In the wake of President Obama's commanding speech before a joint session of Congress last Wednesday, much of the dialogue surrounding Democratic efforts to bring health care to the 47 million Americans who lack insurance today - and control costs on behalf of everyone else - is once again focusing on ways to afford these critically important reforms.
