RESTAURANT INSPECTIONS

— All reports are from regular food service/food store inspections unless otherwise noted.

• Critical violations are defined as items that relate directly to factors that lead to food-borne illness, and must be corrected immediately.

• Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Sept. 2

World Garden, 2905 S. Walton, Bentonville

Critical violations: Prep table is not holding food at 41 degrees or below. Walk-in not holding food at 41 degrees or below. Sanitizer at threecompartment sink is only 100 ppm. Should be 200 ppm.

Noncritical violations: Do not reuse single-use food containers for food storage. Milk cooler thermometer is broken. Sanitizer at dishwasher is too strong. Leak in walk-in. Repair broken tile at kitchen door.

Sept. 4

Bentonville High School, 1901 SE J St., Bentonville

Critical violations: Cool foods properly prior to putting in cold holding. Salad bar is not maintaining food at 41 degrees or below. Deli meats not maintained at 41 degrees or below. Hamburger display is not holding food at 135 degrees or above.

Noncritical violations: Foods stored on top of serving line display cases must be protected from contamination such as covers or sneeze shields. Do not allow milk bottles to get submerged in water. Only kitchen employees should get into the ice machine.

Sept. 8

Rogers High School, 2300 S. Dixieland, Rogers

Noncritical violations: Light bulb shields needed in bottom walk-in and freezer. Excessive condensation dripping from freezer compressor. Caulking coming off around hand sink.

Kirksey Middle School, 2930 S. 1st St., Rogers

Critical violations: Sanitizer at three-compartment sink is not strong enough. Should be 200 ppm. Selfserve cold case has thermometer reading 50 degrees.

Noncritical violations: Only authorized persons should be getting into the ice machine. This includes custodial staff and teaching staff. Condensation leak in freezer compressor.

Old Wire Road Elementary, 3001 S. Old Wire Rd., Rogers

Critical violations: Clean and sanitize interior of ice machine.

Noncritical violations: Gasket on hot holding case is coming off. Condensation leak from freezer sprinkler and compressor. Excessive condensation on the exterior of freezer door.

Church's Chicken, 1313 W. Walnut, Rogers

Critical violations: Small sandwich cooler is not holding food at 41 degrees or below. Employees must wash hands after breading chicken prior to touching anything else.

Sonic, 208 S. Walton, Bentonville

Critical violations: Eggs stored over ready to eat foods in upright refrigerator. Bag of French toast sticks laying on top of raw hamburger meat.

Noncritical violations: Condensation leak in walk-in refrigerator. Excessive water in walk-in floor from leak. Spraying floor with water hose. Splashes of water was getting sprayed on open bread.

Mama Fu's Asian House, 700 SE Walton, Bentonville

Critical violations: Employees drink on back prep table did not have a lid. Calimari and raw chicken must be kept at 41 degrees or below. Eggs were being stored above vegetables in walk-in cooler. Raw chicken, raw shrimp, cocoa milk in walk in cooler must be kept at 41 degrees or below.

Noncritical violations: Raw foods in prep coolers were not being date marked properly per establishment policy.

Johnny Brusco's New York Style Pizza, 700 SE Walton, Bentonville

Noncritical violations: Breaded chicken and ham filled above fill line was not keeping at 41 degrees or below. Thermometers were missing in prep coolers and beer cooler. Using spoon as scoop in parmesan cheese. Spoon handle was in contact with product. Need to clean cabinet storing kegs for tap.

Sept. 14

McDonalds, 101 E. Centerton Blvd., Centerton

Critical violations: No time mark on cheeses kept out of temperature. Employee drink without lid and straw in walk in cooler.

Noncritical violations: Employee drink spilled on top of packaged muffins.

Taqueria Y Frteria Burrito, Centerton

Critical violations: Storing raw beef above ready to eat foods in back refrigerator. Spray bottles must have labels indicating contents. Hotdogs at 48 degrees. Ranch dressing at 50 degrees. Must be kept at 41 degrees or below.

Noncritical violations: Handwashing sink was not draining properly. Need thermometer in back refrigerator.

Imperial Cake Design, 27 Centerton Blvd., Centerton

Noncritical violations: Need thermometer in juice/beverage cooler. Bathroom door must be self closing.

Sept. 15

Sleep Inn, 215 SE Walton Blvd., Bentonville

Critical violations: Paper towels were not available at hand-washing sink.

Noncritical violations: Need toclean floor under ice machine.

Waffle House, 2308 SE Walton, Bentonville

Critical violations: Sanitizing bleach in wiping bucket was at 200 ppm. Must be between 50 and 100 ppm.

Noncritical violations: Floor in walk-in cooler is not smooth and easily cleanable. Dishes are not being sanitized. Dishwasher is not getting dishes hot enough to sanitize properly.

Taco Bell, 102 S.Walton, Bentonville

Critical violations: Dishes were being sanitized at 400 ppm. Employee did not wash hands before putting on gloves and then proceeded to prepare food.

Noncritical violations: Quat sanitizer was at 400 ppm. Should be at 200 ppm.

Ron's Hamburgers and Chili, 1702 S.Walton, Bentonville

Critical violations: Ham and turkey at 48 degrees. Not being kept at 41 degrees or below.

Noncritical violations: Need thermometer in upright refrigerator, prep cooler and walk in.
