Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Salad bar is not holding food at 41 degrees or below; keep items on the bar stirred; bottom of food is below 41 degrees, but the top half portion is not.

Core violations: No paper towels at the handwashing sink (corrected). Clean wall behind the back prep table; food and grease debris on the wall. Employee using dish sink to wash hands. Hands should be washed in a sink specifically for hand washing. Clean floor and wall in storage/proofer room. Coffee mugs and pitchers stored on top of ice machine; they should be stored in or on something to protect them from contamination. Clean around the soda machine regularly. Replace broken/missing floor tiles under ice machine.

Priority violations: Ice only works when it completely surrounds the food items (sour cream at 51 degrees) (destroyed). Gravy still at 63 degrees after about four hours; cool to 41 degrees below in two hours or destroy. Food must be stirred to be effective in holding temperature (corrected). Macaroni and cheese not being held at 41 degrees or below (destroyed).

Core violations: Salad cooler is not holding food at 41 degrees or below (service call made during inspection) (food destroyed as needed). New policy: Cut tomatoes and cut leafy greens now required to be maintained at 41 degrees or below. No bare hands in contact with ready-to-eat foods.

Blaze’n Burrito Two

Priority violations: Maintain hot food at 135 degrees above or above; chicken was 127 degrees and beans 123 degrees; the food was destroyed. Sanitizer is not strong enough; needs to be at 50 to 100 ppm. No sanitizer in the wiping cloth bucket. Date mark foods consistently that are held for more than 24 hours. New policy: Cooling requirements are that foods have to be cooled from 135 degrees to 70 degrees in two hours or less and from 70 degrees to 41 degrees or below in four hours or less.

Priority violations: Cooler No. 208 is not working properly; thermometer reading 44 degrees and the food is registering 43 degrees. Not all items are date marked; several are missing the dates. Out-takes cart is not holding food at 41 degrees or below; chicken Cordon Bleu was at 48 degrees. Food on serving line is not at 41 degrees or below; ice was below the pan level. Hot grab and go case was not holding some foods at 135 degrees or above (chicken nuggets at 117 degrees and breakfast sandwich at 127 degrees). Food stored in reach-in cooler was not cooled properly; gravy is at 108 degrees, eggs at 77 degrees and sausage at 76 degrees.

John Q. Hammons Center

Priority violations: Some items in prep cooler are not at 41 degrees or below; possible improper cooling.

Core violations: Condensation leak in freezer; a pan is catching drips. Clean or replace rusty shelves in service cooler. Final rinse/sanitizing gauge is not working at all. Establishment uses an alternative checking method.

Embassy Suites Hotel

3303 Pinnacle Hills Parkway, Rogers

Priority violations: Reach-in cooler at Caffeina’s is not holding at 41 degrees or below. Clean interior of ice cream freezer. Dead insects in cooler. Clean/replace the gasket-moldy. Clean between ice cream freezer and prep table. Mushroom stuffing dated Nov. 17, 2012, and the other had no date at all. No date marking on sliced meats (meat counter).

Core violations: No thermometer in Caffeina’s milk cooler; check daily. Clean the interior, especially the shelves, in Caffeina’s milk cooler. Allowing customers to use some products (half and half milk) and then putting those items away in the establishment cooler and using the same products. No thermometer in breakfast-to-order coolers. Replace dishwasher if it cannot be repaired; excessive water leaking from the door; it should not leak that way. Replace water curtains so they work properly; curtains are cracked and brittle.

Basil’s Cafe

Priority violations: Inconsistent date-marking.

Core violations: Clean all cooling units inside and out. Monitor back cooler to ensure it holds foods at 41 degrees or below. No permit posted.

Theo’s Restaurant

Priority violations: Chicken stock at room temperature (destroyed). Top of salad prep table not holding at 41 degrees or below. Sanitizer at three- compartment sink is not dispensing. Inconsistent date marking. Not all items are date marked.

Core violations: Need test strips for quaternary ammonia sanitizer; check daily.

Market Place Express

Priority violations: Pasta cooler is not holding food at 41 degrees or below so foods in drawers and top of unit are not being held in proper temperature (repeat violation). Back of slicer blade is not clean. Automatic dishwasher was not sanitizing; machine primed; monitor to ensure machine holds prime (corrected). Hot foods on one section were not 135 degrees or above (no water in hot unit); food was destroyed.

