RESTAURANT INSPECTIONS: April 22nd

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Tamales were made at an unapproved facility. No bare-hand contact with ready-to-eat foods. Flautas 122 degrees, rice 107 degrees; hot holding is 135 degrees. Sour cream 45 degrees; cold holding is 41 degrees or below.

Priority violations: Chicken wings 111 degrees, chicken breast 128 degrees; hot holding is 135 degrees or above. Ham 56 degrees, tomato 55 degrees, turkey 46 degrees; cold holding is 41 degrees or below.

Priority violations: Potato salad 47 degrees, chopped eggs 47 degrees, melon salad 48 degrees; cold holding is 41 degrees or below.

Core violations: Wiping cloths need to be stored in a sanitation bucket. Employee involved in food prep may not wear jewelry on hands or wrist except for a plain wedding band. Employee involved in food prep must wear a hair restraint, i.e. beard restraint. Water at handwashing sink needs to mix correctly, currently either hot or cold, no warm water.

Priority foundation violations: Refrigerator in back and smoothie prep is not keeping foods at 41 degrees or below.

Priority violations: Employees did not wash hands prior to putting on gloves; was stopped and instructed to wash hands; multiple employees. Wings are not being held at 135 degrees or above (destroyed). Boxes of lettuce sitting on the floor in walk-in (discarded). Employees need to be reminded of proper handwashing and glove use policies. Milk shake wand has residual milk/ice cream on it.

Core violations: No beard restraints worn by food employees. No thermometer in chicken breading cooler. Broom stored on top of box/packages of filler paper (corrected).

Priority violations: Ozark Jon’s special blend and Local Folks Foods did not have address of manufacturing/packaging (corrected). Rice 86 degrees, chicken 107 degrees, chili 118 degrees in hot well of small steam table was not reheated properly to 165 degrees or above before putting into hot holding; 135 degrees or above (corrected). Missing written policy and record for breakfast sandwiches on display at 125 degrees; must be kept at 135 degrees or above if not on time control. Chlorine sanitizer on automatic dish was at 0 ppm; must be between 50-100 ppm for chlorine (corrected). Wrap deli meat did not have date it was opened and exceeded the seven days allowed to be kept under refrigeration, discarded (corrected). Cut melons 54 degrees on grab-n-go display; must be kept at 41 degrees or below.

Core violations: Missing label on breakfast sandwiches and desserts in display and prepackaged sandwiches in deli display and fudges; label must list ingredients in order by weight and address of packaging. Handwashing sink in ice bagging area was not accessible for handwashing (corrected). Missing light cover for light in freezer (corrected).

Kennedy Coffee & Roasting

Priority violations: No sanitizer in dishwasher.

Core violations: Clean interior of drawer next to dump sink; remove cardboard; cardboard is not a cleanable surface. Clean under cabinet by trash can. No test strips for dishwasher. No test strips for sanitizer, quaternary ammonia. No sanitizer in wiping cloth bucket, so wiping cloths are not in sanitizer. Drain for dishwasher is overflowing and not draining properly.

Sushi House

2501 S.E. 14th St., Bentonville

Priority violations: Back room handwashing sink is not accessible and the paper towel dispenser is not working. Clean knife magnetic strip (thrown away). Exceeding the four hour time limit for sushi rice. Dish washer did not wash hands after handling dirty dishes before handling clean dishes (corrected). Sam’s rolls in walk-in at 50 degrees; cool foods properly.

Core violations: Reusing single-use containers for food storage. Clean fry cart regularly, excessive food buildup.

Decatur Livestock Auction

Priority violations: Need to date mark ready-to-eat potentially hazardous foods held more than 24 hours. Need to reheat foods to 165 degrees for hot holding. Food must come from an approved source. Sanitizer tested over 200 ppm; should be 50-100 ppm for chlorine.

Core violations: Need a thermometer to check food temperatures.

Gallery Cafe

Priority violations: Dishwasher tested 0 ppm; should be 50-100 ppm for chlorine; using three-compartment sink until fixed.

Core violations: Need alarm panel on chemical dishwasher. Plumbing leaking under dishwasher. Food may not be stored in customer areas.

Priority violations: Sleeve in ice machine needs to be cleaned. Bucket used to store season mix has not been cleaned in a while. The device used for pressing cabbage is not clean to sight and touch.

Core violations: Device used for pressing cabbage is not food quality equipment and is not made of smooth, cleanable, nonabsorbent surfaces. Cup stored as a scoop in flour container did not have a handle; must be handled with handle stored out of product (corrected). Dump sink in area near drink station has a leak under sink. Reusing five-gallon soy buckets to store potatoes; reusing one-gallon ice cream containers to store vegetables; not approved multi-use containers. Thermometer in true cooler by fryer was not working.

Hardee’s

Priority violations: Employee handled raw sausage patties and did not remove gloves before handling dining numbers; instructed to wash hands (corrected). Omelet mix was sitting out; temperature 55 degrees (food destroyed).

Core violations: Sanitizer for wiping cloth was not strong enough (corrected).

Core violations: Dead fly in the bottom drawer in the refrigerator. Food employee wearing jewelry during food preparation. Manila refrigerator thermometer reading 45 degrees but was in use and new groceries (destroyed).

Priority violations: Cole slaw on serving line 50 degrees; unit was turned off (discarded).

Priority violations: Some items not date marked.

Core violations: Stagnant water and broken tiles behind/under ice machine. Grout missing from ties around soda/ice machine. Clean and organize back storage building; boxes of to-go containers on floor; remove unnecessary cardboard and other items. Salad bar top is no longer smooth and easily cleanable in some areas; new top on order.

Taco Bueno

Core violations: Tongs stored in sanitizer water at room temperature for beef and chicken by grill. Tongs should not be stored in any water at room temperature. Bacteria will grow and reduce sanitizer strength at room temperature because tongs are constantly rinsed (corrected).

Core violations: Handle on ice cream freezer is broken, allowing air to escape. Needs repair. Small lexan pans have cracks in them, replace. Lids of pans on stream table have juice on them. Clean non-food contact surfaces and lids when service slows down in the afternoon.
