RESTAURANT INSPECTIONS

— Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service/food store inspections unless otherwise noted. Critical violations are defined as items that relate directly to factors that lead to food-borne illness and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

JULY 13

Taqueria Martinez

2910 S. Walton Blvd., Bentonville

Noncritical violations: Need drying rack for drying dishes. Do not set on towel to dry. Ceiling tiles must be smooth and easily cleanable in food preparation area and dish room.

Kobe

1311 S. Walton Blvd., Bentonville

Critical violations: Keep rice log of when sushi rice is made for each batch every day. Clean interior of ice machine (shared with gas station), excessive buildup on ice guard is dripping into ice. Keep food in front refrigerator at 41 degrees or below.

Keep sushi cooler doors closed to ensure food stays at 41 degrees or below.

Noncritical violations: Need thermometer in refrigerator (front).

JULY 14

McDonald’s

903 W. Walnut St., Rogers

Critical violations: Quaternary ammonia sanitizer dispenser in three-compartment sink was 300 ppm, needs to be 200 ppm.

Noncritical violations: Food prep employee did not have proper hair restraint.

Marion’s Donuts

816 W. Walnut St., Rogers

Critical violations: Handwashing sink is not accessible for handwashing; full of personal items. Frosting mixer in back area has excessive buildup, must be cleaned daily (repeat violation). Wooden utensils used for donut icing and glazes are not being cleaned daily (repeat violation).

Need test strips for sanitizer.

Noncritical violations: Floors in back where trash is stored need to be cleaned.

Trash being stored in back area needs to be in an enclosed container. Lid to large flour container is cracked and repaired with duct tape. It must be smooth, easily cleanable and non-absorbent material (repeat violation). Gourmet beverage dispenser needs to be cleaned inside.

Need thermometers in all refrigerators.

Taco Tico

Critical violations: Quaternary ammonia in spray bottle was at 400 ppm, must be 200 ppm (corrected on site). Sour cream 47 degrees, ranch dressing 47 degrees, pico sauce 47 degrees in walk-in cooler, must be kept at 41 degrees or below.

Noncritical violations: Large microwave door’s outer guard is broken; needs to be replaced. Need signs at handwashing sink and in bathrooms stating “Employees must wash hands.”

Marvin’s IGA Store

Atlanta and Arkansas 59, Gravette

Noncritical violations: Excess ice should be removed from walk-in freezer. Fans in meat cooler should be cleaned (butcher area).

Hair restraints needed in butcher area. Light shields needed in meat display coolers.

Noncritical violations: Light shield needed in kitchen.

JULY 15

ABC Happy Kids Learning Center

301 S.E. 28th St., Bentonville

Noncritical violations: Remind teacher to wash hands immediately prior to putting on gloves. Proper handwashing is for 20 seconds. Employee did not wash for required length of time. Sanitizer at three-compartment sink was not strong enough, should be between 50 to 100 ppm (corrected). Spray bottles must be labeled with contents. Finish shelf under prep table must be smooth and easily cleanable, non-absorbent, non-porous.

Noncritical violations: Wooden shelves must be stained or painted for ease of cleaning.

Critical violations: Hand soap and disposable paper towels must be provided at handsink in kitchen (corrected).

Noncritical violations: Food employee must not wash hands in threecompartment sink, use handsink; frozen beef must be thawed using an approved method.

Taqueria Guanajuato

Noncritical violations: Wet wiping cloths must be stored in chemical sanitizer between use; frozen shrimp must be thawed using an approved method; repair peeling paint on interior surface of vent hood an wall next to hood.

Noncritical violations: Hole in front of mop sink basin requires repair; slipresistant pad on bottom stair is loose and requires repair.

July 20

Harps 117 Deli/Bakery

715 N. Second St., Rogers

Noncritical violations: Light shield needed in kitchen.

Rolando’s

224 S. Second St., Rogers

This is a follow-up inspection from July 13. Only critical violation from previous inspection was looked at. All other critical violations have been corrected since last inspection.

Critical violation: Guacamole 44 degrees, cooked beef 44 degrees, milk 44 degrees in walk-in, must be kept at 41 degrees or below.

Sonic

923 W. Walnut St., Rogers

Noncritical violations: Electrical tape around table handle, not a smooth, nonabsorbent, easily cleanable surface (repeat violation). Tile around drain near three-compartment sink needs to be repaired so that there is no standing water in area (repeat violation). Cork board on wall across from three-compartment sink needs to be replaced. Prep table has standing water on bottom shelf. Bottom shelf of prep table has seams coming undone.
