RESTAURANT INSPECTIONS: November 19th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Oct. 30

Old High Middle School

406 N.W. Second St., Bentonville

Priority violations: When pulling items from the freezer put a pulled out/on date on that item. Need written policy for time as control.

Core violations: Do not store trash can next to ice scoop (corrected).

Building Bridges at Central

402 N.W. Second St., Bentonville

Core violations: Cutting board is cut enough that surfaces are no longer smooth and easily cleanable.

Priority violations: Foods not date-marked. Raw eggs stored above butter; store raw eggs below ready-to-eat items.

Core violations: Employee food stored above establishment food. Need a thermometer to check food temperatures.

Chutny’s Indian Restaurant

2850 W. Walnut St., Rogers

Priority violations: Reach-in refrigerator is not holding food at 41 degrees or below (service call made). Food containers stored above uncovered food containers. Thawing shrimp at room temperature and the container is sitting on the floor; rice soaking in container on floor. Reheat foods to 165 degrees prior to putting in hot holding.

Core violations: Bulk items are not kept covered; keep covered to prevent contamination. Clean interior of store room. Clean interior of prep table. Reusing single use containers; replace with NSF approved containers.

Hampton Inn

4501 W. Walnut St., Rogers

Priority violations: Maintain milk at 41 degrees or below at all times.

Core violations: Milk for employees stored in establishment refrigerator; store employee food in employee refrigerator.

Hyatt Place

4610 W. Walnut St., Rogers

Priority violations: No soap at handwashing sink. Handwashing sink does not look used; employee said washing hands at three-compartment sink. Cottage cheese 44 degrees stored on a unit that specifically says not been performance tested for storage of potentially hazardous foods. Milk not being held at 41 degrees or below; ice core is completely melted. Double stacking cheese on top of pans in prep table. Date on ham is 10/12; unknown when it was pulled from the freezer. No date mark on some foods; Greek yogurt and other yogurts expired 10/20/12 and 10/31/12. Moldy guacamole in refrigerator.

McDonald’s 32861

2601 Pleasant Crossing, Rogers

Priority violations: Dishes not being sanitized before use and after cleaning; corrected by using sanitizer in three-compartment sink at 200-400 ppm quaternary. Southwest vegetables 46 degrees and sliced tomatoes 52 degrees on salad prep; must be kept below 41 degrees in cold storage. Sliced tomatoes, cheese and other items were not marked with a discard time for using time as health control (corrected).

Priority foundation violations: Dish machine not reaching 165 degrees required by machine to sanitize; thermolabel did not adjust; wash 140 degrees and rinse 160 degrees.

Core violations: Dish machine and salad prep unit are not in good repair.

JJ’s Grill & Chill

4500 W. Walnut St., Rogers

Priority violations: Food in walk-in hot, not cooing properly (corrected). Tomato basil soup date marked 10/26/12; spinach dip date-marked 10/23 and 10/27; no more than seven days; food destroyed. Prep tables are not holding food at 41 degrees or below.

Core violations: No test strips for either sanitizer in the kitchen. Excessive water behind bar; water not draining properly (work order submitted). Concrete wearing away in kitchen so floors are no longer smooth and easily cleanable; large gravel exposed allowing areas of water/food to stand in grooves. Employees must be trained on proper cooling and ensure refrigerators are working daily.

Central Park at Morning Star Elementary

1400 S.W. Liberty Ave., Bentonville

Priority violations: Foods (meatballs) not heated to 165 degrees prior to placing in hot holding; temperatures were 132 degrees/133 degrees (reheated). Hot foods in walk-in 84 degrees/85 degrees; improper cooling of foods (food destroyed). Food out of date in walk-in; redate items when pulled from the freezer; use potentially hazardous foods within seven days (food destroyed). Need time as a control policy; log being used already. Sanitizer at three-compartment is not strong enough.

Core violations: No hot water on the east side of the kitchen sinks.

El Pueblo Market

1707 S. Eighth St., Rogers

Priority violations: Queso Fresco in nine bags of approximately 1 pound each. Person in charge provided labels stating that the cheese is made in facility with same owner at 1551 W. Hudson St., Rogers. The labels are not attached to product and do not include an ingredients list including information on whether the product was made with pasteurized milk. You may not sell, move or destroy product without the Arkansas Department of Health’s written consent and a representative of the Health Department must witness the action (corrected, product discarded).

Pupuseria Miraflores

1845 S. Eighth St., Rogers

Priority violations: One baby cockroach observed near handsink, one juvenile cockroach observed on wall near rear refrigerator and one adult cockroach observed in dining (corrected, roaches killed on sight). It is important to clean areas where the dead roaches are such as in florescent ceiling fixtures to prevent attraction of pests. No paper towels at only handwashing sink; use disposable towels to dry hands (corrected).

Orange Leaf

2603 Pleasant Grove Road, Rogers

Core violations: Strong smell of sewer gas in back room; contact a plumber to determine the cause and correct the problem. Tighten spring on restroom doors so they are self-closing. No hot water at back handwashing sink.

Nov. 7

Chen’s Garden Buffet

428 E. Town Center, Bella Vista

Core violations: Rice paddles stored in water at room temperature (repeat). Ice cream scoops stored in water at room temperature. If utensils are stored in water it must be at 135 degrees or above or 41 degrees or below. It is acceptable to use a crock pot with water at 135 degrees or above to store utensils in. Recommend using pre-portioned ice cream servings instead of allowing customers to use scoops. Soda nozzles are dirty; clean as necessary to prevent mold growth (repeat). It is usually recommended to soak pieces overnight in soda water; it may be easier to replace nozzles you have now than to clean off the accumulation.
