Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Core violations: Check sanitizer every time it is made; should be between 50-100 ppm. Wash, rinse and sanitize in that order.

Nola’s Cafe

980 S.E. Eagle Way, Bentonville

Priority violations: Sandwiches and tuna salad plate are not being held at 41 degrees or below; sandwiches were made about two hours ago. Employees did not wash hands long enough or properly.

Core violations: Employee’s hair is not effectively restrained; employee kept moving the hair out of the way. No sanitizer in wiping cloth bucket.

NWACC Global Business

900 S.E. Eagle Way, Bentonville

Priority violations: Barbecue sauce still hot in the refrigerator 128 degrees; not cooled properly.

Core violations: Carpet on floor in dry storage area; floors for food storage areas must be smooth and easily cleanable. Gauge on dishwasher is not reading accurately; repair, calibrate or replace as needed. No vacuum packaging without an approved variance with the Arkansas Department of Health.

Core violations: Some condensation leaks in freezer; be sure not to store any open food under until leaks are fixed. Scoop handles laying down on some bulk food items; store handles up when stored in product.

Core violations: Ice machine is leaking from the unit itself; water is continuously running from drain pipe. Clean interior of machine prior to the start of 2013 season.

Priority violations: Turkey and cheese sandwich 48 degrees, salad (lettuce) 54 degrees; cold holding is 41 degrees or below.

Priority violations: Hot taco meat put in upright cooler; food temperature still 128 degrees/147 degrees; food not being cooled properly (corrected). Some deli sliced meats not date marked; ham ends date marked 10/17/12.

Priority violations: Container of cottage cheese and one carton of liquid egg exceeds use by date. Item pulled from freezer date 10/2; redate when pulled.

Core violations: Condensation leak in freezer.
