RESTAURANT INSPECTIONS

— Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted. Critical violations are defined as items that relate directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Sanitizer in spray bottle was over 200 ppm for chlorine; must be between 50-100 ppm (corrected).

Noncritical violations: Light bulb above fry station is not covered or shatterproof.

Noncritical violations: Employee involved in food prep must wear a hair restraint, also beard restraint. Floors and walls in food handling areas must be smooth and easily cleanable. Toilet facilities should be supplied and clean. Women’s bathroom has out or order sign and men’s bathroom sink is clogged and facilities dirty.

Noncritical violations: Missing paper towels at handwashing sink (repeat violation; corrected). Using cups as scoops in salsa containers in three-door cooler; must be a scoop with a handle with handle stored out of product (repeat violation). Reusing large buckets of chicken flavored soup container to store cabbage and salsa; not an approved multi-use container. Ice scoop being stored with handle toward ice; using cup as a scoop in dry beans customer display (corrected; repeat violation). Nozzle dish sprayer is broken and does not turn off easily.

Noncritical violations: Missing light cover on bulb in walk-in cooler and men’s bathroom or need shatterproof bulb.

Critical violations: Cabbage in water 47 degrees; air temperature 47 degrees in double-door cooler; milk 47 degrees in refrigerator by grill.

3402 E. U.S. 412, Siloam Springs

Critical violations: Cut honeydew 65 degrees, tomato slices 61 degrees and pineapple juice with pineapple bits 61 degrees in middle buffet (repeat violation; corrected); placed new honeydew from fridge at 41 degrees on buffet line. Recommend using time to limit bacterial growth. Placing large pans of hot noodles in refrigerator does not cool quickly (corrected); placed pans in freezer for 30 minutes and stirred to cool rapidly. Recommend rinsing with ice and cold water before placing in cold storage to limit time to cool properly.

Noncritical violations: Mold on outer part of soft serve nozzle. Mold in ice machine. Clean at end of night when machines can be emptied. Label soft serve machine with tape for mix used, “non-dairy soft serve” or “lactose free soft serve.”
