Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

March 29

India Orchard

3404 S.E. Macy Road, Bentonville

Follow-up Inspection

Priority violations: Prep table is not holding food at 41 degrees or below. Storing yogurt sauce in a bucket used for detergent; chemical transfer contamination risk (destroyed). Food in walk-in is not covered; water dripping onto uncovered food. Oranges stored under raw chicken. Food uncovered under dirty shelves, possibility for dirt from shelves to fall into food; keep food covered at all times.

Core violations: Handwashing sink filled with dishes and food. No thermometer in refrigerator. Need test strips for checking sanitizer.

Core violations: Store wet wiping cloths in sanitizer. Packaged food items grab-and-go need to have the ingredients and source on package.

Priority violations: Potato salad 50 degrees, diced eggs 48 degrees, ranch dressing 46 degrees on salad bar; must be kept at 41 degrees or below. Chlorine sanitizer in automatic dishwasher was at 200 ppm for chlorine; should be between 50-100 ppm.

Priority foundation violations: Salad bar is not keeping foods at 41 degrees or below.

Core violations: Missing test strips for quaternary ammonia sanitizer. Handwashing sink had dishes in it; no longer clean, sanitized, keep clean and accessible. Missing paper towels at handwashing sink (repeat). Using cup as a scoop in potato salad container in walk-in cooler (repeat). Employee preparing pizza did not have proper hair restraint. Missing date marking on refrigerated potentially hazardous foods in walk-in cooler.

Priority violations: Meatballs 116 degrees in hot well, soup 124 degrees in hot well; must be kept at 135 degrees or above (corrected).

Core violations: Missing handwashing sign at handwashing station that states “Employees Must Wash Hands.”

Priority violations: There is no detectable sanitizer in the final rinse of dishware washing cycle. Wares washed in machine were not sanitized after cleaning. Dishes may be sanitized in three-vat sink with plain, unscented bleach at 50-100 ppm chlorine after being washed in dish machine until sanitizer strength is restored on machine.

Priority violations: No bare hand contact with ready-to-eat foods. Dishwasher was not making to heat sanitation temperature.

Core violations: No soap at front handwashing sink. Employees involved in food prep must wear a hair restraint, also beard restraint. Employees involved in food prep may not wear jewelry on hands or wrists except for plain band. Must thaw in cooler or under constant running water. Wiping cloths must be stored in a sanitation bucket.

Follow-up inspection

Priority violations: Frozen package of beef thawing in utility sink that is designated to be used for mop water (corrected).

Core violations: Beef and chicken thawing on shelf in kitchen in pans (corrected). Frozen foods must be thawed under refrigeration or under cool running water to limit the time food is in the danger zone of 135 degrees to 41 degrees where growth of potentially hazardous bacteria may be accelerated. Sanitizer on dish machine is still not working.

Priority violations: Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. Must keep records for time as a control. Raw chicken over raw beef and peppers. May not store powdered detergent with batter mix.

Core violations: No water at handwashing sink in back dessert/salad prep area. No hand drying provisions at west wall station, back dessert/salad prep area or dishwashing, handwashing sinks. Must wash hands at handwashing sink, not at prep sink or bathroom sink prior to handling food. May not reuse bulk food containers that are single use.

Complaint Inspection

Priority violations: Tomato slices 46 degrees and pico de gallo 48 degrees in top of left prep fridge; arroz blanco in walk-in cooler 45 degrees; all cold food should have an internal temperature of 41 degrees or below. Arroz blanco in walk-in cooler had made-on date of 4/1/13, so it should have been discarded by 4/7/13; employee tried to change made-on date to 4/6/13 (corrected); product was discarded.

Priority violations: Carnitas 102 degrees, beef 124 degrees, chicken 126 degrees, sliced chicken 111 degrees; hot holding is 135 degrees or above; also sauteed vegetables 108 degrees. No bare hand contact with ready-to-eat foods. Raw beef over fresh vegetables. Shredded cheese 58 degrees; cold holding is 41 degrees or below. Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. Must was hands when they are soiled; may not wipe on dirty cloth.

Core violations: Employee involved in food prep must wear a hair restraint, i.e. beard restraint. Wiping cloths must be stored in sanitation bucket. Floors and walls in food areas must be smooth and easily cleanable.

Priority violations: Sanitizer tested more than 500 ppm; should be 200 ppm for quaternary ammonia. Must sanitize food contact surfaces.

Core violations: No hand drying provisions at handwashing sink by three-compartment sink. Employee involved in food prep may not wear jewelry on hands or wrists except for a plain band. Wiping cloths must be stored in a sanitation bucket.

Core violations: Dishwasher racks are covered in grease/grime that has built up over the years; clean or replace. White RFP walls in dishwash area are black with mold and grime; clean.

Core violations: Gaskets on two of the doors of the pizza maker are torn and not forming a tight seal.

Priority violations: Waitress must wash hands prior to touching chips. No bare hand contact with ready-to-eat foods. Employees need to know proper hand washing. Beans 56 degrees; cold holding is 41 degrees or below.

Core violations: Wiping cloths need to be stored in a sanitation bucket. Employee involved in food prep must wear a hair restraint.

Airport Plaza

Priority violations: White gravy 80 degrees; hot holding is 135 degrees or above. Must date mark ready-to-eat potentially hazardous foods held more than 24 hours. May not use splashless bleach for sanitizer.

Core violations: No paper towels/hand drying provisions dispensing at kitchen handwashing sink. Single use utensils need to be stored handle up.
