— Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted. Critical violations are defined as items that relate directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Potato salad 48 degrees in top of prep table one. Employees not washing hands prior to putting on gloves. Reheating food must be done from 41 degrees to 165 degrees in two hours or less; meats in hot unit 104 degrees to 139 degrees (corrected). Potentially hazardous food are not all date marked; larger upright cooler is not holding food at 41 degrees or below (tuna salad 59 degrees). Some items exceed manufacture use by date. Salad bar was not turned on; foods not 41 degrees or below.

Noncritical violations: Reusing single-use containers. Need food thermometer. Reach-ins at bar are not draining properly; establishment was required to correct this situation upon opening. Water condensation from reach ins is leaking under units and following the drain lines. Sanitizer at bar not strong enough (using for wiping cloths). Salad toppings must be shielded, such as sneeze shield or containers with lids. Rolls need to be protected from customer contamination. Store silverware handle up only.

Critical violations: Pan of chicken was over milk (corrected). Ice level was below the food level on yogurt at breakfast bar. Sausage patties in drawer warmer not at 135 degrees or above (corrected).

Critical violations: Ice cream milk chocolate is 45 degrees in the hopper; should be 41 degrees or below. Ice cream machine needs to be cleaned prior to using.

Noncritical violations: Check temperature of items removed from serving lines prior to putting away; items must maintain 41 degrees or below to be reserved. Employee towel drying dishes; dishes must be air dried. Need a sneeze shield or some other form of protection over self-serve cookies. Clean soda nozzles regularly; excessive build up on the nozzles.

Sam Walton Development

2001 S.E. 10th St., Bentonville

Critical violations: Employees did not wash hands prior to putting on gloves. Eggs still 95 degrees from breakfast; use proper cooling methods. Cream cheese is 47 degrees; was in cooler 203 (destroyed).

Noncritical violations: Portion cups in handwashing sink. No test strips for sanitizer. No person in charge was identified during inspection.
