Benton County Restaurant Inspections

All reports are from regular food service/food store inspections unless otherwise noted.

Critical violations are defined as items that relate directly to factors that lead to food-borne illness, and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Dec. 2

w My Friends and Me, 901 N.E. “J” St., Bentonville

Noncritical violations: Coolers at 47 degrees. Coolers should maintain at 41 degrees or below.

Dec. 3

Airport Plaza, 7402 S.W. Regional Airport Blvd., Bentonville

Critical violations: Chicken legs were 136 degrees. Chicken breast was 134 degrees. Chicken must be 165 degrees or above. Must date mark ready to eat foods. Must wash hands with soap for at least 20 seconds. Must wash hands before putting on gloves.

Noncritical violations: Employees involved in food prep may not wear jewelry. Need thermometers for food temperatures.

NWA Children’s Shelter, 7702 S.W. Regional Airport Blvd., Bentonville

Noncritical violations: Test strips needed for sanitation. Wiping cloths need to be stored in a sanitation bucket. Employee involved in food prep must wear hair restraint.

My Other Mother Child Care, 218 N. 14th St., Rogers

Critical violations: Evidence of mice in the storeroom and under an old ice maker. Remove or repair ice maker. It is the source of the smell. Date mark items not used within 24 hours. Spray bottles not labeled with contents. Do not reuse chemical bottles.

Noncritical violations: Do not reuse single use items for food storage. Shelf liner in store room is not smooth and easily cleanable. Clean out refrigerator in front room. Contact a professional pest control company.

Sushi House, 2501 S.E. 14th St., Bentonville

Critical violations: Fully cook chicken. Maintain cold foods at 41 degrees or below. Time is being used for some items, but items are not marked with the time. Dishwasher not washing hands between dirty and clean dishes.

Noncritical violations: Shrimp thawing in mop sink.

Comfort Suites, 2011 S.E. Walton Blvd., Bentonville

Noncritical violations: Maintain eggs, yogurt and cream cheese at 41 degrees or below.

John Q. Hammons Center, 3201 Pinnacle Hills Parkway, Rogers

Noncritical violations: Condensation leak in freezer. Hot water not working at handwashing sink. Water turned off at concession stand. Not in use at this time. Paper towel dispensers are not working properly.

Tortilleria Palominos, 1803 S. Eighth St., Rogers

Critical violations: Keep cold food at 41 degrees or below. Adjust refrigerator to make it colder. Cool foods properly. Sanitizer is not strong enough. Should be 50 to 100 ppm. No soap at handwashing sink.

Dec. 4

Red Robin Gourmet Burgers, 4300 W. Walnut St., Rogers

Noncritical violations: Dishwasher tested 0 ppm. Boiled eggs are 46 degrees. Cold holding is 41 degrees or below. Employees must wash hands after talking on cell phone and touching face and hair before returning to food prep. Small plastic portion cups being used for scoops. Must use a scoop with a handle. Employee involved in food prep may not wear jewelry. Bottle mixture unlabeled in bar area. Growth on bottle.

Across the Creek, 19149 Arkansas 127, Garfield

Critical violations: Cold water was turned off at the handwashing sink because of a leak.

Noncritical violations: Bowls stored in bulk items. Utensils should have a handle stored up out of food. Crab legs in one cooler were not cold enough. No ice was on this one batch.

The Bus Stop Grill, 18395 Marshall, Garfield

Critical violations: No cold water at handwashing sink. Meat slicer must be cleaned and sanitized. Sanitizer at three compartment sink was too strong. Clean the very top of the shake machine.

Noncritical violations: Shelf lining is not approved. Not smooth and easily cleanable. Clean under grill more frequently. Clean excessive grease on deep fryer. Do not store cups in bulk items.

Korner Kafe, 20204 U.S. 62, Gateway

Critical violations: Raw hamburger stored above ready to eat foods. Prep table not keeping foods at 41 degrees or below.

Noncritical violations: Store scoops handle up.

Ashmore’s Creek Flea Market, 18701 U.S. 62, Garfield

Critical violations: Do not leave food at room temperature. Automatic dishwasher is not working. All dishes and equipment must be sanitized.

Noncritical violations: Duct tape is not an approved repair material.

The Wright Stop, 17047 Marshall, Garfield

Critical violations: Ammonia should be 200 ppm.

Noncritical violations: Replace weather stripping on back door.

Dec. 7

C’ville Market, 2401 W. Olive St., Rogers

Noncritical violations: Cole slaw is 46 degrees. Cold holding is 41 degrees or below. Display cooler not maintaining food at 41 degrees or below. Food made in other facilities needs to have a label.

Pupuseria Salvadorena, 108 21st, Rogers

Noncritical violations: Tamales are 75 degrees. Cole slaw is 72 degrees. Shrimp is 65 degrees. Cold holding is 41 degrees or below. No soap at hand washing sink in kitchen. No soap or paper towels in kitchen. No soap or paper towels in bathroom. Not washing hands with soap. Not sanitizing dishes. May not reserve bulk salsa and cole slaw from guest to guest. Flour for papusas next to open trash can. Needs to be kept with food. Need to keep food in temperature, cold holding and hot holding. All employees need to know proper hand washing and sanitation.

