RESTAURANT INSPECTIONS: February 4th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Bentonville High School North

Priority violations: Portioned sour cream is 48 degrees; maintain 41 degrees or below. Some of deli meat exceeding use-by-date in walk-in (corrected). Nugget at the DOD Express hot box 118 degrees. Dish washer did not wash hands after handling dirty dishes before handling clean dishes.

Core violations: Check accuracy of thermometer frequently.

Priority violations: Multiple date labels on the same container and multiple containers; completely remove old labels. Employee did not wash hands between glove changes (corrected).

Core violations: Reseal around the mop sink; clean area well to ensure a good seal; possible mold around base of wall kit floor joint.

Core violations: No detergent in automatic dishwasher; alarm does not sound; should be some audible or visual alarm to indicate when detergent is not dispensing.

Priority violations: Clean interior of ice machine. Clean pan used for ice scoop regularly to prevent accumulation of dust/debris.

Core violations: Thermometer under the counter reach-in is broken. No thermometer in drive-through beverage cooler. Clean floor drain of soda machine in drive-through; check the drain of the front soda machine.

Dink’s BBQ

3404 S.E. Macy Road

Priority violations: Chlorine sanitizer in bucket was at 200 ppm; must be between 50-100 ppm (corrected).

Core violations: Reusing five-gallon pickle buckets to store sauces; not an approved multi-use container (repeat). Back handwashing sink is not working (repeat). Employees doing food prep did not have proper hair restraint (repeat).

White Oak Station

Priority violations: Meat loaf 127 degrees, gravy 123 degrees, chicken 119 degrees in hot holding display; must be kept at 135 degrees or above (repeat).

Core violations: Ice scoop being stored on top of ice machine; not a sanitized surface (repeat). Soda spillage in display walk-in cooler on floor. Missing time as a public health control records for hot holding Grab-n-Go.

Jan. 24

Westside Elementary

2200 W. Oak St., Rogers

Core violations: Shelves in walk-in are rusty, no longer easily cleanable, smooth; debris could fall from one shelf onto containers of food below; replace or recoat (60 days). Exposed insulation covering dish machine pipes; cover/replace as needed to prevent contamination of clean dishes; pans ding pipes when slid out of dish machine (two weeks).

Rogers High School

2300 S. Dixieland Road

Priority violations: Items on garden bar must be held at 41 degrees or below. When using ice to keep foods cold, ice must surround product to be effective. Portable hot box is not holding food at 135 degrees or above. Dishwasher is not sanitizing at the correct temperature; check wash temperature often (manually sanitizing until repaired). Remind employees to get clean gloves after they have washed their hands.

Sugar Creek Elementary

Core violations: Dishwasher is leaking and leaving a pool of water in dish room.

Core violations: External thermometers in milk coolers are not giving an accurate reading; one reading 46 degrees, the other one 42 degrees.

Priority violations: Maintain hot food at 135 degrees or above. Maintain cold food at 41 degrees or below; customer service cooler holding at 44 degrees; big fridge holding at 42 degrees; adjust as needed or replace equipment as needed; milk in upstairs cooler 45 degrees.

Core violations: Need new thermometer for downstairs big fridge; broken.

Dickey’s BBQ Pit

Priority violations: Clean and sanitize all items not held at temperature every four hours (knives, cutting board, salad bowl). Employees must wash hands between glove changes (corrected).

Core violations: Using tape to keep inside of prep table lid together; repair/replace as needed (30 days). Need an ice scoop. Clean exhaust vents.

Priority violations: Gravy cooling in deep pan in reach-in cooler; cool rapidly in ice bath or in shallow pan so food is below 41 degrees within four hours (corrected).

Priority violations: Person in charge did not know that utensils and scoops used for hot dogs and ice were to be washed, rinsed and sanitized; store uses three-compartment sink next door. Tongs used for hot dogs and scoop used for ice were not sanitized after washing.

Core violations: Missing test strips for chlorine sanitizer.