Priority violations: The reach-in in the front line is not holding food at 41 degrees or below; unit thermometer is reading 44 degrees.

Core violations: Replace floor under the fryer so that it is easily cleanable. Excessive grease collecting under fryer. Repair it in a way so that the repair actually lasts (90 days). Interior of ice cream machine is rusty so replace as needed because it is no longer smooth and easily cleanable (90 days). Expired permit. Permit not posted. Grill brush is no longer easily cleanable (thrown away). One sink in men’s restroom is not working (has two sinks); repair or remove (90 days). Slight sewer gas smell in dining room.

Kentucky Fried Chicken

Core violations: Buckets for to-go clam shells stored on top of reach-in covered with cardboard and duct tape (corrected); this is not easily cleanable. Ice coming from between freezer and walk-in cooler.

Priority violations: Ice cream cooler is not holding product at 41 degrees or below. When using ice to keep foods cold the ice must be at or above the level of food to be effective.

Core violations: Leak in condensation drain line in walk-in (repeat violation). Repair/replace broken and cracked tiles through out and especially around the ice cream machine. Standing water around floor drain.

Priority violations: Dishwasher is not holding a prime; monitor carefully to ensure dishes are sanitized (service call made).

Core violations: Condensation in freezer. Box of pizza dough stored open under leak. Do not store open items under leak. Repair leak as soon as possible (within 10 days). Ice machine lid is broken and no longer easily cleanable; replace (60 days).

Duffer’s Cafe

638 W. Lancashire Blvd., Bella Vista

Priority violations: Some food stored above the load line or above the ice level; food out of temperature for this reason; sausage at 43 degrees and 47 degrees; destroyed as needed. Ensure proper cooling of food prior to putting in cold holding. Employees have not effectively restrained hair and touched their hair and pushed up glasses and did not wash hands. Spray bottles not labeled with contents.

Clean all food holding equipment and prep tables inside and out, also clean the walk-in shelves. Sauce and vegetable cooler is not holding food at 41 degrees or below. Dishwashing machine is not draining properly. Automatic dishwasher is not dispensing sanitizer.

Core violations: Repair lights so that the lights work in the back room. Walk-in cooler is leaking badly. Condensation leak in freezer. Beard restraints required for food employees. Foods not used in 24 hours that require refrigeration for safety need to be date marked.

Priority violations: Handwashing sink blocked by trash can (corrected). Employee using phone and did not wash hands prior to going back to food preparation (corrected). Crab mix (California roll) not at 41 degrees or below; cool properly. Raw steak package stored on top of vegetables (corrected). No time on item using time as a control out front. Items in back are timed at 2:30 and food is still there. Time log for rice is not complete for afternoon this day.

Follow-up inspection

Core violations: Food on buffet bar needs to be protected. Sanitation bucket tested 50 ppm and it should be 200 ppm for quaternary ammonia. Management needs to ensure employees follow proper sanitation, cold holding and food protection. Cream cheese at 67 degrees and cold holding must be at 41 degrees or below.

Priority violations: Utensils such as scoops and blades in bucket on rack near dish machine have black ash on them (corrected by washing).

Core violations: Walls have streaks where food debris has built up over time. Green slime on floor tile near three vat and dish machine. Rack holding soda boxes is rusty. Racks holding bread and in walk-in cooler are grimy. Wax paper under pans in racks under bread and in walk-in cooler are soiled.

Zary’s Acropolis Restaurant

905 N.W. 13th St., Bentonville

Priority violations: No consumer advisory is listed on breakfast menu though eggs are offered to cook to customer preference. Menu must identify items that may be raw or undercooked and remind customers that consuming these items may increase your risk of food-borne illnesses. Will have this done by Feb. 1, 2013.

Core violations: Dish machine has an excessive buildup of lime scale. Single-use Tzaiki buckets used to store items in freezer. Dispose of single use buckets as they are not designed for re-use or easy to sanitize. No health department permit is posted. It is a violation of the law to operate without a valid permit. Contact the Little Rock office to ensure that you are in compliance with the law.

Carniceria Cabanas

Core violations: Cheese at 44 degrees and ham at 44 degrees; cold holding must be at 41 degrees or below. Must date mark ready-to-eat foods that are potentially hazardous if held more than 24 hours.