Mary’s Little Lamb, 105 Moberly Lane, Bentonville

Noncritical violations: Raw eggs on top shelf.

Carrabba’s Italian Grill, 3300 Pinnacle Hills Parkway, Rogers

Critical violations: Employees must wash hands prior to putting on gloves. Bar cooler is not holding milk at 41 degrees or below.

Noncritical violations: Need thermometer in bar cooler. Clean interior of bar cooler.

Bonefish Grill, 3201 Market St., Rogers

Critical violations: Ammonia should be 200 ppm. Do not mix chemicals. Employees must wash hands prior to putting on clean gloves.

Wagon Wheel Country Cafe, 4080 N. Thompson St., Springdale

Critical violations: Commercial bleach solution was more than 200 ppm. Employee drink on shelf above crock pot of beans in food prep area did not have a lid or straw.

Las Fajitas, 5266 N. Thompson St., Springdale

Critical violations: Raw beef was 47 degrees. Raw chicken was 45 degrees on cold prep table. Must be kept at 41 degrees or below. Cooked chicken was 46 degrees. Raw beef was 46 degrees. Milk was 45 degrees. Must be kept at 41 degrees or below. All spray bottles must be labeled indicating contents. Sanitizer solution in spray bottle was over 200 ppm for bleach.

Creative Kids Day Care, 3184 N. Thompson St., Springdale

Critical violations: Bleach sanitizer bottles, concentration was 0 ppm.

Dec. 8

Daylight Donuts, 10 Cunningham Corner, Bella Vista

Noncritical violations: Sausage roll is 94 degrees. Jalapeno sausage roll is 94 degrees. Hot holding is 135 degrees or above. Employees must wear hair restraints when involved in food prep. Employee must wash hands after assisting customers. Pans put up without washing hands are no longer clean and sanitized. Must wash, rinse and sanitize drinking cups.

Town Place Suites, 3100 S.E. 14th St., Bentonville

Noncritical violations: Milk is 48 degrees. Cold holding is 41 degrees or below. Food in customer self service needs to be protected. Tongs need to be stored handle up in container.

On The Border, 577 N. 46th St., Rogers

Noncritical violations: Light shields needed in dishwasher area. Hand towels needed in bar area.

Malco Theatre, 621 N. 46th St., Rogers

Critical violations: No soap at back handwashing sink. No bleach at three compartment sink. Bleach is what establishment uses to sanitize utensils. Establishment has no way to sanitize at this time.

Noncritical violations: Clean behind shelves in popcorn store room periodically.

Mc. B’s Steakhouse, 1043 W. Walnut St., Rogers

Critical violations: Ammonia at mop sink is not strong enough. Should be 200 ppm.

Noncritical violations: Do not reuse single use containers for food storage. Quat test strips appear to be getting old and are not as accurate. Clean interior of ice machine around drop shield.

Dec. 9

Quality Inn and Suites, 3609 Moberly Lane, Bentonville

Noncritical violations: Eggs are 50 degrees. Cold holding is 41 degrees or below. No soap or paper towels at hand washing sink. Sanitation solution tested over 200 ppm. Fruit cocktail in customer self service needs to be protected. Tongs on breakfast bar need to be protected. Management needs to be aware of proper sanitation strength and procedures.

Veterans of Foreign Wars, Post 3031, 11160 N. Old Wire Road, Rogers

Noncritical violations: Light shield needed above dishwasher.

Jake’s BBQ, 120 W. Rose, Rogers

Noncritical violations: Single serve Styrofoam containers should be six inches off floor. Eggs should not be stored above other ready to eat foods. Hand towels needed at hand wash sink. Hair restraints needed for food prep employees.

Papa Mikes at the Highlands, 500 Glasgow, Bella Vista

Critical violations: Sausage and raw eggs sitting out at room temperature. Potato slicer and meat slicer have not been cleaned or sanitized properly. Cool foods properly prior to placing in cold holding.

Noncritical violations: No paper towels at either hand washing sink. Foods uncovered in walk in and freezer. One pan was placed on top of another uncovered pan of food. Clean interior of ice machine.

Concordia Retirement, 1 Concordia Drive, Bella Vista

Critical violations: Raw eggs stored above milk in milk cooler. Dishwasher must wash hands between dirty and clean dishes. Maintain dressings on salad bar at 41 degrees or below.

Noncritical violations: Condensation leak in freezer Do not store open food under leak until repaired. Salad cooler does not have a working thermometer.

Duffer’s Cafe, 638 Lancashire Blvd., Bella Vista

Critical violations: Chemical spray bottles not labeled with contents.

Noncritical violations: Ice to keep foods cold must completely surround food to be effective. Women’s restroom door does not close tightly by itself. Wiping cloths stored in sanitizer that was too strong.
