Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Happy Trails Corral Daycare

Priority violations: Sanitizer tested more than 500 ppm; should be 200 ppm for quaternary ammonia.

Ron’s Hamburger and Chili

Priority violations: Employees must wash hands when going from raw hamburger prior to touching cheese. Employee must wash when going from dirty to clean dishes. No bare-hand contact with ready-to-eat foods is required. Thawing chicken in mop sink. Sanitation bottle has so much chlorine. It is bleaching out test strips; should be 50-100 ppm; chemical bottles need to be labeled. Employees need to know proper handwashing, sanitation.

Core violations: Handwashing sink filled with sponges and scrubbers. May not thaw at room temperature; must thaw in refrigeration. Employees involved in food prep must wear a hair restraint, also beard restraint, when necessary. Employees involved in food prep may not wear jewelry on hands or wrist except for a plain band. Wiping cloths must be stored in a sanitation bucket. Ice scoop may not be stored in ice.

Priority violations: Maintain meats, cut tomatoes and cut lettuce at 41 degrees or below. Mayonnaise sitting out at room temperature; maintain 41 degrees or below.

Core violations: Using pool test strips for bleach water. Need test strips for bleach water. Bleach should be 50-100 ppm.

Priority violations: Open drink on prep table; drink on bread rack above bread.

Core violations: No beard restraints.

Priority violations: Sanitizer in dishwasher is not the correct concentration (establishment says they do not use it). Sanitizer at three-compartment sink is not strong enough; contact company to adjust as needed.

Core violations: Bag of ice stored in with loose ice; do not store bags of ice with loose ice to prevent contamination.

Supermercado La Michoacana 2

Priority violations: Meat next to chemicals. Bacon and chorizo stored above tortillas. Some medications not allowed to be sold.

Core violations: Meat stored in grocery-type bags in freezer. Meat thawing at room temperature. Handwashing sink did not appear to be used; said washed at three-compartment sink. Catfish not labeled properly. Label tortilla bags.

Priority violations: No sanitizer available.

Core violations: Need quaternary ammonia test strips. No paper towels at handwashing sink. Bathroom door needs to be self-closing.

Priority violations: White gravy 117 degrees; hot holding is 135 degrees or above; sausage 109 degrees. Sanitation sink tested more than 200 ppm; should be 50-100 ppm for chlorine. Core violations: Employee involved in food prep must wear a hair restraint, i.e. beard restraint. Taped, ripped plastic cover is not smooth and easily cleanable. Trash area needs to be separate from food area.

Priority violations: Canadian bacon stored above the load line; the part above is 44 degrees; lower half is 39 degrees (corrected). Lasagna cover, lid has something spilled on it. Salad cooler is not holding food at 41 degrees or below; food is 53 degrees in holding unit. Clean ice tea nozzles; excess buildup. Temperature log is required for all cooling units until further notice.

Core violations: No date marking on items in sandwich table. Clean walk-in cooler shelves; excessive debris, food on shelves. Clean fan covering in walk in. Condensation leak in freezer. Check salad bar to ensure it is working since food on bar came from cooler that is not working.

Siloam Springs Senior Center

750 Heritage Court, Siloam Springs

Priority violations: Sanitizer is coming out of pre-mix unit at over 400 ppm; pre-mix unit should be adjusted; dilute solution to 200 ppm until pre-mix unit can be adjusted.

Nov. 28

Sonic

1124 Highway 412 West, Siloam Springs

Priority violations: Prep table is not holding food at 41 degrees or below; items at 43 degrees; unit was shut off overnight because it freezes up; turned on at opening and items placed in unit at opening; do not store extra items in unit until repaired. Sanitizer comes out of pre-mix unit too strong; establishment dilutes; unit should be set up to dispense accurately without diluting.

Core violations: Sour drain under mix machine; flush, clean drains and under units. Side panel off of ice cream machine; replace panel as soon as possible. Clean area under panel on inside of ice cream machine unit, excessive accumulation.

Core violations: Gasket on right side of two-door refrigerator one is torn.

Sonic Drive In

Priority violations: Ice cream is 51 degrees in left hopper and 53 degrees in right hopper of electro freeze; foods must be at 41 degrees or below in cold holding (corrected, hopper discarded). Quaternary ammonia sanitizer in three-vat sink is less than 150 ppm; should be 150-400 ppm to sanitize (corrected).

Core violations: Door on bottom of electro freeze ice cream machine is not maintaining a tight seal; repair. Recycle dumpster needs a lid on right side.

Core violations: Water leaking from pipe under scrape sink near dish machine; fix leak to prevent floor damage (repeat). Reach-in on server line with milk and whip cream does not have a working thermometer.
