrected).

Oriental Express

Core violations: Dirty floor areas under prep tables; clean to prevent attraction of pests.

Smokin Joe’s Ribhouse

2504 E. Central Ave., Bentonville

Core violations: Hole 12 to 15 inches tall in wall next to walk-in cooler. Hole in wall in corner of wall near freezer and three-compartment sink. Physical facilities should be maintained intact to prevent the possible harborage of insects.

Pinnacle Golf and Country Club

Priority violations: Sanitizer bucket tested over 200 ppm. Should be 200 ppm for quaternary ammonia.

Las Palmos

Priority violations: Must wash hands prior to touching food. Wait staff is handling tortillas without washing hands. Employees must wash hands prior to cutting fruit. There is no handwashing sink in the bar area, so fruit is cut in the kitchen. No paper towels at handwashing sink in kitchen. Chicken 117 degrees and rice 118 degrees. Hot holding is 135 degrees or above.

Core violations: Plumbing from drink machine must drain over a drain.

Shogun Japanese Steakhouse

3606 Magellan Blvd., Bentonville

Priority violations: Sanitation sink tested 500 ppm. Should be 200 to 400 ppm according to chemical bottle. Sanitation bucket tested 0 ppm. Should be 200 ppm for quaternary ammonia. Sushi rolls made with rice from unknown time mode (all items made with above rice were thrown away).

Core violations: Employees involved in food prep must wear hair and beard restraints. Employees involved in food prep may not wear jewelry.

Priority violations: Cool foods from 135 degrees or below in two hours or less and from 70 degrees to 41 degrees or less in four hours.

Core violations: Reusing a chicken stock container. Do not reuse single-use containers for food storage. Sanitizer for wiping cloths is too weak; it should be 50 to 100 ppm.

Hibachi Grill Buffet

Priority violations: Fruit salad tub stored on floor (corrected). Pans of food stored on top or in uncovered pans of food. Foods must be covered when in walk-in storage. Date marking required on all potentially hazardous foods that are not used in 24 hours. Tubs of food stored in other tubs of uncovered food in freezer. Maintain hot food on buffet at 135 degrees or above (reheated). Walk-in is not holding food at 41 degrees or below.

Core violations: Re-using single use containers (soy sauce) for food storage. Leak in the drain of three-compartment sink.
