RESTAURANT INSPECTIONS: November 5th

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Oct. 23

Antrim’s Pancake House

106 N. Bloomington St., Lowell

Core violations: Reusing strawberry 2-quart containers to store other foods; reusing whipping cream 5-gallon containers to store pancake mix; not approved multi-use containers. Women’s bathroom door is not self-closing.

El Compesino

106 N. Bloomington St., Lowell

Priority violations: Chicharon drying at room temperature; sliced tomato 50 degrees; must be kept at 41 degrees or below or 135 degrees or above.

Core violations: Men’s bathroom door is not self-closing. Reusing grocery-type bags to store meat in freezer. Prep table is not working as a cooler; no food currently being stored inside cooler. Missing test strips for chlorine sanitizer.

La Taquiza

106 N. Bloomington St., Lowell

Priority violations: Raw beef stored above whole onions in three-door cooler (corrected).

Core violations: Using grocery-type bags to store cooked meat in steam table and raw meat in freezer and vegetables in freezer; not an approved food quality bag for storage. Missing test strips for quaternary ammonia sanitizer (repeat).

Gravette High School Fieldhouse

607 Dallas St. SE, Gravette

Core violations: No thermometer in refrigerator. No test strips for bleach sanitizer; bleach should be 50-100 ppm when sanitizing.

Marvin’s IGA Food Store

101 Atlanta St. SE, Gravette

Core violations: There is no handle on left side of deli prep cooler; replace. Paper towel holder at hand sink is empty (corrected). Wire racks and metal under racks on dairy open face display coolers are dirty; clean.

ANH Montessori School

2730 N. Woods Lane, Rogers

Core violations: Clean interior of toaster oven; accumulated debris inside. Potty booster seats stored on top of Igloo coolers. Refrigerator thermometer reading 42 degrees; no food temperature was taken. Need to seal around the pipe leading out the wall from dishwasher.

Kum & Go 403

2400 W. Hudson Road, Rogers

Priority violations: Maintain hamburgers (extra) at 135 degrees or above or use time as a control same as the ones already assembled.

Core violations: Water in bulb covers in walk-in; correct as needed as soon as possible.

Rogers Pre-Kindergarten

2710 N. Woods Lane, Rogers

Priority violations: Sanitizer is not dispensing the correct amount of chemical for 200 ppm; all equipment/utensils must be resanitized after it is repaired. Employee came into kitchen to get their food out of the establishment cooler, but did not was their hands first.

Core violations: Clean interior of fans on inside of refrigerator. Employee food stored above establishment food; keep employee food stored to prevent possible cross contamination or employees have their own refrigerator so no employee food should be stored with establishment.

Papa John’s 14

2810 W. Walnut St., Rogers

Core violations: A few cracked and broken containers/lids; no longer smooth and easily cleanable; replace as needed.

Oct. 24

Taj Indian Cuisine

310 S. Walton Blvd., Bentonville

Priority violations: Using Styrofoam cup as a scoop in flour container; must be handled with handle stored out of product (corrected). Tomato chutney 48 degrees and curd rice 54 degrees on buffet cold table; must be kept at 41 degrees or below (repeat). Ice scoop being stored on top of ice machine; not a sanitized surface.

Core violations: Faucet on handwashing sink is loose and hard to use; must be tightened or repaired. Food prep employees are not wearing proper hair restraint.

Papa John’s 3993

231 N. Walton Blvd., Suite 5, Bentonville

Priority violations: Sausage 53 degrees and sliced tomatoes 48 degrees on counter in cutting area; must be kept at 41 degrees or below. Employee did not wash hands after touching money and before preparing pizza (corrected). Quaternary sanitizer in three-compartment sink was at 100 ppm; must be kept at 200 ppm per label (corrected). Spray bottles were not labeled and not labeled correctly.
