RESTAURANT INSPECTIONS: February 18th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Feb. 5

India Orchard

3404 S.E. Macy Road, Suite 24, Bentonville

Priority violations: Handwashing sink clogged; not draining at all (corrected); appears handwashing sink is being used for dishes. Evidence of mice in multiple places in the kitchen (pest control has been contacted). Maintain cold foods at 41 degrees or below at all times.

Core violations: Clean kitchen and store room completely, under all equipment, all walls and all shelving. Clean walk-in shelves and walls/floor. Store all food at least six inches off the floor.

Core violations: Floor under three-compartment sink is peeling, causing standing water buildup.

Tortilleria La Popular

400 S. Eighth St., Suite F, Rogers

Priority violations: Raw chicken being stored above pan of guacamole in walk-in cooler (corrected). Spray bottles not labeled with content (repeat violation, corrected).

Core violations: Missing soap and paper towels at handwashing sink in prep area (corrected). Sauce in large containers being stored on floor. Reusing five-gallon buckets in walk-in cooler; not approved multi-use containers (repeat violation). Prepackaged desserts and salsa containers in customer grab-and-go cooler on sales floor are missing labels indicating ingredients listed in order by weight and place of packaging/manufacturing.

Acambaro Restaurant

215 S. Eighth St., Rogers

Priority violations: Beef fajita 91 degrees and chicken fajita 85 degrees in hot well; must be kept at 135 degrees or above after cooking to 165 degrees (corrected). Tortillas in package being stored on top of raw beef; raw chicken being stored above raw beef (corrected). After touching raw egg yolk, employee did not take gloves off and wash hands before handling vegetables (corrected).

Core violations: Missing paper towels and soap at handwashing sink in cook prep area (corrected). Floor tiles missing or need repaired near steam table; no longer smooth/cleanable surface. Missing floor tiles near water heater. Employee did not have proper hair restraint while preparing food. Storing spatulas in water between uses; must be clean and dry before use and washed, rinsed, sanitized every four hours (corrected).

Shogun Express

1401 S. Walton Blvd., Bentonville

Priority violations: Fried rice sitting at room temperature; no time indicating time as a control is being used. Rice in walk-in is still 44 degrees to 47 degrees from the day before; rice will be destroyed as needed; cool foods properly. Employee went from dirty dishes to food preparation without washing hands (corrected). Sushi rice log has not been completed for a few weeks. No written policy for time as a control.

Core violations: Need new thermometers; some not accurate.

Flying Burrito

1401 S. Walton Blvd., Bentonville

Priority violations: Tomato dicer is not clean, yet it was put away.

Core violations: Storing knife between prep tables; not clean. Chip scoop laying on top of chips. Leak under three-compartment sink (drain); work order submitted. No hot water handle on one faucet at three-compartment sink; work order submitted.

Priority violations: Sanitizer tested over 200 ppm; should be 200 ppm.

Core violations: Floor in dish room is no longer smooth and easily cleanable; concrete is no longer sealed and water is able to pool.

Apple Blossom

Priority violations: Delivery company placed boxes of raw meat on top of a box of celery and raw pork on top of a box of dairy.

Shipley Donuts Siloam

271 W. U.S. 412, Siloam Springs

Priority violations: No written record of when kolaches were removed from temperature control (corrected).

Core violations: Red trays for display of kolaches in lobby have a plastic basket weave pattern that is not easy to clean; use papers with these trays so the paper is the food contact surface.

Wendy’s

2200 W. Walnut St., Rogers

Priority violations: Prep tables are not holding food at 41 degrees or below. Employee did not wash hands before handling ice after taking out the trash. Drive-thru reach-in cooler thermometer is reading 44 degrees ; creamer is 45 degrees. Vanilla frosty mix in hopper is 44 degrees .

Core violations: Cart of salad toppings sitting at room temperature while employee was finishing cutting lettuce; foods need to be put in the refrigerator as soon as that item is finished being prepared. Condensation leak in walk-in cooler drain line (repeat violation); food not stored under leak. Condensation leak in drain of freezer; food is being stored under leak.

Bellview Elementary School

Priority violations: Dishwasher not making final rinse temperature; will use three-compartment sink until fixed.

Core violations: Rinse gauge not working on dishwasher.

Core violations: Raw food in prep areas.

Feb. 12

Pea Ridge High School

781 W. Pickens Road, Pea Ridge

Priority violations: No time as a control policy (log). Sliced turkey in walk in is still 48 degrees ; cool properly; slice in batches (corrected).

Core violations: Personal food items stored with other food items; store personal separate from other foods (corrected). Ice paddle stored on the floor in a container in the freezer; do not store on the floor; container holding the ice paddle has excessive debris in it. Excessive condensation in freezer. Rubber cove base is falling off in dish room. Floor drain in dish room is not flush with the floor so it does not drain properly; water stands around drain. New floor is peeling in dish room. Clean interior of freezer; dead bugs in the corner by the door.

Pea Ridge Primary School

Priority violations: No use-by date on open carton of liquid eggs.

Core violations: Wash temperature is not reaching 160 degrees; adjust as needed; monitor regularly to ensure it is working properly.

Priority violations: Must mark items under time as a control.

Core violations: Gauges for dishwasher are not working; need to be able to know when hot water sanitation dishwasher is working. Managers in food service working on the prep line must wear a hair restraint.
