Restaurant Inspections

— Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority foundation violations: Four live juvenile and three adult roaches observed at time of inspection. Dead roaches observed in grease trap area. Clean area to remove dead roaches and prevent competition with pest control bait.

Priority violations: Chicken 115 degrees and fish 115 degrees in hot holding; must be kept at 135 degrees or above; meat was pulled and discarded; whole chicken is deboned, blast chilled and made into salads within six hours.

Bentonville Butcher & Deli

1201 S. Walton Blvd., Bentonville

Priority violations: Chicken holding unit 96 degrees; unit was set to hold at 120 degrees (corrected); set air temperature at 155 degrees or above so that food will have a temperature of 135 degrees or above while holding.

Core violations: No thermometers provided inside cold and hot holding units.

Core violations: Using bucket for waste water under dish sprayer in dishwashing area; must be plumbed according to state plumbing code. Missing tiles in wall under dishwasher; not a smooth/cleanable/nonabsorbent surface (repeat violation).

Mulligans & Highlands Golf Club

Priority violations: Raw eggs stored over cooked sausage patties and biscuits (corrected); eggs may only be stored over raw meats that are to be cooked before service. Tomato slices 52 degrees in top of three-door prep fridge; cheese brick 48 degrees, cheese slices 48 degrees, sliced mushrooms 48 degrees in main part of three-door prep fridge (corrected); cold foods must be kept at an internal of 41 degrees or below (repeat). Plastic water tumbler on prep surface of three-door prep fridge (corrected). Drinks in prep areas need lid and straw to help prevent spills.

Priority foundation violations: No paper towels available at two hand sinks in kitchen; always have disposable hand drying available to encourage hand hygiene.

Core violations: Compressor on three-door fridge is frozen and is preventing unit from holding cold food at proper temperatures (repeat); repair. This is the second consecutive inspection where food in three-door prep top has not been maintained at 41 degrees or below. Do not use top of unit if it will not maintain food at proper temperatures. Soda nozzles have mold; clean as necessary to prevent growth; replace if necessary. Ice scoop stored with towels; store in hanging bucket with handle up.
