Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Raw eggs stored above bags of romaine (corrected).

Core violations: Scoop stored in flour with handle in product. Cracked scoop in flour no longer smooth, easily cleanable. Unfinished wood must be sealed so it is smooth, easily cleanable, nonabsorbent, nonporous. No paper towels at handwashing sink.

Louie’s Grill

Priority violations: Sliced turkey stored above the load line 45 degrees/54 degrees; do not store foods above the load line. Top of large prep table is not consistently keeping foods at 41 degrees or below. Top of small prep table is not holding all food at 41 degrees or below. Automatic dishwasher is not dispensing sanitizer properly; manually sanitize until repaired.

Core violations: Holding serving utensils in water stored at room temperature; either keep utensils hot with the food or in running water or keep water at 135 degrees or above or 41 degrees or below. Employee wearing ring on finger. Bar handwashing sink not accessible; glasses in it (corrected). Milk at bar is expired (corrected).

Priority violations: Raw chicken on prep table; being thawed and transferred to pan (corrected). Flat of raw eggs stored on top of ready-to-eat foods (corrected). Chicken salad manufacture use by date 10/29/12 (destroyed). Raw chicken stored above ready-to-eat foods in exterior cooler (corrected). Sanitizer too strong; quaternary ammonia 200 ppm. Bleach water in spray bottle appears to be mixed with something.

Core violations: Repair the threshold of new door entry.

Priority violations: Maintain hot food at 135 degrees or above at all times (corrected). Walk-in ceiling is leaking; repair as soon as possible. Ice cream mix is not being held at 41 degrees or below.

Core violations: Freezer condensation leak; repair. Waffle cones being stored in a wicker basket; store in something that can be washed. Need test strips for sanitizer; 50-100 ppm.

Core violations: Reach in is not holding food at 41 degrees or below; monitor unit (food moved to walk in). Hot and cold water lines crossed at three-compartment sink. Establishment thermometer is not accurate; recalibrate/replace as needed.

Priority violations: Not all items are time/date-marked.

Core violations: Employee food in reach in cooler on the line; designate an area for employee food to prevent contamination of establishment food. Mint chips stored in regular disposable cup; store in something designed to wash and sanitize. No thermometer in salad cooler line or cooler under coffee front line. Ice scoop stored laying down in ice.

Priority violations: No sanitizer dispensing in the automatic dish machine (corrected).

Core violations: Repair floor in the walk-in; floor gaps and is not easily cleanable. Seal between freezer and cooler is peeling off the floor/door joint. Employees restroom door must be self-closing. Replace/recoat the shelves in the upright cooler to ensure smooth and easily cleanable.

Whole Hog Cafe

1400 S.E. Walton Blvd., Bentonville

Priority violations: Hot holding hot box unit is not keeping food at 135 degrees or above; adjusted. Walk in is not holding food at 41 degrees or below.

Core violations: Reusing single use container for ranch dressing. Repair floor/cove base and give the dishroom a complete cleaning. Gnats in dishroom around drain. Effective hair restraints are required for food staff.

Days Inn and Suites

3408 S. Moberly Lane, Bentonville

Priority violations: Maintain hot food at 135 degrees or above; eggs 86 degrees. Food must be date-marked. Bleach water stored in some other chemical bottle; do not reuse chemical bottles. Spray bottle has straight bleach in it; employees were told to use the spray bottle directly on dishes. Employees must be trained properly on all aspects of food safety.

Core violations: Store silverware handle up only to prevent contamination from customers. Dishes must be submerged in bleach water, not sprayed with bleach water. Test strips must be accessible to employees to test chlorine concentration. Need paper towels for handwashing sink.
