— All reports are from regular food service/food store inspections unless otherwise noted.

Critical violations are defined as items that relate directly to factors that lead to food-borne illness, and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Oct. 21 Sonic

316 S. Gentry Blvd., Gentry

Critical violations: Corn dogs are 49 degrees in upright cooler. Must be kept at 41 degrees or below.

The Station

400 Gentry Blvd., Gentry

Critical violations: No paper towels at handwashing sink.

Chemical cleaning supplies stored above cans of sausage gravy. Polish sausage on a stick is 128 degrees. Corn dogs are 111 degrees. Must be kept at 135 degrees or above.

Noncritical violations: Bottom shelf of tallest upright freezer needs to be cleaned.

Need covered receptacle for sanitary napkins.

Gentry Intermediate School

307 W. Second, Gentry

Critical violations: Raw eggs stored above apples in walk-in refrigerator. Employee washed hands with gloves on hands.

Bleach sanitizer in spray bottle was more than 200 ppm. Must be between 50 and 100 ppm.

Noncritical violations: Back door has gap at threshold.

Marvin’s Deli

300 S. Gentry, Gentry

Critical violations: Spray bottle containing chemicalsmust be labeled. Chicken strips are 128 degrees. Must be kept at 135 degrees or above. Bleach solution in spray bottle was at 0 ppm. Must be between 50 and 100 ppm.

Noncritical violations: Need thermometer in meat hotholding display. Need light shield in freezer. Need test strips for bleach sanitizer.

Marvin’s

Critical violations: Spray bottle needs to be labeled with contents.

Noncritical violations: Need light shields for walk-in freezer. Need bleach test strips for bleach solution.

Oct. 22 Boston’s Restaurant

211 S.E. Walton, Bentonville

Critical violations: Chicken is 45 degrees. Ham is 48 degrees. Pepperoni is 48 degrees. Must be kept at 41 degrees or below. Tongs being kept in bucket of quat sanitizer at 400 ppm. Should be 20 ppm.

Noncritical violations: Need to repair door on salad station prep cooler. Need to clean cabinet of salad station. Need sanitizer test strips in bar area.

Rubaie’s Cafe

3404 S.E. Marry, Bentonville

Critical violations: Handwashing sink must be kept accessible at all times.

Storing raw foods above cooked beef and chicken in upright refrigerator. Using bowl as a scoop in rice container. Employees in prep area are wiping hands on apron without washing hands.

All spray bottles must be labeled with contents.

Noncritical violations: Employees in food prep areamust wear hair restraints.

Missing light cover on light above deep fryer.

Bud’s Family Style Chicken

2505 S. Walton Blvd., Bentonville

Critical violations: Coleslaw is 47 degrees. Must be kept at 41 degrees or below.

Noncritical violations: Handwashing sink had utensils and ice. Must be kept clean and clear for hand washing only. Thermometer missing in upright cooler.

Oct. 23

Ventris Trails End Resort

9484 Simrell Drive, Garfield

Critical violations: Sanitizer is too strong. Should be 200 ppm.

Noncritical violations: Test strips are not accurate. New strips required.

Short Stop Grill

18444 Marshall, Garfield

Critical violations: Chicken was not holding at 135 degrees. Unit was adjusted.

Noncritical violations: Need new test strips.

Oct. 26 Sam’s Club

3500 S.E. Club, Bentonville

Noncritical violations: Single-serve items should be stored at least 6 inches off floor. Hand soap needed at sink.

Mazzio’s Pizza

1117 N. U.S. 71, Bentonville

Noncritical violations: Light shield needed in store room.

Pollo A La Prilla 2505 N. 17th, Rogers

Critical violations: Raw meat and eggs above cooked meats.

Noncritical violations: Clean behind and under prep table.

Need new test strips forbleach. Do not reuse singleuse containers. Need covering along the bottom of the wall in store room. Evidence of possible mouse droppings and cockroaches.

Panificadoro Merchado

2515 W. Hudson, Rogers

Critical violations: No paper towels at handwashing sink.

Baking pans must be cleaned and sanitized every day. No paper towels in restroom.

Use paper towels to dry hands. Whisk for mixer is held together with string, twine and shoelaces. Replace. Eggs sitting at room temperature.

Noncritical violations: Do not reuse single-use containers for food storage.

Restroom door must be self closing. Need test strips for sanitizer.

Apple Blossom

2501 N. 22nd St., Rogers

Critical violations: Sliced meats must be date marked once the original package is opened.

Noncritical violations: Establishment is using an iodine solution and needs test strips.

Kum and Go

2400 W. Hudson, Rogers

Critical violations: Raw eggs and bacon stored above ready to eat foods.

Noncritical violations: Sanitizer test strips do not indicate the desired concentration or how long to immerse test strips in solution.

Clean interior of cup holder on shake machine.

Oct. 27 Aroma

1100 S.E. 14th St., Bentonville

Noncritical violations: Vent hood needs cleaning.
