RESTAURANT INSPECTIONS: February 25th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Feb. 8

Acambaro

301 S. Walton Blvd., Bentonville

Priority violations: Storing spatula in standing water in bucket at room temperature between uses; must be kept dry and clean between uses and sanitized at least every four hours of use or between use.

Core violations: Missing soap and paper towels at handwashing sink (corrected). Concrete around drain for automatic dishwasher needs repair. Thawing raw beef in sink at room temperature without running water over top; must either thaw in refrigeration at 41 degrees or below or under cool running water that continuously drains (corrected). Food prep employees did not have proper hair restraint.

Walmart Deli & Bakery

Complaint inspection

Priority violations: Knives being stored in sanitizer water between uses; must be kept dry and clean between uses and sanitized and air dried at least within four hours (corrected)

Feb. 13

Papa John’s Pizza

2810 W. Walnut St., Rogers

Priority violations: Employee did not wash hands after answering phone before returning to food preparation. In-use utensils must be washed and sanitized every four hours. Employees must wash hands at approved handwashing sink, not in the bathroom.

Core violations: Keep restroom door closed at all times.

Chutny’s Indian Restaurant

2850 W. Walnut St., Rogers

Priority violations: No paper towels at handwashing sinks. Prep table is holding food at 50 degrees (repeat violation). Date marking is required on foods not used in 24 hours. Automatic dishwasher was not sanitizing (corrected).

Core violations: Spices do not have tops or covers on containers; keep items covered.

Barnes & Noble Booksellers

261 N. 46th St., Rogers

Priority violations: Can of whipped cream sitting at room temperature. Back cooler is not holding food at 41 degrees or below. Quiche in front cooler 46 degrees -48 degrees (destroyed); do not store potentially hazardous foods on top shelf in food display unit. In-use utensils must be washed and sanitized every four hours.

Core violations: Top of ice machine is leaking onto the bottom part of machine. Soup thawing at room temperature; do not thaw at room temperature, thaw under refrigeration.

Feb. 14

Papa John’s

231 W. Walnut Blvd., Suite 5, Bentonville

Priority foundation violations: Person in charge did not know counters in food prep area must be sanitized after cleaning and at least within every four hours of operation.

Core violations: Hot water not available at handwashing sink up front; hot water available at other sinks. Missing working thermometer in small cooler for dipping sauces.

Little Caesar’s

Core violations: Clean rack covers; food on covers. Clean exterior of buckets and other lids.

Cherry Berry

200 Progress Ave., Siloam Springs

Priority violations: Maintain yogurt at 41 degrees or below; keep product at cooling level.

Elizabeth Richardson Center

1300 N. Patriot Parkway, Siloam Springs

Priority violations: Other chemicals store in bottles labeled another chemical in laundry room (corrected).

Core violations: Clean out refrigerator in the Dancing Turtles room.

Waffle House

2823 E. U.S. 412, Siloam Springs

Priority violations: Walk-in thermometer reads 45 degrees; butter is 44 degrees ; internal thermometer is 50 degrees; monitor to ensure not in defrost (unit at 31 degrees at end of inspection).

Core violations: Clean back room, especially around back door and around/under three-compartment sink. Keep interior of dishwasher clean. Remove unused/broken equipment from the back room. Clean interior of waffle reach in, dirty. Dishwasher gauges are not reading the accurate temperatures.

Feb. 15

Walmart Neighborhood Market Deli

1817 S. Eighth St., Rogers

Core violations: Floor tile in walk-in cooler needs repaired/replaced in order to have a smooth/cleanable/nonabsorbent surface.

Carousel Castle

2703 N. 13th St., Rogers

Priority violations: Raw eggs stored above ready-to-eat foods. No sanitizer in spray bottle (corrected).

Kobe

1311 S. Walton Blvd., Bentonville

Priority violations: Employee using a cloth towel to dry his hands. Employee did not wash hands after coming in from outside before going back to food service (sushi chef). No bare hand contact with ready-to-eat foods; sushi chef cutting cucumbers with bare hands. Raw meat stored above foods not cooked in reach-in (corrected). Raw chicken on top of white tuna; do not store raw meats with or next to or on top of sushi fish (corrected). Raw egg sitting at room temperature (destroyed). Sushi refrigerator is not holding food at 41 degrees or below.

Core violations: Sesame shaker is being held together with tape. Personal food stored with restaurant food. Reusing single-use containers for food storage.
