RESTAURANT INSPECTIONS

— Information is from Arkansas Department of Health records. Restaurants are listed in order of inspection date. All reports are from regular food service/ food store inspections unless otherwise noted.

Critical violations are defined as items that related directly to factors that lead to food-borne illness, and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Noncritical violations: Bathroom door needs to be self-closing. Employee involved in food prep must wear hair restraint. Wiping cloths need to be stored in sanitation buckets. Back door needs to be able to seal shut.

Critical violations: Meatball 101 degrees, meatball 98 degrees; cooking temperature should have been 165 degrees or above - corrected on site. Employees must wash hands before putting on gloves. Employees must wash hands after putting on hair restraint. Need hand drying provisions in back of kitchen handwashing sink. Raw chicken stored above ham dripping on container below.

Noncritical violations: Must use scoop with handle in bulk container bins, not bowls and cups. Employees involved in food prep may not wear jewelry on hands and wrist watches and bracelets. Slicer and mixer attachment have grime onthem, need to be cleaned.

Noncritical violations: Shredded chicken 46 degrees, tamales 45 degrees, rice 58 degrees, beef enchiladas 54 degrees; cold holding is 41 degrees or below. Fix gauges on dishwasher; gauges not working properly. Walk-in cooler handle needs to be fixed. Walk-in cooler temperature 45 degrees; needs to be able to maintain food at 41 degrees or below.

Employees involved in food prep must wear hair restraints.

Hand towels needed at handwash sink - corrected on site. Raw chicken and beef out of temperature; should be 41 degrees or less. Dishwasher not sanitizing properly. Dishwasher not operating per manufacture’s limits. Ground beef out of temperature; should be 135 degrees or higher. Chemical stored near food products in bar storage area - corrected on site.

Noncritical violations: Hand towels needed at handwash - corrected on site. Chemical spray bottles need proper labeling and chemical should be stored separate from food products - corrected on site. Sanitizing solution should be between 50-100 ppm. Raw beef patties stored above uncovered container of cornmeal - corrected on site. Employees in food prep should wash hands when handling raw foods then handle cooked foods.

Critical violations: Orange juice in dispenser was at 50 degrees; must be kept at 41 degrees or below (repeat violation).

Noncritical violations: Thermometer was missing in refrigerator. Floor under ice machine has build up of calcium/rust deposits; needs to be cleaned (repeat violation).

Rubaie’s Cafe

3404 S.E. Macy Road, Suite 20, Bentonville

Critical violations: Storing raw shrimp above mushrooms in refrigerator; storing raw beef above carrots and spinach (repeat violation) - corrected on site.

Spoon in powered sugar was being stored with handle in product - corrected on site.

Storing utensils in bleach water before use; must wash, rinse, sanitize and air dry utensils before use - corrected on site. Sanitizer solution in dishwasher was at 0 ppm; sanitizer solution in sink was at 200 ppm; must be between 50-100 ppm.

Milk 44 degrees in salad prep table; must be held at 41 degrees or below (milk discarded).

Noncritical violations: Light above grill is not shielded.
